After two decades of experiences with endovascular interventions, with new devices, laser angioplasty and stents, the initial procedural success rate improved, the complications decreased and a broader range of lesion subsets could be successfully treated. However, the angiographic restenosis rate remained the same in the 30% to 40% of patients undergoing balloon angioplasty and 15% to 20% in patients undergoing stenting of simple, focal lesions. Many modalities of treatment, including radiation therapy, were explored to decrease these high restenotic rates. Preclinical studies on animal models did support the role of radiation. The first human experience was reported in 1997. Since then many randomized trials have begun to study the efficacy and effectiveness of different radioactive sources and methods of delivery. The dawn of an new exciting era has begun.
