    Notes: A new international orthodoxy has developed on health sector reform. Thedominant theme of the orthodoxy is the alleged benefits of market stylereforms for health development. This is shaping changes formulated, andbeing implemented, in the British NHS and other European health services(including Central and Eastern Europe), Latin America and a number ofdeveloping health systems in Africa and Asia. Sets out a ten-pointdescription of the orthodoxy. Contends that the orthodoxy is showingdistinct signs of restricting the analysis and development of healthmanagement and planning. This is a matter for considerable concern asthe adoption of market-style reforms can generate unforeseen and, insome cases, negative consequences. There is clearly a need forstrengthening management research and development as a basis foreffective health sector reform.

    Notes: Improving the nation's health is a major challenge. It has a longhistory within the specialty of public health medicine, with itscommitment to knowledge and action. But achieving a balance betweenthese activities has been a source of considerable tension within thespecialty. The commissioning function, with its related dimensions ofpurchasing and contracting, provides the best chance for decades ofaligning public health knowledge with the action that can produce healthimprovement. The concept of public health management seeks to bridgeknowledge and action, drawing on a range of skills. But there is a longway to go before the necessary skills and capacities are demonstrated inpractice. The organizational and personal development agenda remainsvery large. Yet, what is occurring in the NHS has not been achievedanywhere in the world. Describes one practical approach to strategichealth development and commissioning. But it will require sustainedcommitment if it is to succeed.

    Notes: Abstract The increasing incidence of breast cancer in the United States and the international variation in risk have led to speculation that environmental risk factors are an important cause of breast cancer. We review the epidemiologic evidence on the breast cancer risk associated with ambient environmental exposures experienced passively by the US population, and discuss the difficulties associated with measurement of specific exposures in environmental studies. We review geographic variation of breast cancer rates in the United States, and exposure to organochlorines, ionizing and electromagnetic radiation, and passive smoking. Results are inconclusive but do not support a major role of environmental risk factors in the etiology of breast cancer.

    Notes: The recent community care reforms have placed a high premium on interagency collaboration between health and social care agencies to ensure the delivery of high quality services to users. An examination of the historical record reveals the problematic nature of such activity. This paper provides a review of this record and then illustrates contemporary inter-agency issues through an analysis of a local experiment in joint service delivery in Leeds (UK). The paper's examination of the literature on joint working provides some clues as to why the experiment's original aims and objectives remained largely unrealized. The difficulties encountered by the key actors were a mix of cultural, professional and organizational factors.

    Notes: The public sector in Britain has been subjected to over a decade of major reform aimed at breaking up public service monopolies, at containing costs while at the same time opening services up to greater consumer choice. Health and social services have not been exempt from this revolution in the organization and management of public sector services. The long-standing policy of care in the community is being subjected to market principles and the introduction of a ‘contract culture’ very similar to the NHS reforms introduced in 1991.This paper reviews the origins of these developments in the doctrines of ‘new public management’, a movement which has proved attractive to policy-makers in many countries. Local authority social services departments have been identified as the lead agency for the development of a mixed economy of care following a review of community care policy by the government's health adviser, Sir Roy Griffiths, and a subsequent white paper. This paper examines the limited empirical evidence available on how managers and providers are meeting the challenge bestowed upon them, and concludes that most authorities are moving ahead cautiously if at all. Only a handful of authorities studied have embraced the reforms with any degree of enthusiasm.The paper concludes with an assessment of the reforms from two perspectives: a pessimistic one and an optimistic one. There are many worrying features of the reforms, not least among these being a lack of clarity over their intended purpose. Tensions and contradictions are plentiful, which places in jeopardy the certainty of the reforms in becoming user led rather than provider driven. A more optimistic scenario is that the changes are leading to a loosening up of services and practices which have often suffered from sclerotic tendencies, paternalism and sometimes complacency. If the reform process is skilfully handled and not rushed and if the ends are clearly established and communicated then users and carers could prove to be the principal beneficiaries.
