    Description / Table of Contents: Résumé A l'occasion d'une étude rétrospective de 96 prothèses de hanche de type intermédiaire, les auteurs ont défini de nouveaux critères radiologiques permettant d'évaluer la position anatomique de la cupule dans le cotyle. Ils proposent une méthode de mesure de migration cotyloïdienne basée sur une étude radioanatomique préalable. Les repères utilisés, les constructions géométriques ainsi aue les méthodes de calculs sont décrits. L'application radio-clinique s'est révélée tout à fait fiable, et a permis de déterminer les facteurs de pronostic de mauvaise tolérance des cupules de hanche en regard du cotyle.
    Notes: Summary A retrospective study was carried out on 96 partial hip prostheses. Results of this study led us to define new radiological criteria for evaluation of the anatomical position of the prosthetic cup in the acetabulum. A method of measurement of the migration of the prosthetic cup in the acetabulum is proposed based on the results of roentgenological and anatomical study. The anatomical landmarks, geometric constructions and methods of computation of such measurement are described. The application of this technique to radiological/anatomical study was found to be reliable and allowed the determination of prognostic factors indicative of poor acetabular tolerance of the prosthetic cup.

    Description / Table of Contents: Résumé 100 drains thoraciques ont été mis en place chez le cadavre au niveau du premier espace intercostal en avant, selon des repères anatomiques précis. L'étude des plastrons nous montre la fiabilité de ces repères et l'absence de risque pariétal dans ce type de drainage. Enfin le drainage antérieur par le premier espace est moins dangereux pour les éléments du hile pulmonaire que le drainage antérieur par le deuxième espace.
    Notes: Summary One hundred thoracic drains were installed in fifty cadavers via the first anterior intercostal space according to precise anatomical landmarks. Study of the thoracic region demonstrated the reliability of these landmarks and the absence of parietal injury with this type of drainage. The results of this study allow to conclude that anterior drainage via the first intercostal space carries a lesser risk of injury to the structures of the pulmonary hilus compared to drainage through the second intercostal space.

    Notes: Conclusion Le drainage pleural au ler espace est devenu pour nous un geste habituel et sûr, dont les indications sont représentées essentiellement par le pneumothorax de la grande cavité pleurale et le drainage aérien des exérèses partielles apicales. Ce geste est simple et s'appuie sur des repères anatomiques précis. Cette expérimentation sur le cadavre nous conforte dans notre attitude et prouve, de manière indéniable, l'innocuité de la méthode.

    Description / Table of Contents: Résumé L'étude anatomique de 20 sujets a permis de préciser les possibilités d'utilisation des greffes costales pédiculisées en îlot dans la chirurgie du rachis. La longuer du pédicule dépasse environ 5 cm pour les côtes n° 3 à 10 dont la longueur utile est voisine de 12 cm. Ce potentiel permet une greffe osseuse de 4 à 6 vertèbres lombaires ou thoraciques.
    Notes: Summary The rib is frequently used as a bone graft in the surgery of the anterior spine. The advantage of the rib is that it is easily used as a pediculated vascularized graft when it is transfered to the anterior vertebrae. The average length of the pedicle is over 5 cm for the ribs number 3 to 10. Their useful length of bone is 12 cm. With this technique fusion of 4 to 6 lumbar or thoracic vertebrae is possible.

    Notes: Conclusion Si l'ouverture antérieure de l'anneau pelvien est une caractéristique du bassin dans l'exstrophie vésicale, ce travail a permis d'en préciser la dynamique, établissant l'évolution, en trois phases distinctes, de l'écart interpubien. L'existence de son aggravation rapide chez le nourrisson, puis en phase pubertaire, doit être connue. La stabilité de l'écart entre 5 et 10 ans doit inciter à la prudence dans l'appréciation à trop court terme des résultats de la chirurgie osseuse [1–3].

    Notes: Summary The laparoscopic treatment of inguinal hernia is currently gaining in popularity. Many surgeons are discouraged by the difficulties inherent in learning this technique, by the choice of the laparoscopic approach, and by the absence of long-term follow-up results. Less postoperative pain, more favorable cosmetic results, and rapid recovery are, however, all good reasons to prefer this type of surgery over other procedures. This study concerns our retrospective experience with 210 hernias. The extraperitoneal approach was used for all hernias in our hospital between 1 January 1996 and 31 August 1999, and by the same operator. One hundred and fifty-five patients were operated for 210 hernias. The extraperitoneal approach was used in all cases. 3 ports (4 for bilateral hernias) were used. Non-absorbable mesh (15 × 10 cm) was left in place in all patients. Patients were contacted in August 1999 to evaluate long-term outcome. 147 men and 8 women with an average age of 57.5 years (21–86) were studied. 17 patients were treated for recurrent hernias and 18 others for a first-time contralateral episode. 53 patients underwent bilateral treatment. Conversion to laparotomy was necessary in 0.6% of patients. Mean operating time was 54 minutes. Early morbidity occured in 7.6% (urinary retention, seroma) with only 1 hernial recurrence (0.4%) Late follow-up was at 24 months on average and showed 7 seromas, 6 inguinal hematomas, and 10 patients with residual pain. Average hospital stay was 23 days (1–5). Good results were achieved in our series using the laparoscopic approach for inguinal hernias. 24 months follow-up confirmed minimal recurrence (0.4%) using this approach and an overall morbidity comparable to other surgical procedures.

A terme, la hauteur entre l'épine iliaque postéro-supérieure et l'ischion mesure environ 40 mm. L'application est représentée par la chirurgie orthopédique de l'exstrophie vésicale. La correction de la malformation peut associer une ostéotomie iliaque postérieure bilatérale et une section des ligaments sacro-sciatiques. Une voie d'abord médiane unique est décrite.
    Notes: Summary Anatomic study of the pelvis of 30 fetuses and newborn children, yielded gave precise relationship between the sacrotuberous ligaments and size of the gluteal region. At birth, the distance between the posterior superior iliac spine and the ischium is 40 mm. Closure of the anterior defect in bladder exstrophy sometimes requires an orthopedic surgical procedure. Bilateral posterior iliac osteotomy and section of these ligaments can be easily done by a single medial Posterior approach herein described.

La croissance en longueur et en largeur de ces bassins s'effectuent conformément aux courbes de Tanner. Cependant, l'écart interpubien augmente de façon irrégulière, rapidement de 0 à 5 ans, lentement de 5 à 10 ans, et à nouveau rapidement de 10 à 15 ans. La coxométrie confirme l'absence d'anomalie d'orientation cotyloïdienne.
    Notes: Summary Pelvic growth in bladder exstrophy was studied in 10 patients. Growth of their pelves in length as well as in width was similar to that found by Tanner. Meanwhile, the speed at which the diastasis of the pubic symphysis increases is variable. Before 5 years of age and during the end of growth it is fast, whereas it is slow between 5 and 10 years of age. There are no radiological abnormalities of hip joint orientation, and the acetabular angles are normal.
