Zusammenfassung Die zur Tolerisierung eines Transplantates notwendige Immunsuppressive Therapie bewirkt eine Unterdrückung des cellulären und humoralen Abwehrsystems. Die daraus resultierende erhöhte Infektanfälligkeit manifestiert sich in lokalen oder generalisierten infektiosen Komplikationen. An lokalen Infekten werden Wundinfektionen, Harnwegsinfektionen Bowie intraabdominelle und retroperitoneale Abscesse beoba.chtet. Bei den generalisierten infektiösen Komplikationen handelt es sich vor allem um Sepsis, Diverticulitis, Hirnabscesse, Meningitis, Encephalitis und besonders fulminant verlaufende, therapeutisch nur schwer zu beeinflussende Pneumonien. Als Erreger der pulmonalen Komplikationen werden vor allem Cytomegalieviren, Pneumocystis carinii sowie Hefen und Pilze gefunden. Das klinische Bild ist charakterisiert durch Hustenreiz, Dyspnoe, Cyanose Bowie fehlendes oder nur geringgradiges Fieber; röntgenologisch manifestiert sich die Erkrankung zuerst in der perihilären Region, um dann in Form weicher, konfluierender Fleckschatten auf alle Lungenabschnitte überzugreifen. Das klinische Bild, die röntgenologische Symptomatik und serologische Untersuchungen lassen jedoch keine Differenzierung der einzelnen Erregergruppen zu, die wegen der unterschiedlichen Therapie unbedingt anzustreben ist. Wir führen daher bei Auftreten erster Symptome und röntgenologisch. nachweisbarem Infiltrat eine percutane Lungenbiopsie durch. Bei Cytomagalievirus-Infektion müssen Corticosteroide, bei Pneumocystis carinii Pentamidin und bei Pilzen antifungale Medikamente verabreicht werden.

Summary The immunosuppressive therapy which is necessary to induce transplant tolerance at the same time suppresses all the cellular and humoral defence systems. The resulting increased susceptibility to infection manifests itself by local or generalized infectious complications. Local infections observed were: wound infections, infections of the urinary tract, and intra- and retro-peritoneal abscesses. Generalized infectious complications which occurred were mainly: gen. sepsis, diverticulitis, brain abscesses, meningitis, encephalitis and particularly acute forms of pneumonia which did not respond well to treatment. The causative agents of the pulmonary complications were mainly cytomegalo-viruses, pneumoeystis carinii as well as yeasts and fungi. The clinical picture is characterized by dry cough, dyspnoea and cyanosis without, or with only slight, pyrexia. Radiologically the disease first manifests itself in the perihilar region and spreads from there in the form of soft, confluent patchy shadows to all parts of the lung. The clinical picture, the X-ray appearance and the results of serological examination do not, however, permit a differential diagnosis according to the causative agents, though this of course would be most desirable from the therapeutic point of view. We therefore carry out a percutaneous lung biopsy at the onset of the first symptoms and an appearance of the characteristic infiltration on the X-ray. In cytomegalic viral disease, corticosteroids, in Pneumocystis carinii, Pentamidine and in fungal disease, fungicidal drugs, should be administered.
