    Notes: Abstract. Although the presence of valves in the distal portion of the internal jugular veins has been widely described by anatomists and pathologists, their existence remains underscored in the medical literature. We set out to demonstrate their sonographic aspect and to evaluate their morphology, location and prevalence. Seventy-five cadavers were evaluated for the presence and aspect of such valves through neck dissections. Concurrent evaluation of the sonographic character of these structures was performed in another cohort of 75 adults. Valves of the distal portion of the internal jugular veins were found in 93 % of the cadaveric studies. Ultrasonography easily demonstrated the valve leaflets in 87 % of patients; all were observed in the distal portion of the internal jugular (IJ) veins. Sixty percent were bilateral with the majority of unilateral veins found on the right side. High-resolution ultrasonography easily demonstrates valves in the distal portion of the IJ veins which are present in the great majority of persons. Their evaluation may be of use in percutaneous procedures using this venous access.

    Notes: Summary The fine structure of an ovarian clear cell carcinoma in a 65 year old woman was analysed. The tumor cells were of both clear and “hobnail” types. Both were characterized by the presence of short microvilli, abundant glycogen granules, a well-developed granular endoplasmic reticulum and Golgi apparatus, scanty lysosomes and very few lipid bodies. The tubules and gland-like spaces were always separated from the stroma by a basement membrane. At ultrastructural level the tumor cells do not resemble at all those of clear cell renal carcinomas. On the other hand, their submicroscopic features are strongly similar to those of the clear cell tumors found elsewhere in the female genital tract, emphasizing therewith their very probable müllerian origin. It is recommended to eliminate the term “mesonephroma ovarii” to designate the clear cell carcinomas of the ovary.

    Notes: Summary The ultrastructural features of three granulosa cell tumors are presented. The neoplastic non-luteinized granulosa cells are characterized at sub-microscopical level by severely indented nuclei with prominent nucleoli, sparse to moderately developed predominantly granular endoplasmic reticulum, scanty lipids and lysosomes, small mitochondria with lamellar cristae and abundant intracytoplasmic filamentous material. The luteinized cells display a strongly developed tubular agranular endoplasmic reticulum and mitochondria with tubular cristae. These findings are compared with those of previous reports and discussed in relation to the well-known hormonal activity of these tumors.

    Notes: Summary Ovarian Sertoli-Leydig cell tumors are rare, most of them being endocrinologically active and causing virilization. Their histogenesis and nomenclature are still under discussion. Six such tumors were identified in the Laboratory for Histopathology of the University Women's Hospital, Zürich. The clinical data as well as the macroscopic, microscopic and ultrastructural features of these tumors are analysed and discussed. In an attempt to clarify their histogenesis, the ultrastructural features of these tumors have been compared with those of two granulosa cell tumors as well as with those of a testis in a patient with testicular feminization. Our own results and a study of published data suggest that Sertoli-Leydig cell tumors arise from ovarian stroma. Because of their obvious morphological similarities to testicular structures they should be termed “gonadal stromal tumors of android type”.

    Notes: Zusammenfassung Reine, homologe Uterussarkome gehören zu den bösartigsten Malignomen des weiblichen Genitale. Sie wachsen lokal ungemein agressiv und metastasieren rasch haematogen und lymphogen, so daß die Prognose ausgesprochen schlecht ist. Die Prognose korreliert in erster Linie mit dem Stadium. Die operative Radikalität entscheidet über den weiteren Verlauf. Die adjuvante Radiotherapie reduziert wahrscheinlich die Lokalrezidive, ohne jedoch die Überlebenszeit zu beeinflussen. Dadurch bekommt die Chemotherapie eine immer größer werdende Wichtigkeit. Dank der modernen, adjuvanten Chemotherapie in den letzten Jahren erscheint ein gewisser, vorsichtiger Optimismus berechtigt zu sein.

    Notes: Abstract. We report the case of a young woman who died from rupture of an aneurysmal dilatation of the left pulmonary artery. She suffered from tricuspid atresia type Ib and underwent a classic Glenn anastomosis at the age of 11 months; at 11 years a direct laterolateral anastomosis was constructed between the ascending aorta and the left pulmonary artery rather than a Fontan procedure for technical reasons. She subsequently developed severe pulmonary hypertension and an aneurysmal dilatation of the left pulmonary artery and was refused any further surgical correction.

    Notes: Abstract: A 30-year-old woman with mixed connective tissue disease was admitted with Wernicke’s aphasia and progressive dyspnoea with chest pain. Multiple brain infarcts on a computed tomographic scan were compatible with a thromboembolic aetiology. Echocardiography showed marked hypokinesia of the posterior wall, biventricular dilatation and a decreased left-ventricle ejection fraction (40%). A diagnosis of myocarditis was made on myocardial biopsies disclosing interstitial lymphocytic infiltrates and myocardial fibre necrosis. A treatment with steroids and monthly pulsed cyclophosphamide was introduced. The heart function rapidly improved as assessed by a left-ventricle ejection fraction of 55% and remained stable 17 months thereafter.
