    Notes: The new long-acting somatostatin analogue octreotide (SMS 201–995) was investigated for its influence on segretatagogue-stimulated human exocrine pancreatic secretion. Eighteen healthy volunteers participated in the study. During duodenal intubation with a background stimulation of either secretin 1 U.kg/h or secretin 1 U. kg/h + ceruletide, 120 ng.kg/h, octreotide was infused at doses of 5, 20 and 80 μg/h in a placebo-controlled randomized double-blind crossover trial. Duodenal juice samples were collected in 10-min intervals, and amylase, trypsin, chymotrypsin, and bicarbonate were measured in the individual fractions.During secretin stimulation, amylase was inhibited between 41 and 59%, trypsin between 28 and 72%, chymotrypsin between 55 and 70%, and bicarbonate between 0 and 31% with 5, 20 and 80 μg/h octreotide. During secretin and ceruletide stimulation, amylase was significantly inhibited by 84%, 78%, 81%, trypsin by 76%, 55%, 52%, chymotrypsin by 77%, 55%, 60%, and bicarbonate by 25%, 11%, 19% with 5, 20, and 80 μg/h octreotide, respectively (all decreases P 〈 0.05).The long-acting somatostatin analogue octreotide was confirmed to be a potent inhibitor of stimulated human exocrine pancreatic secretion. The near maximal inhibitory potency of octreotide was achieved at a dose of only 5 μg/h. This finding may be of value in the planning of therapeutic studies with octreotide.

    Description / Table of Contents: Zusammenfassung. Die duodenumerhaltende Pankreaskopfresektion wurde vor 25 Jahren von Beger entwickelt. Dieses Verfahren ist indiziert bei Patienten mit chronischem Schmerzsyndrom, entzündlichem Pankreaskopftumor und/oder Gallengangstenose, Pankreasgangstenose und Obstruktion der retropankreatischen Gefäße. 74 Patienten wurden zwischen 1993 und 1996 am Inselspital in Bern mit diesem Operationsverfahren chirurgisch versorgt. Die mittlere Operationszeit betrug 380 min bei einem medianen intraoperativen Transfusionsbedarf von 0 (0–6) Konserven. Postoperativ ist kein Patient verstorben. Die Gesamtmorbidität betrug 13 %. Ein Patient wurde am Tag 17 wegen Ileus reoperiert. Die mittlere Hospitalisationszeit betrug 11 Tage. Postoperativ trat bei 2 Patienten ein Diabetes neu auf. Die duodenumerhaltende Pankreaskopfresektion ist ein organerhaltendes chirurgisches Prinzip, welches die Komplikationen der Erkrankung behandelt und auch im Langzeitverlauf bei über 80 % der Patienten Schmerzfreiheit ermöglicht.
    Notes: Summary. Duodenum-preserving resection of the head of the pancreas was developed 25 years ago by Beger. This procedure is indicated in patients suffering from chronic pain in combination with inflammation of the head of the pancreas, common bile duct obstruction, pancreatic duct obstruction and/or obstruction of the retropancreatic vessels. At the Inselspital in Berne, 74 patients underwent this operation between 1993 and 1996. The median length of the operation was 380 min, with the need for transfusion in a median of 0 units (0–6). There was no postoperative mortality. Total postoperative morbidity was 13 %. One patient needed relaparotomy on day 17 for small bowel obstruction. Median length of hospital stay was 11 days. Postoperatively, two patients developed diabetes. Duodenum-preserving resection of the head of the pancreas represents an organ-preserving principle of surgery. This procedure treats the complications of chronic pancreatitis and provides long-term pain relief in more than 80 % of patients.

    Notes: Abstract Molecular alterations play a key role in the pathogenesis of gastrointestinal cancers. In the present paper we describe relevant molecular alterations in human pancreatic adenocarcinomas. Overexpression of growth factor receptors (EGF receptor, c-erbB2, c-erbB3, TGFβ receptor I–III), growth factors (EGF, TGFα, TGFβ-1-3, aFGF, bFGF), adhesion molecules (ICAM-1, ELAM-1) and gene mutations (p53, K-ras, DCC, APC) are present in a significant number of these tumors. These changes stimulate tumor growth and enhance the metastatic behavior of pancreatic cancer cells and thereby may contribute to shorter postoperative survival following tumor resection.

    Notes: Summary To study the source and role of circulating phospholipase A2 (PLA2) catalytic activity we monitored the serum from patients with necrotizing pancreatitis (n=8), diffuse peritonitis (n=6), and multiple injuries (n=11). Immunoreactive PLA2 serum protein concentration was analysed using a fluoroimmunoassay based on an antibody against human pancreatic PLA2. Serum PLA2 catalytic activity was analysed using a radiochemical method based on a substrate with tritiated palmitic acid in beta position. In necrotizing pancreatitis immunoreactive PLA2 and PLA2 catalytic activity both increased. Obviously, in necrotizing pancreatitis the major part of serum catalytic activity stems from the pancreas. In patients with diffuse peritonitis and multiple injuries, as a rule, immunoreactive phospholipase A2 serum concentration appears to be within the normal range. In contrast, in these patients we demonstrated high serum catalytic PLA2 activity comparable to that in necrotizing pancreatitis. The source of catalytic PLA2 activity in peritonitis and multiple injuries seems not to be the pancreas. There was a correlation between pulmonary insufficiency and serum PLA2 catalytic activity in patients with necrotizing pancreatitis, peritonitis, and multiple injuries.

    Description / Table of Contents: Zusammenfassung. Die cervicale Nahtinsuffizienz ist mit einer Frequenz von 20–50 % einer der wesentlichen Morbiditätsfaktoren in der Chirurgie des Oesophaguscarcinoms. Wir berichten über eine neue Technik der Gastroplastik, die Fundusrotationsgastroplastik, welche bei 53 Patienten angewandt wurde. Es handelte sich um 49 Patienten mit Oesophaguscarcinom und 4 Patienten mit gutartigen Erkrankungen. Die Klinikletalität nach Oesophagektomie und Fundusrotationsgastroplastik betrug 5,7 % (3/53), die Nahtinsuffizienzrate 7,5 % (4/53). Vorteile der Fundusrotationsgastroplastik gegenüber der konventionellen Magenplastik liegen in der besseren Durchblutung und der größeren Länge des Schlauchmagens. Klinische Vergleichsstudien werden belegen müssen, ob die Fundusrotationsgastroplastik der konventionellen Magenplastik überlegen ist.
    Notes: Summary. Cervical leakage, occuring on average in 20–50 % of the patients, is one of the major causes of morbidity following oesophagectomy for cancer. We report on a new technique of gastroplasty, namely fundus rotation gastroplasty which was used in 53 patients. There were 49 patients with oesophageal cancer and 4 with benign lesions. Hospital mortality was 5.7 % (3/53) and the leakage rate 7.5 % (4/53). The advantages of fundus rotation gastroplasty over conventional gastroplasty are the better blood supply and the greater length of the gastric tube. Controlled clinical trials will be necessary to confirm the advantages of fundus rotation gastroplasty versus conventional gastroplasty.

    Notes: Abstract Antibodies of various immunoglobulin classes to different cow's milk proteins were studied with the fluorescent immunosorbent test in 601 newborns, infants, children and adults (A). The antibody levels, expressed as the geometric mean (gm) of four antibody titres to casein, β-lactoglobulin, α-lactalbumin and bovine serum albumin, showed a clear dependence on age. They were compared with the antibody levels in children with cow's milk protein intolerance (C), other gastrointestinal disorders (B) and coeliac disease (D). The 20 children with cow's milk protein intolerance clearly differed (significance level 2×10-11) from those of the two control groups (A, B) insofar as the criterion adopted was not the titre against a single protein but the gm of the four antibody titres, and insofar as allowance was made for the age of the patients. All patients with cow's milk protein intolerance also showed elevated gm titres of IgE, IgA and IgM antibodies. However, since a number of children in the control groups also showed higher values, particularly with regard to IgE antibodies, the determination of the IgE, IgA and IgM antibodies adds little to the diagnosis and at best provides a further discriminatory aid. Although antibody titres fall immediately after placing the child on a milk-free diet, it is a matter of months before they become negative (titre 〈1∶20). After challenge titres rise again. In a longitudinal study of 25 children with acute gastroenteritis (E) it was shown that the antibody titres remained unchanged during and after the attack. This contradicts the often expressed opinion that the cow's milk antibodies frequently observed in healthy infants are induced as a consequence of gastroenteritis. In contrast to the other groups, all 26 children with proven coeliac disease (D) had antibodies to gliadin, irrespective of whether their gm cow's milk antibody titre was high or low.

    Notes: Abstract The analyzing power for elasticn-d scattering was measured atE n=13 MeV. The result is in good agreement with experimental data atE n =12 MeV andE n =14.1 MeV. At backward angles the data are well above the predictions of a rigorous Faddeev calculation using the Paris potential.
