    Notes: Abstract Idiopathic scoliosis is a progressive lateral curvature of the spine usually appearing during early adolescence. It may result in severe deformity with complications that profoundly affect the socio-economic future of the patient. The Milwaukee brace, frequently used in nonsurgical ambulatory treatment of scoliosis, seems to correct the disease by exerting traction and local pressure on the spine. Its lower end girdles the pelvis and the upper end bears on the occiput and mandible. In order to define precisely how the Milwaukee brace acts, and establish rational criteria for its use, a program has been undertaken to measure the forces exerted by the brace during treatment. In this paper, measurement by two three-component dynamometers of the forces the brace exerts on the occiput and mandible is described. The design of compact, sensitive measuring units, their incorporation into the brace and the associated instrumentation are discussed. A brief description of results obtained in an initial study of four patients is given. The dynamometers developed are suitable for use in force measurement in a wide range of orthotic and prosthetic appliances.

    Notes: A study has been made of the fractures and associated material from 34 prostheses broken in service. All the fractures appear to be of the fatigue type. Search by me-tallographic analysis for cracklike generating defects in material near the fracture reveals such conditions as abnormally coarse crystals, large nonmetallic inclusions, large inclusion population, undissolved master alloy particles, abnormal carbide segregation, interdendritic segregation, shrinkage, and gas porosity. Recommendations are made for the choice of metallic alloys for femoral stem application.

    Notes: Wear rates of orthopaedic polyethylene in 37°C water are not very contact stress dependent below 1000 psi (6.9 MPa) but above that level they accelerate substantially. The pressure dependence overall follows an exponential function. Creep in the contact pressure range of 3-17 MPa and above is a much larger factor than wear in indentation effects. For accurate measurement of wear rates by depth measurements it is necessary to permit creep to proceed under static load until it ceases before beginning the wear test.

    Notes: Using a metallographic technique to identify and count corrosion pits, the tendency for passive film breakdown has been studied under conditions that simulate a crevice and a combination of two different materials. Potential corrosion couples were formed among: 316L stainless steel, cast Co—Cr—Mo alloy, wrought Co—Cr—W—Ni alloy, Ti-6% Al-4% V alloy, a Co—Ni—Cr—Mo (multiphase) alloy and graphite. Only 316L stainless steel seems significantly prone to pitting corrosion in a crevice condition. Dissimilar metals in the crevice configuration did not seem to accelerate the corrosion of stainless steel.

    Notes: The wear rate of ultrahigh molecular weight (UHMW) polyethylene over a long campaign of sliding distance is not constant. Both unfilled and graphite filled polyethylene show a high rate early, which reduces to a low value. The graphite-filled product shows a late stage increase in wear rate. Increased molding temperature reduces the molecular weight and increases the wear rate. Titanium articulating with UHMW polyethylene develops frequently an abnormally high wear rate with the generation of a black wear product.

    Notes: Fatigue tests have been conducted on commercially produced cast Co-Cr-Mo alloy in tension-tension while surrounded by a physiological solution at 37°C The data have been analyzed by statistical techniques to yield an allowable stress such that there is 95% confidence of less than one failure in 1,000 after 5 × 107 cycles of stress fluctuation. These results are valid for a substantially sounder material than may be common in commercial use and therefore a more conservative allowable stress probably ought to be used for design purposes.

    Notes: Bar-shaped polymethylmethacrylate test specimens removed from rabbits after implantation for times up to 26 months showed a significant change in fracture stress as determined by three-point bending in the period between 12 and 26 months.     Notes: Porous titanium fiber implants for cementless skeletal fixation by bone ingrowth were treated with a calcium phosphate coating applied by a plasma flame-spray technique. In a paired experiment, treated and control implants were inserted in the humeri and olecranons of 36 adult dogs for periods of 1, 2, 4, and 6 weeks. After the animals were sacrificed, a biomechanical evaluation of the strength of skeletal fixation of the implants and a histologic evaluation of bone ingrowth was done. The mean shear strength of skeletal fixation at four weeks for the calcium phosphate-coated implants was 24% greater (p 〈 .01) than for paired controls. No difference in strength of fixation between treated and control implants was present at other time periods. The osteoconductive properties of the ceramic coating were demonstrated by bone forming in direct contact with the calcium phosphate coating on the metal fibers of the treated implants. No significant increase for the volume of bone ingrowth was established for treated implants compared to paired controls at any time period.

    Notes: Alloys of cast Co-Cr-Mo, wrought Co-Cr-W, wrought Co-Ni-Cr-Mo (MP35N), Ti-6% Al-4% V, and graphite were incorporated in specimens which simulated both a couple and a crevice corrosion condition. These specimens were implanted in a dog for 30 months. Post-sacrifice examination showed no substantial evidence of corrosion activity on the surfaces of the metals except for a tarnish film on the titanium alloy specimens. The presence of the tarnish film is taken as some evidence of corrosion activity and this correlated with a thicker average fibrous membrane, cellular activity in and near the fibrous membrane, and some increased vascularity. These must be regarded as preliminary observations.
