    Description / Table of Contents: Zusammenfassung Etwa 4–6 Tage nach intracerebraler, intraperitonealer oder subcutaner Inoculation einer Suspension von Arbo-Virus (Typ: Mucambo BeAn 10967) kommt es zu einer Stase im Capillarsystem der Niere, zu einer Obliteration des Bowmanschen Kapselraums durch eine Schwellung der Deckzellen und durch Herniation von geschwollenen Hauptstückzellen in den Kapselraum. Die Schwellung des Hauptstückepithels geht z.T. in Nekrosen von Tubulusepithel über, wobei Cylinder aus Zell- und Kerndetritus entstehen. Gleichzeitig bilden sich Nierenveränderungen im Sinne einer chromoproteinurischen Nephrose aus. Ein Erregernachweis gelingt in der Niere nicht. Die Nierenbefunde werden als Folge der renalen Intoxikation mit Schädigung der Blutcapillarwand und einer Zirkulationsstörung bei der vorher und gleichzeitig ablaufenden virogenen Hepatitis und Encephalitis angesehen.
    Notes: Summary Four to six days following intracerebral, intraperitoneal or subcutaneous inoculation of Mucambo-Virus one can observe a sludge of the capillary system in the kidney, obliteration of the Bowman space by swollen podocytes and herniation of swollen tubular epithelium into the capsule space. The swelling of the epithelium of the proximal tubules leads to necrotic ruptures of tubular epithelium with formation of cylinders of cellular and nuclear debris. In addition, a chromoproteinuric nephrosis can be found. Virus particles can not be demonstrated in kidney tissue cells. The renal alterations are thought to be caused by renal intoxication with damage of capillary walls and circulatory disturbance due to the viral hepatitis and encephalitis.

    Notes: Abstract A 9.5-year-old girl had popliteal arterial and venous compression by a distal femoral osteochondroma. Magnetic resonance imaging demonstrated the relation of the vessels to the osteochondroma and a three-phase bone scintigram showed asymmetry of arterial perfusion and evidence of venous stasis.

    Notes: Abstract The majority of children with traumatic injuries of the brachial plexus recover with only minor sequelae. Some patients develop residual deformity, however. This deformity may consist only of internal rotation contractures, but in some patients subluxation of the humeral head may be present. CT is useful in the evaluation of: the presence or absence of subluxation, deformity of the glenoid and the proper placement of the humeral head in plaster or orthotic devices.

    Notes: Abstract The objective of this study was to assess five cacao cultivars (selfs) and 20 hybrids with regard to their general-and specific-combining ability for yield components using method 1, model I, of the diallel analysis system. The selfings and the hybrids were obtained through controlled crossings, tested in the field in a random block design with four replications and plots containing 16 plants. The experiment was set up in the Centro de Pesquisas do Cacau, in Itabuna, Bahia, Brasil, in 1975. The characteristics studied were: the number of healthy and collected fruits per plant (NHFP and NCFP), the weight of humid seeds per plant and per fruit (WHSP and WHSF), and the percentage of diseased fruits per plant (PDFP), for 5 years (1986–1990). The F-test values, highly significant for general combining ability (GCA) and specific combining ability (SCA), demonstrated the existence of variability for both effects. However, the effects of SCA were greater than those of GCA, when compared in terms of the average squared effects. This condition held for the characteristics NHFP, NCFP and WHSP, which shows the relative importance of the non-additive genetic effects over the additive effects. The reciprocal effects did not show significance. Breeding methods which explore the additive portion of genetic variance should be employed for obtaining higher-yielding cacao and high seed weight. For this, the segregant populations should involve cultivars CEPEC 1, SIAL 169 and ICS 1. Combinations involving the cultivar ICS 1 presented the most favorable results for the characteristics WHSP and WHSF, where the hybrid SIAL 169 x ICS 1 and its reciprocal were outstanding.

    Notes: Abstract Hepatitis C virus (HCV) exhibits a dramatic genetic variability and several mechanisms of immunological response are unable to control hepatic and extrahepatic replication. Genotype 1 b is associated with more severe clinical manifestations and is less responsive to interferon. In addition, we have reported an increase of HCV RNA viral load after renal transplantation. Anti-thymocyte globulin (ATG) is supposed to increase viral replication and liver dysfunction in chronically infected renal graft recipients. We evaluated the genotype profile in HCV + patients of our Renal Transplant Unit and studied the effects of ATG, as part of the induction of immunosuppression, on viral load and liver enzymes abnormalities. From 726 renal graft recipients, 104 patients, with a mean follow up of 3.9 ± 2.9 years, were anti-HCV + by ELISA II. HCV RNA was measured by quantitative PCR. We correlated the viral load and biochemical liver parameters with genotype, exposure to ATG as induction therapy, early acute rejection episode and the duration of infection. Of the 81 patients tested, 72 % were viraemic and genotype 1 b was the predominant viral strain (66 %). The majority of these patients (65 %) were coinfected by two or more strains.There was no correlation between HCV RNA blood levels and liver enzymes. We did not find higher viral load with genotype 1 b infection (68 ± 88 mEq/ml vs 75.8 ± 123 mEq/ml in the others) nor with ATG induction therapy (43.5 ± 71.3 mEq/ml vs 64.1 ± 110.5 mEq/ml). Early acute rejection and longer follow up were not associated with higher levels of HCV RNA. The biochemical liver profile showed no relationship with the variables studied. We concluded that genotype 1 b is the predominant strain in our HCV + population and there is a great prevalence of coinfection with several genotypes. Our results did not confirm a deleterious effect of the use of ATG as induction therapy in these HCV-infected patients. Prospective randomised studies with liver biopsy evaluation are needed to answer more fully the remaining questions about the best immunosuppressive therapy in renal graft recipients with chronic HCV infection.

    Notes: Abstract Nuclear medicine provides several methods for increasing the accuracy of surgical removal of bone lesions with focally increased uptake. In this paper, three intraoperative procedures are discussed: remote control by imaging, intraoperative control by imaging, and intraoperative control by scintillation probe. All techniques require preoperative injection of bone imaging tracer. Remote operative control calls for a gamma camera to mark the skin over the lesion prior to surgery, providing optimal preoperative localization and imaging of the excised lesion to ensure complete removal. Intraoperative control procedures require that a portable camera or a scintillation probe be used in the operating room; these permit direct monitoring of localization and resection. Our experience with 18 procedures performed on 15 patients suggests that these techniques are worthy of continued use.

    Notes: Abstract  A hospital-based matched case-control study was conducted in order to identify risk factors for the development of bloodstream infections in adult hospitalized patients. Between January 1993 and December 1994, 264 episodes of bloodstream infection were evaluated. Significant variables identified by univariate analysis were included in a multivariate model that showed that central venous catheter [odds ratio (OR), 6.71], poor performance status (OR, 3.40), weight loss (OR, 2.47), hematologic diseases (OR, 2.24), and previous antimicrobial therapy (OR, 2.12) independently influenced the outcome. The knowledge of modifiable risk factors is useful in the development of strategies that may contribute to the prevention of bloodstream infections.
