    Preoperative predictors of cancerous involvement of the neurovascular bundles in patients with localized prostate cancer (2002)

    Notizen: Abstract Background: The purpose of this study was to identify preoperative variables that would be useful in objectively selecting prostate cancer patients for nerve-sparing prostatectomy.Methods: Twenty-six patients with clinical T1c–T2c cancers were evaluated for cancerous involvement in the region of the neurovascular bundles (NVB) from prostatectomy specimens. Preoperative prostate-specific antigen (PSA) and pathologic features in systematic biopsy specimens also were reviewed.Results: A total of eight (31%) patients had cancerous involvement in the region of the NVB, including four on the right side, three on the left side and one on both sides. The percentage of each biopsy specimen occupied by the cancer was scored from zero to four and defined as the positive biopsy score. Preoperative PSA (P = 0.046), mean positive biopsy score (total sum of positive biopsy score divided by number of biopsy specimens; P = 0.001), number of cores containing cancer (P = 0.011), percentage of cores involved (P = 0.036) and maximum positive biopsy score (P 〈 0.001) were significant for predicting cancerous involvement in the NVB region using univariate analysis. However, only the mean positive biopsy score was independently significant according to multivariate analysis. To predict cancerous involvement in the region of each NVB, we found that ipsilateral mean positive biopsy score (total sum of corresponding positive biopsy score divided by number of ipsilateral biopsy specimens), number of cores involved on the ipsilateral side, percentage of cores involved on the ipsilateral side and maximum positive biopsy score on the ipsilateral side were significant predictive variables: the ipsilateral mean positive biopsy score being most appropriate for clinical practice.Conclusion: Ipsilateral mean positive biopsy score in systematic biopsy specimens can be an appropriate variable for selecting patients with localized prostate cancer for nerve-sparing prostatectomy.

    Notizen: We report a case of bilateral (2.4-cm left and 6.5-cm right) adrenal pheochromocytoma in a 27-year-old man that was treated by laparoscopic adrenalectomy. The patient had no evidence of multiple endocrine neoplasia, type II. The left adrenalectomy was performed first, with the patient in a semilateral position, and then the patient was turned before the procedure was begun on the right side. His postoperative convalescence was uneventful, and he continues corticosteroid replacement therapy. Transperitoneal laparoscopic adrenalectomy can be a safe procedure for bilateral pheochromocytoma, if the patients are carefully selected and the procedures are performed by experienced laparoscopists.

    Notizen: Objective: To determine the effects of an anterior urethral stitch, referred to as an endopelvic anterior urethral stitch (EAUS), in reducing recovery time for post-prostatectomy urinary incontinence.Methods: The urinary continence recovery time for 24 patients, who received a retropubic radical prostatectomy with the EAUS procedure, was compared to that of a historical control series of 22 patients without EAUS. The EAUS is a simple 2-0 polyglactin stitch placed between the bunched dorsal vein complex and the anterior urethra. This procedure was performed at the time of urethro-vesical anastomosis. Continence recovery time was defined as the day after removal of the urethral catheter when the patient no longer required pads for incontinence.Results: A significantly shorter time for continence recovery (median 8.5 days) was obtained in EAUS patients compared with that of the control series (median 72 days) (P 〈 0.0001). Early recovery of continence was observed in 12/24 patients (50%) within 1 week and 18/24 patients (75%) within 1 month in EAUS patients. No adverse effects or complications were observed in the EAUS patients.Conclusion: A surgical procedure, the EAUS, has been developed that reduces urinary incontinence in patients who have undergone a radical prostatectomy. This procedure is simple and quick and improves recovery of continence without any side-effects.

    Notizen: We report a rare case of synchronous testicular seminoma and adrenocortical carcinoma. A 57-year-old man had a left testicular seminoma (clinical stage 1MB) with metastases to the lung and paraaortic lymph node. A complete response was obtained after 3 courses of chemotherapy with single-agent carboplatin. However, a left adrenal tumor was detected 1 2 months later and demonstrated a tumor volume doubling time of 2.1 months. Chemotherapy with bleomycin, etoposide and cisplatin failed to stop the tumor growth. A laparoscopic adrenalectomy was performed and pathology revealed an adrenocortical carcinoma. The patient has been free of recurrence for 42 months postoperatively.

    Notizen: Abstract A 73-year-old woman underwent a pelvic exenteration for transitional cell carcinoma of the bladder and radiation-induced rectovaginal fistula. The patient had undergone radical hysterectomy and radiotherapy for cervical cancer 30 years earlier. Fifteen months after the operation, she suffered from ureteroaortic fistula, which was controlled by intraluminal ureteral occlusion using Gianturco coils. During 53 months of follow up, she has been free of hemorrhagic episodes.

    Notizen: Alumina/zirconia nano-composite coating was fabricated by plasma spraying usingagglomerated feedstock from fine powders of about 100 nm. The coating was consisted of fineγ-alumina and zirconia crystals with size of several nano meter and some amorphous boundary layers.The amorphous phase was crystallized and disappeared after heat treatment at 930°C. However, thecrystallite size was kept under 50 nm even after 1500°C-100hr heating, so the alumina-zirconianano-composite showed good thermal stability against the grain growth

    Notizen: Polymer impregnation pyrolysis method (PIP method) has some advantages to fabricateSiC fiber /SiC composite with large or complex form. However polymer derived SiC containssome amount of excess carbon and porosity. In particular the excess carbon is easy to be oxidizedin air at high temperature. Therefore reliability of the composite is not sufficient for long term use,because the oxidized matrix makes strong bonding between the fiber and the matrix. In this study,TiO2 as a reactive filler was added into polymer to consume the excess carbon by chemical reaction.Polycarbosilane (PCS) was used as a precursor polymer to synthesize a SiC matrix. It wasconfirmed that TiO2 was formed TiC on reaction with PCS over 1673 K by preliminary study. SiCfiber, Hi-Nicalon®, was used as reinforcement. Al2O3 powder was coated on surface of SiC fiberas interface layer of fiber/matrix. TiO2 added PCS/hexane solution was compounded with thefiber tow by filament winding. SiC fiber/SiC composite was synthesized at 1673 K for 3.6 ks inAr. The composite was densified by reinfiltration of TiO2 added PCS solution and pyrolysis at1673 K. Though densification behavior of the composite with TiO2 is slower than compositewithout TiO2, it was suggested that composition of synthesized SiC is close to stoichiometric one byapparent density. Fracture behavior of the composite indicated that addition of TiO2 was increasebonding strength between fiber and matrix. Bending strength of composite was increased afteroxidation test at 1473 K for 360 ks in air atmosphere
