Abstract Gartner duct cysts derive from remnants of the vaginal portion of the mesonephric (Wolffian) ducts. In cases of incomplete regression of these ducts, cysts can develop due to secretory activity [1]. Clinically, those cysts are usually asymptomatic, their size not exceeding 2 cm in diameter. In rare cases with larger cysts, the presence of dyspareunia and problems in obstetric delivery are described [2, 3]. We present a case of a histologically proven symptomatic Gartner duct cyst with a size of 16 ×15×8 cm. To my knowledge, this is the largest Gartner duct cyst ever reported in the imaging literature.
