    Notes: Summary Patients with diabetes mellitus are at increased risk of developing gallstones. This has been attributed, among other factors, to alterations in gallbladder motility in the presence of autonomic neuropathy. Since high blood glucose concentrations impair gastric emptying in diabetic patients, we have investigated the effect of acute hyperglycaemia on gallbladder motility. Seven Type 1 (insulin-dependent) diabetic patients were studied twice during euglycaemia (blood glucose 5 mmol/l) and hyperglycaemia (blood glucose 15 mmol/l) using a clamp technique. In addition, seven healthy volunteers were studied during euglycaemia and hyperglycaemia. Gallbladder volumes, measured with ultrasonography, were studied before and during infusion of step-wise increasing doses of cholecystokinin-33, 0.25, 0.5 and 1.0 Ivy Dog Unit·kg−1·h−1, each dose for 30 min. Mean basal gallbladder volumes were not significantly different in the four experiments. Administration of cholecystokinin resulted in significant (p 〈0.05) dose-dependent reductions in gallbladder volume in all experiments. During euglycaemia the gallbladder contraction in diabetic patients was not significantly different from the control subjects. During hyperglycaemia the gallbladder contraction in the diabetic patients was significantly (p 〈0.05) reduced compared to euglycaemia only during infusion of 0.25 Ivy Dog Unit·kg−1·h−1 of cholecystokinin (19±6 % vs 33±6 %). Compared to euglycaemia, during hyperglycaemia the gallbladder contraction in the control subjects was significantly (p 〈0.05) reduced during infusion of 0.25, 0.5 and 1.0 Ivy Dog Unit·kg−1·h−1 of cholecystokinin (14±4 % vs 31±3 %; 42±6 % vs 65±5 %; 74±4 % vs 90±3 %, respectively). It is concluded that during euglycaemia the gallbladder contraction in response to cholecystokinin in Type 1 diabetic patients is not significantly different from control subjects. During hyperglycaemia the gallbladder contraction in response to 0.25 Ivy Dog Unit·kg−1·h−1 cholecystokinin, leading to cholecystokinin levels as observed after ingestion of a light meal, is significantly reduced in Type 1 diabetic patients.     Description / Table of Contents: Zusammenfassung Die Tiefenpenetration der Spermien in das Cervikalsekret der Frauen, die Ovanon und Sequilar einnahmen, wurde in vitro täglich für den ganzen Einnahmezyklus unter homogenen Versuchsbedingungen verfolgt. Dabei ergaben sich große Unterschiede für den Raumgewinn im Cervikalschleim der Frauen während eines normalen Zyklus und unter der Einnahme der Zweiphasen-Präparate. Unter Sequilar und Ovanon war der Raumgewinn geringer als im normalen Zyklus. Das Sequilar gleicht im ganzen Einnahmezyklus mit der starken Cervixbarriere für die Samenfäden in diesem Bereich fast einem Einphasen-Präparat. Das Ovanon erlaubt in der ersten Phase eine Tiefenpenetration, die geringer als im normalen Zyklus aber stärker als unter der Sequilareinnahme ist. In der zweiten Phase besteht zwischen beiden Präparaten für die Spermatiefenpenetration kein markanter Unterschied. Die sich daraus ergebenden hypothetischen Daten für das Problem des Cervixkarzinoms werden beschrieben.
    Notes: Summary The in vitro penetrability of cervical mucus by sperm was investigated in women who were taking Ovanon or Sequilar. Measurements were made under constant experimental conditions on every day of the cycle. The depth of penetration differed strikingly between women with normal menstrual cycles and women taking the sequential hormonal contraceptives. The penetration depth was substantially less when Sequilar or Ovanon was used. The Sequilar induced cervical sperm barrier during the entire cycle was almost as strong as that observed when combined preparations are used. During the proliferative phase of the cycle the penetration depth allowed by Ovanon was less than that observed during normal cycles but greater than that seen with Sequilar. During the secretion phase no appreciable difference in the penetrability of cervical mucus between the two contraceptives was observed. The hypothetical relationship between these data and the problem of cervical carcinoma was described.

    Notes: Summary The instability of atlanto-axial subluxation remains a challenging problem in patients with rheumatoid arthritis. In order to preserve as much function of the cervical spine as possible, inclusion of the occiput into the fusion should exclusively be performed when there is a radiologically or clinically manifest pathological condition of the atlanto-occipital joint or marked upward migration of the dens axis. In order to prevent irreversible damage to the spinal cord, an early indication of surgical stabilization is recommended. This article presents a retrospective analysis of the clinical and radiological results of occipito-cervical fusion in 26 patients with rheumatoid arthritis using a modified Brattström technique. The complications encountered were mainly due to the use of wire fixation, reinforcement using bone cement and insufficient reduction of atlanto-axial subluxation.

    Description / Table of Contents: Summary The need for an objective assessment of voice quality can be seen in the increasing use of acoustic analysis methods for clinical diagnosis and research. The hoarseness diagram allows clinicians to objectively describe even highly disturbed or aphonic voices. Its application possibilities are illustrated in three case studies in which changes in voice quality were monitored during voice rehabilitation. The distributions of voice groups that were defined on the basis of specific pathophysiological phonation conditions were then compared for their acoustic differences. The groups comprised various phonation conditions after the resections of laryngeal tumors and different types of laryngeal paralyses. Interpretation of the results suggests a direct correspondence of the hoarseness diagram coordinates and the irregularity of vibration on the one side and the degree of glottal closure on the other. This illustrates the potential usefulness of the hoarseness diagram in a clinical context.
    Notes: Zusammenfassung Die Bedeutung der objektivierbaren Stimmgütebeschreibung spiegelt sich im zunehmendem Einsatz akustischer Analsyemethoden in der klinischen Diagnostik und Forschung wider. Das Heiserkeits-Diagramm ermöglicht die akustische Stimmgütebeschreibung auch hochgradig gestörter Stimmen bis hin zur Aphonie. Die Objektivierbarkeit der Stimmqualitätsverbesserung bei postoperativer Stimmtherapie wird an 3 Fallbeispielen verdeutlicht. Des weiteren wird die Differenzierbarkeit pathophysiologisch definierter Gruppen basierend auf den Ergebnissen der akustischen Analyse untersucht. Dabei werden einerseits verschiedene Phonationsmechanismen nach Tumorresektion, andererseits verschiedene laryngeale Lähmungsformen einander gegenübergestellt. Die Ergebnisse führen zu der Interpretation, daß die Koordination des Heiserkeits-Diagramms direkt die Irregularität der Stimmlippenschwingung einerseits und die Qualität des glottalen Schlusses andererseits widerspiegeln. In diesem Zusammenhang ist auch der potentielle Nutzen des Heiserkeits-Diagramms in der klinischen Anwendung und Forschung zu sehen.

    Notes: Abstract With the Fréjus detector we studied four astrophysical point-like sources by using $$v_e (\bar v_e )$$ and $$v_\mu (\bar v_\mu )$$ interactions in the detector and $$v_\mu (\bar v_\mu )$$ interactions in the surrounding rock. No excess of events was found. Therefore upper limits of neutrino fluxes and source luminositics are quoted. These limits confirm results from other experiments. In addition new limits are presented for spectral indices above 3.
