    Does post-laparoscopy pain relate to residual carbon dioxide? (1996)

    Notes: We studied 20 day case gynaecological laparoscopy patients, who had an erect chest X ray taken before discharge. Patients were telephoned the next day for a semi-structured interview. Particular note was made of shoulder tip pain and pain relieved by changing posture. The X ray was analysed for measurements of the length of arc and height of the gas bubble under each hemi-diaphragm, from which an estimation of bubble volume was also made. We found statistically significant correlations between both the length of arc (p = 0.005) and volume of gas bubble (p = 0.008) on the right side, with the pain score. Residual gas can be a prominent cause of post-laparoscopy pain.

    Notes: We investigated 25 day case gynaecological laparoscopy patients to determine the duration of the pneumoperitoneum, the rate at which residual gas disappeared and the relative contribution of this gas to pain in the early postoperative period. The volume of gas was calculated from measurements of the subdiaphragmatic gas bubble obtained from an erect chest X-ray. Each patient was X-rayed twice, either immediately prior to discharge and at 24 h post laparoscopy, or at 24 h and 48 h post surgery. Patients were contacted by telephone at 24 and 48 h to confirm fitness to return for the X-ray and at the same time verbal pain and activity scores were recorded. We found that residual gas was almost entirely gone by 48 h following surgery and that it appeared to reduce in an exponential manner. The contribution of this gas to postoperative pain was significant in the first 24 h, but by 48 h it was considerably reduced. We conclude that in day case gynaecological laparoscopy patients, postoperative pneumoperitoneum is short-lived, and that by 48 h its volume and contribution to postoperative pain should be minimal.

    Notes: Kinetics of structure formation in electrorheological (ER) suspensions are characterized by the time required to form percolating fibers following application of an external electric field. Experimental results are compared with predictions from simulations using an electrostatic polarization model for ER suspensions. Results from the two approaches agree, with large response times at small concentrations, decreasing to 10 ms at large concentrations and field strengths of 2 kV/mm.

    Notes: 1 Insulin-liposomes of four different lipid combinations were given intragas-trically to normal rats on twenty occasions and to diabetic rats on 164 occasions.2 Normal rats showed no hypoglycaemic response to either sonicated or unsonicated liposomes.3 Diabetic rats failed to respond to sonicated liposomes but six animals became hypoglycaemic on two occasions each after administration of unsonicated insulin-liposomes.4 The responsive animals had all been diabetic for more than 18 days but another thirty-seven similar animals were unresponsive.

    Notes: Summary The prevalence of keratoconjunctivitis sicca (KCS) was studied in a randomly selected group of 104 patients with rheumatoid arthritis (RA). Symptoms of KCS were noted in 33 patients (31.7%) and they were studied further. After rose bengal staining, 22 of these patients were found to have KCS which was diagnosed by the presence of corneal or conjunctival desiccation. The overall prevalence of KCS was 21.2%. Although an abnormal Schirmer's test was noted in 21 of the 22 patients with KCS, only 8 patients had values of 5 mm or less while the remainder had a mild abnormality ranging from 6 to 15 mm. A reduced marginal tear film was noted in 15 patients (68.2%) with KCS and 13 patients (59%) had a reduced tear break-up time. The Schirmer's test alone is inadequate to make a definite diagnosis of KCS and it is essential to perform slit lamp examination to detect epithelial staining with rose bengal.

    Notes: Summary A series of 65 patients suffering from acute inflammatory disease of the orbit was studied by CT. Ethmoiditis was the cause in the vast majority; trauma and dental extraction played a lesser role in causation. Orbital cellulitis was diagnosed in 17 and subperiosteal abscess in the remaining 48. It was not possible to differentiate 33 pus-containing abscesses from the six with inflammatory masses (phlegmons). The satisfactory response to aggressive medical treatment in those patients with inflammatory masses that were not drained justifies a more conservative approach; surgical drainage being reserved for those with a deterioration in proptosis, ocular movements or vision. Six abscesses arose de novo, of which some were in the orbital fat rather than the subperiosteal space.

    Notes: Abstract The results of tissue typing on 115 recipient/donor pairs prior to corneal grafting were analyzed with the proportional hazard regression model for the incidence of the first rejection episode and for graft failure from rejection. Like other investigators, we found that a previously failed corneal graft and the degree of recipient corneal vascularization were significant risk factors for graft rejection. ABO blood group matching had no effect. The absence of mismatches in both the HLA-A and HLA-DR loci decreased the incidence of rejection. However, no difference was observed for the presence of one versus two mismatches. Regression results for the HLA-A and DR loci were not significant. Surprisingly, matching for one or both HLA-B alleles resulted in a significantly higher incidence of graft rejection episodes (P 〈 0.005) and of graft failure (P 〈 0.052). This adverse matching effect for the B locus was proportional to the number of mismatches.

    Notes: Abstract An experiment has been performed with the Fermilab 30-inch bubble chamber and Downstream Particle Identifier to study inclusive charged pion production in the high energy interactions of π±,K +,p and $$\bar p$$ with thin foils of magnesium, silver and gold. The laboratory rapidity and transverse momentum distributions are presented separately for π+ and π− production. Comparisons are made with data from hadron-proton interactions and theA dependence of the cross sections in the different kinematic regions is discussed. We investigate the dependence of the cross sections on the number of observed protons ejected from the nucleus. By using our π− A data from two different beam energies, we study the energy dependence of these spectra. Comparisons are made with the VENUS string model Monte Carlo.

    Notes: Abstract Four patients (six eyes) underwent successful corneal transplantations while receiving systemic cyclosporin. The six eyes had marked corneal vascularization preoperatively and in four, previous grafts had rejected. These eyes were therefore at high risk with respect to the likelihood of rejection and subsequent graft failure. All six grafts have remained clear (mean follow up twelve months) and only one minor rejection episode has occurred: this was easily reversed. The patients were monitored by repeated serum creatinine and trough serum cyclosporin levels. No significant side effects were encountered.
