We studied the effect of oral calcium supplementation (1.0 g/day) for 1 week on baroreceptor reflex function and the lability of blood pressure in association with the changes in autonomic nervous activity in 14 hospitalized patients with mild to moderate essential hypertension (nine males and five females, mean age of 56 ± 11.2 (s.d.) years).2. Baroreceptor reflex sensitivity (BRS) was determined by the change in R-R intervals in response to the pressor response induced by phenylephrine injection. We measured coefficient of variation of R-R interval (CVr-r) and urinary excretion of catecholamines to evaluate the mechanism of change in BRS. We also used coefficient of variation of blood pressure (CVBP) and error of single cosinor analysis as parameters for lability of 24-h blood pressure.3. The means of 24-h systolic and diastolic blood pressures showed no significant changes after calcium supplementation for 1 week. BRS and CVR-R were significantly increased by calcium supplementation. Daily excretions of norepinephrine and epinephrine corrected by creatinine were unchanged. Both CVbp and error of 24-h systolic blood pressure showed a significant decrease after calcium treatment.4. These results indicate that oral calcium supplementation augments baroreceptor reflex function, in part through an enhancement of parasympathetic nervous activity, resulting in reduction of the lability of blood pressure in patients with mild to moderate essential hypertension.

The responses of angiotensin II (AII), AIII, aldosterone and plasma renin activity (PRA) to a single dose of captopril were investigated in hypertensive patients receiving long-term (more than I year) captopril therapy (CT patients) and compared with those of non-treated hypertensive patients (NT patients).2. Baseline levels of AII and aldosterone were significantly lower in CT patients than in NT patients. AM tended to be lower and PRA was slightly higher in CT than in NT patients, but these differences were not significant.3. A single administration of captopril (50 mg orally) significantly decreased plasma levels of AII, AIII and aldosterone as well as blood pressure in both CT and NT patients.4. These results demonstrate that chronically repeated administration of captopril to hypertensive patients effectively reduces the daily blood pressure and concomitantly the plasma AII level to acceptable levels in patients with no experience of ACE inhibition.

    Notes: Abstract Several immunological markers were studied in two patients with primary pulmonary histiocytosis X diagnosed by open-lung biopsy. The population of T- and B-lymphocytes from peripheral blood and the response of peripheral lymphocytes to phytohemagglutinin were within the normal ranges in two patients. The concentrations of serum IgG, IgA, and IgM in two patients were normal. However, Patient 1 showed a decreased serum IgE level and Patient 2 showed a lower border-line level of serum IgE as well. The selective low serum IgE level was the only immunological abnormality seen in primary pulmonary histiocytosis X.

    Notes: Summary Effects of unilateral nephrectomy on plasma renin concentration and renin substrate (angiotensinogen) concentration were studied over an experimental period of 10 days in rats. The maximum increase in plasma renin substrate concentration after bilateral nephrectomy was 9 times higher than the preoperative level. Very low but measureable plasma renin concentrations were found in rats nephrectomized bilaterally 24 hrs previously. In unilateral nephrectomized rats the concentration of renin substrate in plasma increased maximumly 3.5-fold within 24 hrs after the operation and on the third day it was decreased to half of the maximum level. Even 10 days after uninephrectomy the concentration of plasma renin substrate was significantly higher than that of normal rats. Twenty four hours after unilateral nephrectomy plasma renin concentration decreased to similar low levels as those found in bilateral nephrectomized rats. The decreased concentration of plasma renin, however, returned to normal 3 days after uninephrectomy. The inverse relationship was seen between the concentrations of plasma renin and plasma renin substrate. However, substrate concentration in plasma rised faster and longer than plasma renin decreased.

    Notes: Summary The effects of three experimental conditions on the concentration of plasma renin substrate were studied with special reference to plasma renin concentration in unilaterally nephrectomized rats. After simultaneous bilateral nephrectomy the maximum increase in plasma renin substrate was 17 times higher than normal within 24 h, while in rats which were unilaterally nephrectomized 10 days previously, followed by the removal of the remaining kidney (two-step bilateral nephrectomy), the maximum increase in plasma renin substrate was markedly suppressed (6-fold of normal). The maximum increases in plasma renin substrate by estradiol treatment in normal and unilaterally nephrectomized rats were about the same, associated with similarly decreased plasma renin concentrations. The similar increase in plasma renin substrate was found after ureteral ligation in unilaterally nephrectomized rats and bilateral ligation of the ureters in normal rats. This was the case where the plasma renin concentrations changed differently after ureteral ligation. After two-step bilateral nephrectomy plus estradiol treatment the maximum increase in plasma renin substrate was found to be higher than that found after two-step bilateral nephrectomy, but was lower than that after simultaneous bilateral nephrectomy. It is suggested that under the pathological conditions that stimulate renin substrate production, the plasma rein substrate concentration is less affected by circulating renin.

    Notes: Summary Effects of injection of homologous renin and sodium load given through 1.5% saline as drinking water on plasma renin activity and renin content of the kidneys were studied in rats with intact kidneys (group A) and in unilaterally nephrectomized rats (group B). During an experimental period of 10 days, blood pressure was measured four times. At the end of the experiment, the plasma renin activity and the renal content of renin were determined. Renin injection did not cause a rise in blood pressure in both groups. In unilaterally nephrectomized rats, renin content of the remaining kidney increased significantly. This increase was suppressed either by the renin injection or the salt load, whereas neither the renin injection not the salt load affected renal content of renin in group A. In unilaterally nephrectomized rats, the renin injection or the sodium load induced a significant decrease in plasma renin activity when compared to rats in group A.
