    Description / Table of Contents: Zusammenfassung Zwei FÄlle von aggressiver Fibromatose der Schilddrüsenregion werden mit histologischen, immunhistochemischen und elektronenmikroskopischen Methoden untersucht. Mit Hilfe von menschlichem Serum gegen glatte Muskulatur können kontraktile Zellen in sÄmtlichen PrÄparaten, vornehmlich aber in den Randbezirken der Wucherung nachgewiesen werden. Elektronenoptisch werden alle diese Zellen als Myofibroblasten identifiziert. Die zentralen Bezirke der Wucherung zeigen hyalin verdickte und gequollene Kollagenfaserbündel mit nur wenigen zellulÄren Elementen. Somit wird der Myofibroblast als der für die Wucherung charakteristische Zelltyp erkannt. Die sich daraus für Pathogenese und Histogenese ergebenden Schlüsse werden diskutiert.
    Notes: Summary Two cases of aggressive fibromatosis of the thyroid gland region were studied by histological, immunhistological and electron-microscopic methods. Contractile cells can be found in all tissue preparations using human serum containing antibodies to smooth muscle, especially in the periphery of the tumor. These cells have the characteristics of myofibroblasts by electron microscopy. The central areas of the tumor are composed of hyalin thickened bundles of collagen fibres with few cellular elements. The myofibroblast is thus considered to be the characteristic cell type of the tumor, and the pathogenesis and histogenesis of the lesion are discussed.

    Notes: Summary Reclassification of malignant goitres surgically removed between 1952–1975 reveals remarkable epidemiological alterations. There is a proportional decrease in undifferentiated carcinoma, which in males also represents an absolute decrease. The decrease is due to a change in maximum incidence from the 5th to the 7th decade. Differentiated carcinomas, especially the papillary tumours, increase. Comparison with findings in Switzerland shows many conformities. Iodine-prophylaxis apparently influences the morphology of thyroid carcinoma, as indicated by the time lag between the introduction of iodine-prophylaxis and the appearance of alterations in incidence. Iodine-prophylaxis, acting in this way, will improve survival rates in thyroid cancer.

    Notes: Summary Three cardiac Myxomas were studied by light-, immuno-fluorescence and electron microscopy. Acid mucopolysaccharides of the tumour were isolated chromatographically. The surfaces of the “myxomas” were papillary in two cases and smooth in one. The smooth surfaced tumour recurred six months after primary resection; histological specimens were more cellular than those of the papillary tumors and showed nuclear polymorphism and mitoses. The cells of two papillary tumours were identified as endocardial by electron microscopy, however, the cells of the smooth surfaced tumour were typical myofibroblasts. In both types of “myxomas” the cells were rich on cytoplasmic filament that contained acto-myosin. Biochemical investigation failed to reveal any difference between the acid mucopolysaccharide pattern of the two tumour types; isolated mucopolysaccharides being typical for embryonal mesenchymal tissue. The authors agree with Albertini (1963), that “cardiac myxoma” is a misnomer for two different typical endocardial tumours: a true endothelioma and a “special fibroma” (myofibroma or myofibrosarcoma).

    Notes: Summary Blood group isoantigens can be demonstrated immune-histologically on normal urothelium of the urinary bladder by means of the specific red cell adherence test. In preneoplastic and reactive changes of the urothelium and in dysplasia and carcinoma in situ, there is a loss of these antigens. Ultrastructural investigations of carcinoma in situ demonstrate a loss of normal membrane structures such as tight junctions, desmosomes, and glycocalyx. Of 18 patients with carcinoma in situ of the urinary bladder, 12 achieved tumor remission after topical chemotherapy with doxorubicin hydrochloride. A recurrence of blood group isoantigens was demonstrated in patients who achieved tumor remission. The antigenicity reappears before complete normalization of the urothelium. Ultrastructural investigations demonstrated normalization of cellular membrane structures in the respective biopsies. According to these findings and the current literature, the mechanisms of action of doxorubicin hydrochloride in carcinoma in situ may be due to retardation of cell cycle, cell loss by desquamation, cell death, and decrease of growth fraction.

    Notes: Abstract The cellular uptake and subcellular localization of indocyanine green (ICG; absorption band 700– 850 nm), and cell survival and ultrastructural changes following ICG-mediated phototherapy were investigated in vitro in four different cell lines derived from human skin (SCL1 and SCL2 squamous cell carcinoma, HaCaT keratinocytes and N1 fibroblasts). The cellular uptake of ICG (1–50 μM, incubation times 1, 4, 24 h) was saturable, highly cumulative and could be inhibited by the addition of 250 μM bromosulphophthalein indicating the involvement of the organic anion transporting polypeptide (OATP). For HaCaT cells, the maximum cellular uptake (Vmax) and the Michaelis constant (Km) were 9.9 ± 1.1 mM and 47 ± 16 μM, respectively, following a 24-h incubation with ICG. Fluorescence microscopy revealed a cytoplasmic distribution of ICG, probably bound to glutathione S-transferase. Following irradiation with a cw-diode laser (805 nm, 80 mW/cm2) at doses of 24 or 48 J/cm2, the phototoxicity was determined using the MTT assay as a measure of cell viability. For all cell lines, ICG concentrations above 25 μM produced a significant phototoxic effect. The EC50 of ICG for HaCaT cells following irradiation at 24 J/cm2 was 20.1 ± 3.9 μM. Growth curves showed that even HaCaT cells treated at the EC50 were killed within a week following treatment. Electron microscopy 1 h after ICG-mediated phototherapy revealed cytoplasmic vesiculation, dilation of the rough endoplasmic reticulum, the Golgi complex and the perinuclear cisternae and the beginning of chromatin condensation in the nucleus. These ultrastructural findings are not consistent with a photothermal action of ICG-mediated phototherapy. Taken together with those of previous studies by our group these results support photooxidation as a major cell-killing mechanism.

    Notes: Abstract PURPOSE: We evaluated the incidence and prognostic relevance of microscopic intraperitoneal tumor cell dissemination of colon cancer in comparison with dissemination of gastric cancer as a rational for additive intraperitoneal therapy. METHODS: Peritoneal washouts of 90 patients with colon and 111 patients with gastric cancer were investigated prospectively. Sixty patients with benign diseases and 8 patients with histologically proven gross visible peritoneal carcinomatosis served as controls. Intraoperatively, 100 ml of warm NaCl 0.9 percent were instilled and 20 ml were reaspirated. In all patients hematoxylin and eosin staining (conventional cytology) was performed. Additionally, in 36 patients with colon cancer and 47 patients with gastric cancer, immunostaining with the HEA-125 antibody (immunocytology) was prepared. The results of cytology were assessed for an association with TNM category and cancer grade, based on all patients, and with patient survival, among the R0 resected patients. RESULTS: In conventional cytology 35.5 percent (32/90) of patients with colon cancer and 42.3 percent (47/111) of patients with gastric cancer had a positive cytology. In immunocytology 47.2 percent (17/36) of patients with colon cancer and 46.8 percent (22/47) of patients with gastric cancer were positive. In colon cancer, positive conventional cytology was associated with pT and M category (P=0.044 andP=0.0002), whereas immunocytology was only associated with M category (P=0.007). No association was found between nodal status and immunocytology in colon cancer and with the grading. There was a statistically significant correlation between pT M category and conventional and immunocytology in gastric cancer (P〈0.0015/P=0.007 andP〈0.001/P=0.009, respectively). Positive immunocytology was additionally associated with pN category (P=0.05). In a univariate analysis of R0 resected patients (no residual tumor), positive immunocytology was significantly related to an unfavorable prognosis in patients with gastric cancer only (n=30). Mean survival time was significantly increased in patients with gastric cancer with negative cytology compared with positive cytology (1,205 (standard error of the mean, 91)vs. 771 (standard error of the mean, 147) days;P=0.007) but not in patients with colon cancer (1,215 (standard error of the mean, 95)vs. 1,346 (standard error of the mean, 106) days;P=0.55). CONCLUSIONS: Because microscopic peritoneal dissemination influences survival time after R0 resections only in patients with gastric but not with colon cancer, our results may provide a basis for a decision on additive, prophylactic (intraperitoneal) therapy in gastric but not colon cancer.

    Notes: Abstract Multicellular tumor spheroids (MCTS) grown from the bladder cancer cell line RT112 and from the prostate cancer cell line PCA were exposed to 200 or 800 electromagnetically generated focused ultrasound shock waves. RT112 cells showed a distinct but transient decrease in proliferation whereas the effect of PCA cells was less pronounced. Flow-cytometric measurements of DNA content and Ki67 expression revealed no significant changes in the cell cycle distribution. The capacity of RT112 cells exposed to 800 shock waves to re-grow as MCTS was markedly decreased, indicating an alteration of intercellular adhesion.

    Notes: Summary Between September 1990 and July 1991, we treated 17 patients with renal-cell carcinoma by radical nephrectomy and two patients with urothelial carcinoma of the kidney pelvis by ureteronephrectomy. Immediately after nephrectomy, perfusion of the kidneys with cold HTK solution was performed and the organs were kept in hypothermia of 8°C. The tumor-free parenchyma of the kidneys was treated 4 h later with shock waves of different energy levels in an experimental shock-wave system (Siemens Company, Erlangen). Light microscopy and examinations by scanning laser microscopy were performed after treatment. High-energy shock waves (HESW) produce significant changes in the tubulary and blood-vessel system of the viable human kidney, depending on the energy applied. Although our model is limited by hypothermia of the explanted kidneys, the effects of shock waves on the organs can be studied. Our model is suitable for testing the effects of different lithotriptors on the human kidney.

    Notes: Summary Topical chemoprophylaxis with adriamycin was administered to 49 patients with UICC-stages pTA and PT1 (pathological stage 0 and A) of transitional cell carcinoma of the urinary bladder. Tumor specimens obtained prior to initiation of the instillation protocol were evaluated with regard to ABH-antigenicity. ABH-antigens were present in tumors of 19 patients and none of these developed muscle-infiltrating recurrences during or after intravesical chemoprophylaxis. The primary lesions of 30 patients were deleted of antigens; only 5 (16%) developed a progressive recurrence. Compared to a historical control group this is thought to be a true reduction of invasive tumor recurrences in ABH-negative tumors by adriamycin. We herein propose that ABH-antigenicity may serve as a valuable prescreening indicator for or against topical chemoprophylaxis.
