    Notizen: 2 cisplatin was administered intravenously over 1 h. Subsequently, the same dose of droperidol was also injected epidurally every 6 h for 18 h. Postoperatively, the antiemetic effect was evaluated until 30 h after the first epidural droperidol. As a result, all patients expressed almost complete antiemesis until 25 h. After 26 h, some patients complained of mild emesis. No potential side effects, such as hypotension, consciousness disturbance, and extrapyramidal signs, were noticed. Therefore, the present pilot study implies that a sole epidural administration of droperidol remarkably suppresses cisplatin-induced emesis.

    Notizen: Abstract To examine the effects of bilateral cervical sympathectomy on the secretion of adrenocorticotropic hormone (ACTH), thyroid-stimulating hormone (TSH), growth hormone (GH), and prolactin (PRL), 18 male rats were divided into three groups: control (Cont), sham operation (Sham), and bilateral cervical sympathectomy (Symp). All rats were kept under a normal circadian rhythm for 2 weeks. Subsequently, blood was collected and plasma ACTH as well as serum TSH, GH, and PRL levels were measured. The difference in ACTH levels between the Cont and Sham groups was not significant, but ACTH levels in the Symp group were significantly higher than those in the other groups. The difference in TSH levels between the Cont and Sham groups was also not significant, but TSH levels in the Symp group were significantly lower than those in the Cont group. There were no statistically significant differences in GH and PRL levels among these groups. The present results suggest that cervical sympathectomy in the rat increases ACTH secretion and decreases TSH secretion in the pituitary. These effects seem to be due to a mildly increased secretion of melatonin in the pineal body that probably in turn increases corticotropin-releasing factor (CRF) secretion and decreases thyrotropin-releasing hormone (TRH) secretion in the hypothalamus. Extrapolation of these findings to humans suggests that longterm and repeated stellate ganglion block would affect the pituitary secretions of ACTH and TSH.

    Notizen: Abstract Purpose. To examine the effect of propofol per se on cerebral blood flow (CBF), we measured CBF by single-photon-emission computed tomography (SPECT) using a technetium-99m ethyl cysteinate dimer before and after propofol administration. Methods. Ten healthy adult male volunteers were studied. Ten minutes after isotope injection, CBF was measured using a SPECT system. After this first SPECT scan, 1.5 mg·kg−1 of 1% propofol was administered over 30 s and the same dose of isotope was injected 2 min thereafter. Ten minutes later, a second SPECT scan was carried out. A subtraction SPECT image was obtained by reducing the first SPECT image from the second SPECT image. Based on these SPECT images, various regions of interest (ROI) were traced. Changes in regional CBF to each ROI were analyzed by a comparison of the total γ-ray counts in each ROI between the first and subtraction SPECT images. Results. The total γ-ray counts in each ROI did not change significantly after propofol administration. At this time, end-tidal carbon dioxide concentration and heart rate did not change, but blood pressure and oxygen saturation decreased slightly. Conclusion. The present result suggests that the induction dose of propofol does not alter CBF.

    Notizen: Abstract To examine the effects of bilateral cervical sympathectomy on the secretion of gonadotropin-releasing hormone (GnRH), luteinizing hormone (LH) and testosterone (TS), 24 male rats were divided into four groups: control (C), light (L), sympathectomy (S), and light-sympathectomy (LS) groups. The C and S groups were kept under a 12-h light-dark cycle and the L and LS groups were kept under continuous light for 2 weeks. After 2 weeks, blood was collected and the rats were perfused with a fixative. GnRH neurons in the hypothalamus were stained immunohistochemically, and serum LH and TS levels were measured by radioimmunoassay. Although the difference in the number of GnRH neurons between the C and S groups was not significant, the L group was significantly lower than the C or LS groups. The serum LH and TS levels in the L group were higher than in the other groups. The present results suggest that continuous light increases GnRH secretion in the hypothalamus, followed by increased secretions of LH in the pituitary and TS in the testes, and bilateral cervical sympathectomy under continuous light inhibits these hormonal changes. However, a normal circadian rhythm does not affect gonadotropin secretion. Therefore, long-term and repeated stellate ganglion block may inhibit the increases of GnRH, LH, and TS secretions induced by continuous light.

    Notizen: Abstract Purpose. The aim of this study was to elucidate whether urinary trypsin inhibitor excretion differs between general anesthesia (GA) and epidural block during general anesthesia (EPI) in lower abdominal surgery. Methods. Sixteen women undergoing abdominal total hysterectomy were assigned to the GA and EPI groups. The GA group received propofol induction and maintenance with isoflurane, nitrous oxide, and vecuronium. The EPI group received epidural block, followed by propofol induction and maintenance with isoflurane and nitrous oxide. The levels of adrenocorticotropic hormone and cortisol during anesthesia and on postoperative days 1, 2, and 3, and the levels of urinary trypsin inhibitor in 12-h urine from the day of surgery to postoperative day 3, were measured. Results. As compared with the EPI group, the GA group had a higher level of adrenocorticotropic hormone at the completion of anesthesia, higher levels of cortisol at the completion of anesthesia and postoperative day 2, and higher excretion of urinary trypsin inhibitor on the day of surgery and postoperative days 1 and 2. Conclusion. The present results suggest that excretion of urinary trypsin inhibitor into the urine under epidural block during general anesthesia is lower than that under general anesthesia alone in lower abdominal surgery. This is probably due to the difference in endocrine response to surgery between the two types of anesthesia.
