    Notes: One hundred and twenty-eight cases of high-grade malignant B-cell lymphoma were studied with a plastic re-embedding technique and classified according to the Kiel classification. The cytological details could be better recognized than in the original paraffin sections, thus permitting a more precise definition of the various lymphoma types. The entities centroblastic and immunoblastic lymphoma are more precisely defined and supplemented by the addition of several new variants. In contrast to the present Kiel classification we separate Burkitt's from lymphoblastic lymphoma. In all cases investigated, the B-cell nature of the tumour cells was proven by immunohis-tochemistry using monoclonal antibodies. The four entities of high-grade malignant B-cell lymphoma described in this paper are: (1) centroblastic lymphoma with four morphological variants (monomorphic, polymorphic, multilobated and centrocytoid); (2) immunoblastic lymphoma with three morphological variants (with or without plasmacytic differentiation, with many lymphocytes); (3) Burkitt's lymphoma and the closely related Burkitt's lymphoma-like lymphoma with plasmablastic differentiation; and (4) lymphoblastic lymphoma. Only the centroblastic lymphomas (in 17%) showed occasional follicular growth pattern, which further confirms the view that they are derived from germinal centre cells.

    Notes: Three cases of histiocytic necrotizing lymphadenitis are reported. Two patients came from Vietnam and the third from Greece. In all cases there was infection with Yersinia enterocolitica of serogroup 9 or 3. Cervical lymph nodes were examined. Histologically, the characteristic necrosis developed in large foci of so-called T-associated plasma cells. Immunological analysis showed that these cells have characteristic markers of helper/inducer T cells, but do not express sheep erythrocyte receptors. The T-associated plasma cells perished by pyknosis and were then phagocytosed and digested by macrophages, which were present in large numbers. The necrotic areas were exclusively located in hyperplastic T regions. The B-cell system did not play a role in the reaction. T-associated plasma cells have been renamed ‘plasmacytoid T cells’ because they contain abundant rough endo-plasmic reticulum (‘plasmacytoid’) and show immunological features of T cells. It appears likely that plasmacytoid T cells are the counterparts of plasma cells of the B-cell system that secrete lymphokines instead of immunoglobulin.

    Notes: Twenty-nine cases of non-Hodgkin's lymphoma of low-grade malignancy in a European population were investigated for the presence of bcl-2 and bcl-1 gene rearrangement. The cases were classified according to the Kiel classification. It was shown that bcl-2 gene rearrangements were exclusively confined to centroblastic-centrocytic lymphomas. bcl-1 rearrangements were found in two cases of chronic lymphocytic leukaemia. As the chromosomal translocation t(14;18) is reported to occur in up to 85% of follicular lymphomas, our results provide additional evidence that the differentiation of low-grade B-cell lymphomas according to the Kiel classification defines biologically distinct entities.

    Notes: Twenty-eight cases of monocytoid B-cell lymphoma of lymph nodes and 16 lymph node metastases of primary gastric lymphomas, mostly low-grade B-cell lymphomas of mucosa-associated lymphoid tissue (MALT) type were investigated morphologically and immunohistochemically. Both groups showed the same morphological and immunohistochemical features: diagnostically important sites of infiltration were the sinuses and the marginal zones, The tumour cells were either medium-sized or small. The cytoplasm stained grey with Giemsa and was sometimes rather pale. In imprints the grey colour of the cytoplasm was a characteristic feature. The medium-sized cell type was more frequent; in one third of the cases it was combined with a prominent lymphoplasmacytic component from the same clone, and it resembled the monocytoid B-cells of the sinuses. The small cell type was less common, was not combined with a lymphoplasmacytic component and more closely resembled marginal zone cells. The difference was underlined by the negative reaction with the monoclonal antibody Ki-B3 in the small cell type, which, conversely, was positive in the medium-sized cell type and in the monocytoid B-cell reaction of the sinuses. Both of these cell types, however, showed a granular reaction with the new monoclonal antibody Ki-Mlp. The morphological and immunohistochemical parallels are arguments in favour of the assumption that monocytoid B-cell lymphoma is the nodal equivalent of low-grade B-cell lymphoma of MALT type. This is further supported by the fact that in nine of our 28 cases of monocytoid B-cell lymphoma, lymphomas were found simultaneously or subsequently in organs of the MALT. Monocytoid B-cell lymphoma must be differentiated from an infiltration that occurs in the form of clusters of monocytoid B-cells in other low-grade B-cell lymphomas, especially in immunocytoma with a high content of epithelioid cells.

    Notes: Ten cases of classic centrocytic lymphoma as defined in the Kiel classification system were investigated for their immunophenotype, their proliferation activity and by means of molecular diagnostics. The findings were compared to those obtained from a group of nine cases of anaplastic centrocytic lymphoma. Both groups showed virtually identical immunohistochemical characteristics with positivity for CD5 and negativity for CD10 and CD23. In the group of anaplastic centrocytic lymphoma, there were considerably higher proliferation indices as documented by staining for the Ki-67 antigen, up to 80% of the tumour cells being positive. Moreover, the cases of anaplastic centrocytic lymphoma had bcl-1 gene rearrangements in eight out of nine cases compared with three out of 10 cases of classic centrocytic lymphoma. DNA analysis was not able to detect bcl-2 gene rearrangement in any case, pointing to a difference compared with lymphomas of germinal centre origin. The coincidence of anaplastic and sometimes blast-like morphology of the tumour cells, high proliferation index and a rearranged bcl-1 gene in nearly all cases of anaplastic centrocytic lymphoma support their classification as high-grade malignant variants of centrocytic lymphoma and suggest a possible role for the bcl-1 locus not only in the origin but also in the progression of centrocytic lymphomas.

    Notes: Large cell anaplastic lymphoma (LCAL) is a recently described entity recognized by its constant expression of the CD30 antigen and by its morphology. LCAL is classified as high grade malignant lymphoma. We report 10 patients with LCAL of the skin, three with primary LCAL and others in which it developed with cutaneous T-cell lymphoma, mainly mycosis fungoides. The immunophenotype in all but one of these cases, was that of activated T cells.

    Description / Table of Contents: Abstract We present a case of a 48 year old man who complained of progressive dyspnoe. Radiograph revealed a large tumor in the left lung. Biopsy was performed and revealed beneath a normal bronchial epithelium polymorph tumor cells admixed with some histiocytes. These tumor cells were strongly pigmented and expressed HMB-45 and protein S-100 immunohistochemically. Malignant melanoma of the bronchus was included in the differential diagnosis. A thoracotomy and left lower lobe resection was performed. Histology revealed an angiomyolipoma. The lung is an uncommon localisation of angiomyolipoma with so far three reported cases. Lipoma, leiomyoma and lymphangioleiomyomatosis have to be included in the differential diagnosis. The peculiarity of this case is the presence of multiple pigmented tumor cells that express HMB-45. This has not been reported yet.
    Notes: Zusammenfassung Es wird der Fall eines 48jährigen Patienten vorgestellt, der über zunehmende Dyspnoe klagt. Das im Rahmen der Diagnostik angefertigte Röntgenbild zeigte eine umschriebene maximal 9,5 cm durchmessende Raumforderung im Bereich des linken Lungenunterlappens. Es wurde eine transbronchiale Biopsie durchgeführt, die viele pigmentierte polymorphe melanozytäre Zellen aufwies. Es wurde mit Vorbehalt der Verdacht auf ein primäres bronchiales malignes Melanom gestellt. Die anschließend durchgeführte Thorakotomie und Lungenunterlappenresektion zeigte nach histologischer Aufarbeitung ein Angiomyolipom der Lunge. Eine tuberöse Sklerose des Patienten, welche häufig mit Angiomyolipomen assoziiert ist, konnte ausgeschlossen werden. Die Lunge stellt einen ungewöhnlichen Manifestationsort für ein Angiomyolipom dar, mit derzeit nur drei publizierten Fällen. Lipome, Leiomyome sowie eine Lymphangioleiomyomatose müssen differentialdiagnostisch abgegrenzt werden. Dieser Fall weist im Gegensatz zu den bisher publizierten die Besonderheit auf, daß viele pigmentierte melanozytäre Zellen (HMB-45 und S-100 positiv) innnerhalb des Tumors vorkommen.
