Abstract Minimally invasive fixation techniques are growing in importance in internal fixation of tibial plateau fractures. Reconstruction of the joint surface is accomplished through small surgical incisions with arthroscopic and fluoroscopic control. Cannulated screw systems are inserted through stab incisions to fix fracture fragments. Cancellous bone defects are grafted with ceramic bone substitutes such as hydroxy apatites. The joint reconstruction is neutralised towards the tibial shaft by means of push-in preshaped plates inserted through small incisions or hybrid-type external fixators. This reduces surgical soft tissue trauma significantly. Well-defined indications, improved implants and standardised techniques will validate the role of these minimally invasive fixation procedures in the future.

Zusammenfassung Bei der operativen Versorgung von Tibiakopffrakturen rücken zunehmend minimalinvasive Techniken in den Vordergrund. Die Gelenkflächenrekonstruktion erfolgt arthroskopisch und radiologisch kontrolliert über kleine Zugänge. Gelenkfragmente werden über Stichinzisionen mit kanülierten Schraubensystemen stabilisiert. Bei der Auffüllung spongiöser Defektzonen bewähren sich Knochenersatzmaterialien wie Hydroxylapatit. Zur Stabilisierung des rekonstruierten Gelenkblocks nach meta- bzw. diaphysär werden eingeschobene Platten- und Hybridfixateursysteme eingesetzt. Voluminöse Implantate werden vermieden. Insgesamt kann hierdurch das operationsbedingte Weichteiltrauma verringert werden. Klar definierte Indikationen, weiterentwickelte Implantate und standardisierte Techniken werden den minimalinvasiven Osteosyntheseverfahren bei Tibiakopffrakturen in Zukunft einen zentralen Stellenwert einräumen.
