    Notes: Abstract A double-blind, randomized, parallel study was conducted to evaluate the safety and diagnostic utility of gadodiamide injection (a nonionic chelate of gadolinium and diethylenetriamine pentaacetic acid bis-(methylamide) [Gd-DTPA-BMA]) when administered IV for MRI of the central nervous system (CNS) at doses of 0.1 mmol/kg body weight (b. w.) or 0.3 mmol/kg b. w. A total of 24 patients received the lower dose and 23 the higher dose. Gadodiamide injection was administered to patients referred for MRI due to known or suspected CNS lesions. No clinically significant difference in blood parameters or efficacy was found between the two groups. The number of patients with adverse events other than discomfort was higher in the 0.3 group compared with the 0.1 group. In the 0.1 group, none of the 24 patients experienced any adverse events other than discomfort, and in the 0.3 group, 5 of 23 patients (21.7%) experienced a total of five adverse events other than discomfort. All adverse events were of mild or moderate intensity. Postinjection MR scans provided new information that affected patient management in the case of 6 patients in the 0.1 group and 9 in the 0.3 group.

    Notes: Mice immunized intranasally with a formalin-inactivated A/PR/8/34 (H1N1) influenza whole virus vaccine adjuvanted with cholera toxin, outer membrane vesicles from group B meningococci or formalin-inactivated whole cell Bordetella pertussis were protected against replication of the homologous virus in the nasal cavity. Only some mice were protected against clinical illness measured as weight loss and lowered body temperature. All mice immunized subcutaneously with one-tenth the intranasal vaccine dose without adjuvant were protected against clinical illness but not against local mucosal viral replication. Replicating virus was primarily found in animals with low concentrations of immunoglobulin (Ig)-A antibodies in saliva regardless of concentrations of IgG antibodies in serum. Clinical illness was seen only in those with low serum antibodies regardless of antibody levels in saliva. Nonreplicating nasal vaccines may not be sufficiently protective unless they also have a substantial influence on systemic immunity.

The patients were closely questioned about adverse events. In the Gadodiamide injection group, four patients reported six adverse advents, three of which were judged to be related to the contrast medium. In the Gd-DTPA group, two patients each reported one adverse event, both of which had an uncertain relation to the contrast medium. All events were mild and no medical treatment was needed. No significant change in neurological findings, blood pressure, pulse rate or blood parameters were noted in any patient. Both contrast media were effective; no difference in overall efficacy or safety was observed.
