    Notes: Summary Mice immunized with once crystallized or chromatographed insulin at various doses throughout a period of three months, in order to investigate “tolerance induction” to different insulin preparations, unexpectedly revealed lymphocytic infiltrates in the endocrine and exocrine pancreas. Peri-insulitis, periductulitis and sometimes interstitial pancreatitis were observed both in animals injected with chromatographed insulin or once crystallized insulin, which can be fractionated into true Sanger insulin, a number of insulin compounds including proinsulin, and many undefined proteins of higher molecular weight. While the occurrence of periductulitis was not dependent on dosage, peri-insulitis was only found with certain dose ranges in both groups. Injections of once crystallized insulin evoked peri-insular infiltrates at a low dosage, injections of chromatographed insulin at a high dosage. The possibility is discussed as to whether or not the peri-insular and peri-ductular infiltrates reflect a cellular immune response induced by fractions of crystalline insulin, which may differ from true Sanger insulin.

    Notes: Abstract Background: To determine the value of sonography of the upper abdomen in primary sclerosing cholangitis (PSC). Methods: In a prospective study of 23 patients with PSC we performed upper abdominal sonography. Sonographic findings of the bile ducts were correlated with endoscopic retrograde cholangiographic (ERC) findings. Signs of advanced disease and complications were also sought. Results: The major limitation of ultrasound was its inability to exclude intrahepatic duct disease. In six patients with multiple strictures and pruning but without dilatations on ERC, sonography showed no intrahepatic duct abnormalities. Extrahepatic duct disease was adequately demonstrated on ultrasound. Mural thickening of the common bile duct (CBD), the hallmark of PSC in the appropriate clinical setting, was demonstrated in 17 of 18 cases with a stenosis on ERC. Ultrasound confirmed advanced disease manifested by signs of portal hypertension in seven patients. Marked nonsegmental intrahepatic duct dilatation and the presence of a mass lesion occurred in two of three cases in which complicating cholangiocarcinoma was found. Conclusion: Despite its inability to exclude intrahepatic duct disease, sonography is useful in diagnosing and following PSC.

    Notes: Abstract Objective: The aim of this study was to determine whether the increase in post-operative oxygen consumption (ΔVO2) in cardiac surgery patients is related to endotoxemia and subsequent cytokine release and whether ΔVO2 can be used as a parameter of post-perfusion syndrome. Design: Prospective study. Setting: Operating room and intensive care unit of a university hospital. Patients: Twenty-one consecutive male patients undergoing elective coronary artery bypass surgery without major organ dysfunction and not receiving corticosteroids. Measurements and results: Plasma levels of endotoxin, tumor necrosis factor (TNF) and interleukin-6 (IL-6) were measured before, during and for 18 h after cardiac surgery. Oxygen consumption, haemodynamics, the use of IV fluids and dopamine, body temperature and the time of extubation were also measured. Measurements from patients with high ΔVO2 (≥median value of the entire group) were compared with measurements from patients with low ΔVO2 (〈median). Patients with high ΔVO2 had higher levels of circulating endotoxin (P=0.004), TNF (P=0.04) and IL-6 (P=0.009) received more IV fluids and dopamine while in the ICU, and were extubated later than patients with low ΔVO2. Several hours after ΔVO2 the patient‘s body temperature rose. Forward stepwise regression analysis showed that circulating endotoxin and TNF explained 50% of the variability of ΔVO2. Conclusions: This study demonstrates that patients with high post operative oxygen consumption after elective cardiac surgery have higher circulating levels of endotoxin, TNF and IL-6 and also have more symptoms of post-perfusion syndrome. Early detection of high VO2 might be used as a clinical signal to improve circulation in order to meet the high oxygen demand of inflammation. In addition, continuous measurement of VO2 provides us with a clinical parameter of inflammation in interventional studies aiming at a reduction of endotoxemia or circulating cytokines.

    Notes: Abstract We report the association of myasthenia gravis (MG) and autoimmune thrombocytopenic purpura (AITP) in a 13-year-old girl. The co-existence of these autoimmune diseases is rare in adults and, to our knowledge, never described in children. In our patient thymectomy had a therapeutic effect on both MG and AITP, suggesting a altered T-cell function as a pathogenic factor of major importance in both affections.

    Notes: Abstract Crigler-Najjar syndrome (CNS) results from a mutation in one of the five exons of the gene coding for the enzyme bilirubin-UDP-glucuronosyltransferase by exon 1*1 and exons 2–5 of the UDP-glucuronosyltransferase 1 locus, the bilirubin glucuronidating isoform of UDP-glucuronosyltransferase. CNS type 2 is caused by a single base pair mutation leading to a decreased but not totally absent enzyme activity. In these patients the enzyme remains responsive to phenobarbital induction therapy and their bile contains low amounts of bilirubin mono- and diglucuronides. In CNS type 1 the enzyme activity is completely absent. CNS type 1 patients do not respond to phenobarbital and their bile does not contain more than traces of bilirubin conjugates. In 1997 we reported a World Registry on the treatment of patients with CNS type 1. Data were collected on 57 patients, of whom 21 (37%) had been transplanted at the time of data collection. Some 15 patients (26%) had brain damage, in 7 of whom the brain damage was mild and they received a liver transplant. Patients with brain damage at transplantation were significantly older than those without brain damage (14.3 vs 5.9 years). Before transplantation the serum bilirubin level of CNS type 1 patients should be kept below 350 μmol/l with daily phototherapy. Oral calcium supplementation makes phototherapy more efficient. Gene therapy has been performed successfully in the Gunn rat, an animal model for this disease. Liver cell transplantation has recently been done in a child with CNS type 1.

    Notes: Brown trout (Salmo trutta) from anadromous River Lierelva, resident Lake Tunhovd, and resident Nordmarka stocks were exposed to Gyrodactylus salaris-infected salmon parr. The brown trout were fed pellets before the experiments, except for one group of the Nordmarka stock which was starved for 19 days before the experiments. The mean number of parasites declined directly and rapidly post infection for all groups of trout. There were no pronounced differences in resistance between the anadromous and the resident stocks. G. salaris infections tended to persist longer on starved than on fed trout of the Nordmarka stock. The maximum parasite persistence on trout was 50 days, and as parasite numbers increased on some fish parasite reproduction must have occurred on those trout. However, the limited susceptibility and marked innate resistance of trout to G. salaris establishment, development and reproduction, suggest parasite metapopulations will not survive on this species. Nevertheless, trout may still play a role in the dispersal of G. salaris within and between rivers.
