    Notes: Summary Fourteen of 60 cases (23%) of trigeminal neuralgia underwent a repeat trigeminal cistern glycerol injection for recurrent pain. The mean time to recurrence was 14 months and the mean follow-up time following the last injection 10 months. Measurement of trigeminal cistern volume at the first and second procedure with metrizamide showed a significant decrease in the cistern volume from 0.38 ml to 0.29 ml. Clumping of nerve roots indicative of arachnoiditis was observed in 4 cases. In 2 further cases difficulty was encountered in emptying the cistern of metrizamide. After the repeat injection 2 patients developed painful dysesthesia. Six patients showed either new or increased facial sensory deficits. These findings may indicate trigeminal cistern arachnoiditis. The possibility of deafferentation pain developing after trigeminal glycerol rhizolysis should prompt caution in the use of this technique in the younger patient.

    Notes: Summary All linear measurements employed for evaluation of brain atrophy, were performed on 148 computed tomograms of patients aged 28 to 84 without evidence of any nervous system disorder. These included size of lateral, third and fourth ventricles, width of the Sylvian and frontal interhemispheric fissures and cortical sulci and size of the pre-pontine cistern. Various parameters indicated decrease in brain mass with age. Since the atrophic process is a diffuse phenomenon, integration of several measurements evaluating separate brain regions was made. The bicaudate ratio and the Sylvian fissure ratio (representing both central and cortical atrophy) were cominbed arithmetically, resulting in a correlation of 0.6390 with age (P〈0.0005). With a computed canonical correlation analysis; a formula was obtained which combined measurements of the lateral and third ventricles, the Sylvian fissure and the pre-pontine cistern. This formula yielded a correlation of 0.67795 (P〈0.0005). These linear measurements will allow simple and reliable assessment of reduction in brain volume during the normal aging process and in disorders accompanied by brain atrophy.

    Notes: Summary Multiple parameters for brain volume and mass were studied in 85 parkinsonian patients and in 149 normal controls aged 24 to 84 using CT scanning. In controls there was reduction in brain substance with advancing age. Increased brain atrophy in patients with Parkinson's disease (PD) was mainly observed in the younger age group of 24 to 49. This included parameters evaluating the size of the lateral and third ventricles (P〈0.05 and P〈0.01 respectively) and the size of the subarachnoid space in the frontal interhemispheric and Sylvian fissures (P〈0.005 and P〈0.01). With computed canonical correlation analysis a formula was obtained which expressed the tendency of the atrophic process in PD to involve the areas surrounding the third ventricle and the mesial aspect of the frontal lobes more than during normal aging.

    Notes: Summary Two cases of superficial siderosis with subependymal siderosis, secondary to neonatal intraventricular hemorrhage, are presented. High-field spin-echo MR imaging (1.5 Tesla) showed marginal hypointensity of the ventricular walls as well as of the subpial regions. These findings were most evident on T2 weighted images, characteristic of hemosiderotic deposits.

    Notes: Summary A family with 12 children, 6 of whom had Sjögren-Larsson syndrome, was studied clinically. On CT, all six siblings with the disease had patchy regions of white matter hypodensity, most marked in the frontal lobes, which were confluent in the more severe cases. The ventricles were not enlarged and no other abnormality was demonstrated. The CTs of the 6 unaffected siblings and the mother, who was also unaffected by the disease, were normal.
