    Notes: To explore the influence of social support and knowledge of infant development on maternal confidence in performing infant care tasks among adolescents and adults.〈section xml:id="abs1-2"〉〈title type="main"〉Design: Descriptive, exploratory.〈section xml:id="abs1-3"〉〈title type="main"〉Setting: A large urban teaching hospital in the midwestern United States.〈section xml:id="abs1-4"〉〈title type="main"〉Participants: 116 adolescent mothers (ages 13 to 19 years); 101 adult mothers (ages 20 to 41 years).〈section xml:id="abs1-5"〉〈title type="main"〉Main Outcome Measures: Adult mothers reported higher levels of knowledge about infant development than did adolescent mothers. For both adolescent and adult mothers, social support and knowledge of infant development were significantly correlated with confidence in providing infant care. For adolescents, knowledge of infant development explained 15% of the variance in maternal confidence scores, and the number of people in the adolescent's household explained an additional 5%. For adults, parity explained 10%, social support explained 9%, and knowledge of infant development explained 4% of the variance in maternal confidence scores.〈section xml:id="abs1-6"〉〈title type="main"〉Conclusions: An adolescent's knowledge of infant development significantly affects her confidence in providing infant care. Given the learning needs of new mothers and the time constraints for in-hospital postpartum teaching, nurses may need to adjust content and teaching methods for all new mothers, but especially to meet the needs of adolescent mothers.

    Notes: Objective: To examine how expert perinatal nurses in a nurse-managed labor model view their role in caring for mothers during labor and birth. Design: Focus group methodology. Data were analyzed using inductive coding methods to gain understanding from the perspective of those providing the care. Setting: Labor and birth units in four large Midwestern medical centers. Participants: Fifty-four expert labor nurses. Inclusion criteria: 5 years experience in nursing care during labor and birth in institutions where nurse-managed labor was the predominant practice model. Results: Four common themes related to nursing roles were identified. These included knowing the labor process and the intuitive nature of nursing care provided by expert labor nurses based on years of experience, knowing the woman and letting her body guide labor, advocacy for laboring woman, and the autonomous nature of the nurse-managed labor model. Conclusions: Expert labor nurses developed a keen sense of intuitive knowledge based on their years of experience. They reported using hands-on high-touch supportive care techniques with the potential to affect labor and birth outcomes. Autonomy is perceived as a key component of practice within the nurse-managed labor model.

    Description / Table of Contents: Sommario Sono stati confrontati i livelli sierici di Mioglobina (MGB) determinata radioimmunologicamente, dei seguenti tre Gruppi: il Gruppo A comprendente 1 portatrice definita di Distrofia muscolare di Duchenne (DMD), 3 portatrici probabili e 17 portatrici possibili risultate positive ai tests per il depistaggio del gene della DMD; il Gruppo B comprendente 23 portatrici possibili di DMD risultate negative ai tests ed il Gruppo dei Controlli comprendente 26 soggetti normali. È risultata statisticamente molto significativa la differenza dei valori di MGB del Gruppo A in confronto ai Controlli, ed anche del Gruppo A in confronto al Gruppo B. Sono state inoltre valutate le correlazioni fra i valori di MGB e quelli del CK nei medesimi soggetti, che sono risultate significative, e quelle fra MGB ed età, che non sono risultate significative. Viene infine proposto l'inserimento del dosaggio radioimmunologico della MGB fra i tests per depistare le portatrici sane di DMD.
    Notes: Abstract 1 case of Duchenne Muscular Dystrophy (DMD) definite carrier, 3 probable and 17 possible (all analytically positive: “possible +”) carriers were classed as Group A, 23 DMD possible carriers (all analytically negative “possible”) as Group B and 26 normal subjects as Controls. The three groups were subjected to serum myoglobin (MGB) radioimmunoassay. Since the differences between MGB levels of Group A vs. Controls and Group A vs. Group B were statistically significant, it is suggested that the determination of MGB should be included among the tests for DMD carrier detection.
