    Notes: Abstract A 6-year-old girl is described who died following rapid neurological deterioration, ending in lethal cerebral oedema. Despite the absence of severe intestinal and metabolic derangement, Shigella was cultured from the stool. Toxic encephalopathy is responsible for death following this rare complication of childhood shigellosis in developed countries. The pathophysiology is unknown. Conclusion Lethal toxic encephalopathy can be caused by Shigella despite the absence of severe intestinal and metabolic derangement. If shigelllosis is suspected, headache may be a first significant sign for the development of toxic encephalopathy. Early recognition and rapid measures to prevent brain oedema may improve outcome.

    Description / Table of Contents: Résumé Le manque de connaissance élémentaire et clinique au sujet de la pathogénèse d'infections d'implants orthopédiques, en particulier par suite de bactérémie, a motivé la présente étude, où nous avons développé un modèle animal, qui nous a permis d'étudier les infections hématogènes d'implants sans ciment. Nous avons soumis dix lapins à une intervention chirurgicale comportant deux étapes. Un implant cylindrique en titane ou un implant en titane recouvert d'hydroxyapatite fut inséré dans le tibia droit. Après un délai d'au moins quatre semaines, les implants furent sélectivement contaminés avec des doses variables deS. aureus, par l'artére fémorale droite. Pendant la période postopératoire, les animaux furent examinés tous les jours, avec bilans sanguins. Au bout d'au moins une semaine, les animaux furent mis à mort, et des biopsies d'os, de moelle, de foie, de rate, de poumon et de rein furent cultivées. L'implant, ensemble avec l'os qui l'entourait, fut préparé pour examen histologique. L'injection de 5 × 108 cfu a provoqué des cultures positives dans tous les cas, et des réactions systémiques minimales. Dans les coupes histologiques, nous avons trouvé des bactéries et des signes d'une inflammation focale légère. Ce modéle animal est utile pour des études plus amples d'aspects d'interface, de traitement et de prévention d'infections hématogènes d'implants.
    Notes: Abstract There is a lack in basic and clinical knowledge on the pathogenesis of orthopaedic implant infections, particularly as a sequel of a bacteremia. In the present study an experimental animal model was developed, that allows us to study haematogenous infections of cementless implants. Ten New Zealand White rabbits were subjected to a two-stage surgical procedure. A cylindrical Ti6Al4V or hydroxyapatite coated Ti6Al4V implant was inserted into the right tibia. After a period of at least 4 weeks, the implants were selectively contaminated with varying doses of Staphylococcus aureus, via the right femoral artery. During the postoperative period, the animals were examined daily and blood samples were taken. After at least i week, the animals were sacrificed and biopsies of bone, marrow, liver, spleen, lung and kidney were cultured. The remaining implant with surrounding bone was prepared for histological examination. Injection of 5 × 108 cfu causes positive cultures in all cases and minimal systemic reactions. On the histological sections, incidental bacteria and signs of a mild, focal inflammation were seen. This animal model is useful for further studies of interface, treatment and prevention aspects of haematogenous implant infections.

    Notes: Abstract Two strains of Staphylococcus epidermidis isolated from patients with toxic shock symptoms have been reported to carry genes related to S. aureus enterotoxins B and C by dot-blot hybridisation, although the corresponding superantigenic toxins were not detected immunologically. We here show that these strains produce no superantigens capable of stimulating proliferation of human mononuclear leukocytes or rabbit splenocytes, and that no DNA homologous to the seb or sec genes can be detected by PCR. However, stimulation of human monocytes by whole killed bacteria induced dose-dependent production of the cytokines TNFα, IL-1 β and IL-6, which may be responsible for the clinical symptoms in these patients.

    Notes: The purpose of this study was to characterize the destruction of sensitized erythrocytes by human blood monocytes in vitro The incubation in vitro of human monocytes with 51Cr-lalbelled human erythrocytes sensitized with IgG rhesus alloantibodies anti-D EAIgG anti-D) resulted in release of51Cr from the erythrocytes (lysis) as well as uptake of 51Cr-labelled erythrocytes by the monocytes (phagocytosis). The lysis of EAIgG anti-D by monocytes was not dependent on phagocytosis, because cytochalasin B, which inhibited phagocytosis of EAIgG, enhanced lysis. In contrast, hydrocorlisone and colchicine inhibited lysis, but had no effect on phagocytosis. These agents did not affect binding of EAIgG anti-D to monocytes. The effect of these agents on lysis corresponded to their effect on release of lysosomal enzymes by monocytes. The release of lysosomal enzymes, when induced by EAIgG anti-D, was, likewise, enhanced by cytoehalasin B and inhibited by hydrocortisone and colchicine. A significant correlation was found between lysosomal enzyme release and lysis. Together, these results strongly suggest that lysosomal enzymes. released by the monocytes when incubated with anti-D-sensitized erythroeytes, are responsible for the cytotoxic activity of these cells towards sensitized erythrocytes. The action of these enzymes only occurs over a short range, probably at the site of attachment of the erythrocyte, becuuse only erythrocytes that were bound to the monocytes were lysed. The finding of other investigators that removal of monocytes from suspensions of human mononuclear leucocytes results in a strong reduction in the cytotoxic activity of these leucocytes towards sensitized erythrocytes in vitro. was confirmed.

    Notes: Abstract Cytokines play a major role in the pathophysiology of septic shock. In this study, human periferal blood monocytes were stimulated with peptidoglycan and teichoic acid, purified from a strain of Staphylococcus epidermidis. Polymyxin B (PM-B) was added to avoid the effects of possible contamination with endotoxin. Tumour necrosis factor-α (TNF), interleukin-1β (IL-1), and interleukin-6 (IL-6) in the supernates were measured by enzyme-linked immunosorbent assays. Peptidoglycan and teichoic acid induced TNF, IL-1, and IL-6 in a concentration-dependent manner. Teichoic acid was a weaker inducer than peptidoglycan, especially for IL-1. Lipopolysaccharide from an E. coli strain was used as a control, being 100–1000 times more potent than peptidoglycan and teichoic acid.

    Notes: Abstract Tobramycin release from tobramycin-containing bone cement was studied in vivo in a rabbit model. After insertion of cement into the right femur, tobramycin concentration as a function of time for up to 28 days was measured in serum and bone of rabbits. Tobramycin release in the femoral cortex adjacent to the cement was found to exceed the minimal inhibitory concentration for Staphylococcus aureus Wood 46. Serum tobramycin concentrations were below the systemic toxicity threshold.
