    Notes: The numbers of IgA, IgM and IgG-containing cells were studied by means of an indirect immunoperoxidase technique and morphometry in liver biopsies of patients with primary biliary cirrhosis and chronic hepatitis, in whom serum immunoglobulin concentrations were also determined. In patients with primary biliary cirrhosis the absolute and relative number of IgM-containing cells in the liver was significantly higher, whereas the absolute and relative number of IgG-containing cells in the liver was significantly lower compared to patients with chronic hepatitis. IgM-containing cells in liver biopsies of patients with primary biliary cirrhosis correlated strongly with their serum IgM levels. It is concluded that determination of the pattern of immunoglobulin containing cells in liver biopsies may help in the differentiation of primary biliary cirrhosis from chronic hepatitis and that local production of IgM in the liver may contribute significantly to the high serum IgM levels in patients with primary biliary cirrhosis.

    Notes: Summary A copper-containing protein was purified from the liver of a patient with primary biliary cirrhosis by a combination of gel filtration and anion exchange chromatography. This copper-protein had UV absorption and emission spectra, an amino acid composition, and a molecular mass which were characteristic for metallothionein (MT). From 8 livers (3 control, 1 fetal and 4 primary biliary cirrhosis) MT was extracted with non-reducing buffer and centrifuged, and the pellets were re-extracted with a 1% 2-mercaptoethanol-containing buffer. The non-reducing buffer extracted a predominantly copper-containing MT from the livers of patients with primary biliary cirrhosis and a predominantly zinc-containing MT from control lives and the fetal liver. Only from the fetal liver was a copper/zinc containing MT solubilized during the re-extraction with 2-mercaptoethanol-containing buffer. These results indicate that human MT is a unique metalloprotein with age and disease-dependent characteristics.

    Description / Table of Contents: Zusammenfassung Wir berichten über eine rezidivierendeListeria monocytogenes-Bakteriämie bei einer 46 Jahre alten Lebertransplantat-Empfängerin. Durch Serotypisierung wurde festgestellt, daß die beiden Bakteriämien durch verschiedene Stämme ausgelöst wurden. Die Möglichkeit einer wiederaufflackernden persistierenden Infektion wurde ausgeschlossen. Wir gehen davon aus, daß die rezidivierende Bakteriämie bei dieser prädisponierten Patientin durch Reinfektion verursacht wurde und daß die Antibiotikatherapie (Ampicillin plus Aminoglykosid) den Infektionserreger vollständig eliminiert hatte. Es bestand folglich keine Indikation für eine Antibiotika-Langzeit-Suppressionstherapie. Die Infektionsquelle für dieseL.-monocytogenes-Infektionen wurde nicht gefunden.
    Notes: Summary We report a case of a recurrentListeria monocytogenes bacteraemia in a 46 year-old liver transplant patient. Serotyping revealed that the two episodes of bacteraemia were caused by different strains. The possibility of a recrudescence of a persisting infection was rejected. We concluded that the recurrent bacteraemia in this predisposed patient was due to re-infection, and that antibiotic treatment (amoxicillin plus an aminoglycoside) resulted in a complete eradication of the infective microorganism. Therefore long-term suppressive antibiotic treatment was not indicated. The source of theseL. monocytogenes infections was not found.
