Notes: Background. The frequency of tumors in the upper one-third of the stomach has been increasing. The standard operation for proximal gastric cancer has been total or proximal gastrectomy. The aim of this study was to present the pathologic and surgical results of 30 patients with early-stage proximal gastric cancer managed by proximal gastrectomy. Methods. A consecutive series of 30 patients who underwent proximal gastrectomy for early-stage proximal gastric cancer was studied. Sixteen patients underwent jejunal interposition, while 14 underwent gastric tube reconstruction, which consisted of a direct anastomosis between the esophagus and the remnant of the tube-like stomach. Results. Twenty patients (67%) had no abdominal symptoms and the lesions were detected by screening gastric fiberscopy. The tumors were mostly located along the lesser curvature (73%), were grossly depressed type (IIc) (70%), and histologically well differentiated type (63%). The depth of wall invasion was the mucosa in 12 patients, submucosa in 15, and muscularis propria in 3; lymph node metastasis was absent in 28 patients (93%). When compared with patients with jejunal interposition, patients with gastric tube reconstruction had a shorter operation time (327 vs 165 min), less blood loss (508 vs 151 g), and shorter hospital stay after operation (31 vs 17 days). Endoscopy and 24-h pH monitoring showed no evidence of reflux esophagitis, except in 1 patient with gastric tube reconstruction, and no patient died of recurrence. Conclusions. Early-stage proximal gastric cancer can be successfully treated by proximal gastrectomy. Since gastric tube reconstruction is a simple, easy, and safe procedure, proximal gastrectomy followed by gastric tube reconstruction is recommended for patients with early-stage proximal gastric cancer.

Notes: Supports were obtained by the interaction of C4H9MgCl with the reaction mixture of AlCl3 and CH3Si(OC2H5)3 or Si(OC2H5)4 (Mg/Al/Si = 2/1/1). With the combination of Al(C2H5)3 and methyl-p-toluate, immobilized titanium catalysts prepared by the treatment of the supports with TiCl4 and ethylbenzoate showed extraordinary high activity and stereoregularity in the polymerization of propylene.1 By the study of the reaction of AlCl3 with CH3Si(OC2H5)3, the elemental analysis and powder x-ray diffractometric measurements of the supports, it was found that the supports comprised of Cl, Mg, Al, and Si atoms, OC2H5 groups, C4H9 groups, and ethers, and that they were amorphous solids to the extent that the x-ray diffraction peak assigned to the 003 plane in MgCl2 crystals completely disappeared.

Notes: With use of the support prepared by the reaction1 of a Grignard reagent with reaction mixture of AlCl3 and CH3Si(OC2H5)3, an immobilized active stereospecific titanium catalyst was prepared by the three-step treatment of the support, first with TiCl4, second with ethylbenzoate, and third with TiCl4 again. The catalyst was also prepared by the two-step treatment of the support, with the mixture of TiCl4 and ethylbenzoate, and with TiCl4. Solids obtained in each step of the catalyst preparation were characterized by elemental and IR analysis, and their activities for propylene polymerization were determined with triethylaluminum as a cocatalyst under an atmospheric propylene pressure for 1 h at 60°C. The experimental data support the idea that both TiCl4 and ethylbenzoate as donors are immobilized on the surface of the active stereospecific catalyst without any interaction between them.

Notes: AlEt3Cl was modified with TLTTP (trilauryltrithiophosphite) in the catalyst system consisting of TiCl3 and AlEt2Cl. The effects of TLTTP on the polymerization of propylene were studied in comparison with those of alkyl homologues of TLTTP. The catalytic behavior of the TiCl3-AlEt2Cl-TLTTP catalyst system in the polymerization of propylene was also studied in comparison with that of the TiCl3-AlEt2Cl catalyst system. In the study of the effect of various alkylthiophosphites added, it is found that the bulkiness of the alkyl group affects the rate of propylene polymerization and the stereoregularity of the resultant polymers. The TiCl3-AlEt2Cl-TLTTP catalyst system gave different catalytic behavior in the propylene polymerization from that of the unmodified conventional catalyst system (TiCl3-AlEt2Cl). These effects of TLTTP were considered to be due to the bulkiness of the alkyl groups attached to the phosphorous atom and the higher reactivity to TiCl3 of the modified AlEt2Cl than of the unmodified AlEt2Cl.
