Zusammenfassung Aus den Absterbekurven von 937 Patienten, deren Lungenresektion wegen Bronehuscareinom 5 Jahre und länger zurückliegt, geht der Zusammenhang der Lebenserwartung mit der histologischen Tumorform hervor. Der Prozentsatz der 5-Jahresüberlebenszeit steigt vom Plattenepitheleareinom über das polymorphzellig-solide zum kleinzelligen Careinom stufenweise ab. Deutlicher ist die Prognose jedoch vom Stadium der Erkrankung zum Zeitpunkt der Operation abhängig. Innerhalb der Stadien ist die Lebenserwartung für die Pneumonektomie und Lobektomien ziemlich ähnlich. Im Gesamtkrankengut ergeben die Lobektomien einen etwas höheren Prozentsatz 5-Jahresüberlebenszeit als die Pneumonektomien. Es ist daher in geeigneten Fällen die Lobektomie vorzuziehen, da sie auch Patienten mit reduzierter kardiopulmonaler Reserve eher zugemutet werden kann als die Pneumonektomie. Zweifel über die Zweckmäßigkeit der Resektionsbehandlung des Bronchuscarcinoms sind unberechtigt, da die Spätergebnisse besser sind als beim Magencareinom.

Summary The mortality graphs of 937 patients whose pulmonary resection for bronchial carcinoma dates back five years or more shows the relationship of life expectancy to the histological tumor type. From squamous cell epitheleal carcinoma with polymorphic cells to the small cellular carcinoma the percentage of 5 year survival shows a stepwise increase. However, the prognosis more definitely depends on the stage of the disease at the time of surgery. Within the stages the life expectancy after pneumonectomy and lobectomy is relatively similar. In the total patient material lobectomies produced a somewhat higher percentage of 5 year survival than did pneumonectomies. Thus, in suitable cases, lobectomy is to be preferred since it is more suitable for patients with a decrease of the cardio pulmonary reserve than is pneumonectomy. Doubt regarding the indication of resection treatment of bronchial carcinoma is not justified since the long-term results are better than those of gastric carcinoma.
