    Notes: The strain in the bulk of material was evaluated using high energy white X-rays from asynchrotron radiation source of SPring-8 in Japan. An austenitic stainless steel (JIS-SUS304L) wasused for a specimen. The specimen of 5 mm thickness was subjected to the bending. The internal strainof it could be measured using white X-rays which range of energy from 60 keV to 125 keV. Themeasurement of the internal strain with a high accuracy was accomplished using the strain data fromseveral lattice planes of γ-Fe simultaneously. Furthermore, the measurement error of strain could bedecreased by using the diffracted beam with high energy, high peak count and the similar profile withthe Gaussian distribution. The results showed that the high energy white X-rays is effective for theinternal strain measurement in the depth of the order of millimeter

    Notes: This paper presents a basic research on a measurement of strain in the bulk of materialsby using high energy white X-ray from a synchrotron radiation source of SPring-8 in Japan. Ahigh-tensile strength steel (JIS-SHY685) was used as a specimen loaded with bending. Straindistribution in it was evaluated by the energy dispersive method using diffracted X-ray transmittedthrough the specimen. As a result, the internal strain of high-tensile steel of 5, 10 and 15 mmthickness could be evaluated using white X-ray which range of energy from 50 keV to 150 keV.The measurement with a high degree of accuracy was accomplished using α-Fe321 diffraction inthis material. The results showed that the internal strain measurement in the depth of the order ofmillimeter using the high energy white X-ray is practicable at SPring-8

    Notes: Oxygen uptake and release of (Li,Mn) ferrite [LMF; (Li0.60Mn1.20Fe1.20)1−δO4; δ= 0.007 to 0.033] was investigated concerning the relations between redox reactions of ferrite and cation migration. Mössbauer spectroscopy and X-ray diffractometry showed that some of the Li+ and Fe3+ ions migrated from the A sites to the B sites of the spinel-type structure and Mn4+ ions migrated from the B sites to the A sites during oxygen uptake at 573 K. The cation-deficient LMF formed by the oxygen uptake released oxygen molecules in He gas only at 660 K. The cation migration during the oxygen release was in the opposite direction of the movement during oxygen uptake at 573 K.

    Notes: Background. To investigate whether Helicobacter pylori infection, but not drugs, affects gastric somatostatin, interleukin-8 (IL-8), histological inflammation through eradication therapy, and interactions among these parameters.Methods. Twenty-eight H. pylori-positive patients (21 males; mean age 47.0 years) with either gastric ulcer (GU: n = 11) or duodenal ulcer (n = 17) diagnosed endoscopically were treated with dual therapy. Eradication was defined as negative microbiologic tests and 13C-urea breath test. Levels of antral and gastric juice somatostatin and mucosal IL-8 were measured by radioimmunoassay and enzyme-linked immunosorbent assay, respectively. Histology was assessed by the Sydney system.Results.  H. pylori was eradicated in 15 patients (10 males, 6 GU) out of 28 (54%). The patients’ backgrounds did not affect the eradication of H. pylori. Successes in eradication significantly increased antral and juice somatostatin contents, and dramatically decreased IL-8 levels and histological gastritis. In contrast, persistent H. pylori infection did not affect somatostatin and histological gastritis. An inverse correlation was present between changes in somatostatin levels and histological activity. No relationship was observed in changed values between antral somatostatin and IL-8.Conclusions.  These results indicate that eradication of H. pylori, but not the drugs used, induced an increase in somatostatin levels in the antrum and gastric juice, suggesting a close relationship between H. pylori and gastric somatostatin regulation. A close correlation between an increase in gastric somatostatin levels and the normalization of histological activity was present, suggesting that certain peptide-immune interactions in the gastric mucosa exist in H. pylori infection.

    Notes: Summary Six cases of dedifferentiated leiomyosarcoma of the small and large bowel are presented with histological, ultrastructural and immunohistochemical examination. One case arose in the jejunum, two in the ileum, and the other three in the large intestine. The tumours were submucosal in four cases with large areas of ulceration; two were polypoid. Four tumours showed typical leiomyosarcomatous appearance with dedifferentiated components and two were typical leiomyosarcomas at the primary site with differentiated components only in metastatic foci. By immunohistochemistry, typical leiomyosarcomatous areas showed a positive reaction for muscle-specific actin (MSA), MB1, MB2 and myosin. In contrast, desmin-positive cells were scattered throughout the tumour or were not present. Tumour cells in dedifferentiated components were positive for alpha-1-antitrypsin and alpha-1-antichymotrypsin in all cases but one; neuron specific enolase, MB1, MB2 and myosin were positive with variety. MSA was faintly positive in only a few tumour cells of two cases and desmin was not detected in any of the cases studied. Ultrastructurally, tumour cells in typical leiomyosarcomatous areas demonstrated evident smooth muscle features, although in dedifferentiated areas they lacked such features except in one case. Our results indicate that dedifferentiated elements may derive from ordinary leiomyosarcoma and loose muscle features due to dedifferentiation.

    Description / Table of Contents: Résumé Dans une étude antérieure, nous avons démontré que la diminution du débit sanguin de la muqueuse gastrique (DSMG) et la coagulopathie de consommation (CC) étaient facteurs d'hémorragie digestive chez le patient opéré d'un anévrisme de l'aorte abdominal (AAA). Dans cette étude nous avons voulu testé un traitement préventif de l'hémorragic digestive par ulcère ou ulcération aiguë chez le patient ayant un taux de DSMG bas et une CC. Des endoscopies préopératoires chez 66 patients ayant un AAA et chez 60 patients ayant une artériopathie oblitérante (AO) ont démontré que 50.0% des patients ayant un AAA et 16.6% des patients ayant une AO avaient soit un ulcère gastrique soit de multiples érosions. Chez 38 patients ayant un AAA et chez 36 patients ayant une AO, on a mesuré le DSMG par Doppler/laser à travers l'endoscope. Le DSMG était très perturbé chez les patients ayant un AAA et modérément perturbé chez ceux qui avaient une AO. Le taux de plaquettes et de fibrinogène étaient significativement abaissés chez les 38 patients ayant un AAA. Le taux de produits de dégradation de fibrinogène (PDF) chez 36.8% des patients ayant un AAA était supérieur à 20 μg. Basé sur ces résultats, les patients ayant un DSMG bas on été traités par des anti-II2 en postopératoire et ceux avec une CC par de l'héparine en préopératoire. Après un tel traitement, seulement un de 38 patients ayant un AAA après 1988 a saigné en postopératoire et il a pu être soigné sans chirurgie. Avant cette date, nous avons observé une hémorragie postopératoire chez 7 des 112 patients ayant un AAA, trois ayant nécessité une intervention chirurgicale.
    Abstract: Resumen Nuestro grupo ha informado previamente que la reducción del flujo sanguíneo de la mucosa gástrica (FSMG) y de coagulopatía de consumo (CC) son los factores principales en el desarrollo de sangrado gastrointestinal alto postoperatorio en pacientes con aneurismas de la aorta abdominal (AAA). En el presente estudio se investigó si el efecto del tratamiento de pacientes con FSMG disminuido o CC fue eficaz en cuanto a la prevención de sangrado postoperatorio por úlcera gástrica o por lesión aguda de la mucosa gástrica. El examen endoscópico preoperatorio en 60 pacientes con arteriosclerosis obliterante (ASO) reveló que 50% de los casos de AAA y 16.6% de los casos de ASO presentaban úlcera gástrica o erosiones múltiples de la mucosa. En 38 de los pacientes con AAA y en 36 de los pacientes con ASO se midió el FSMG mediante flujómetro de Doppler Láser a través del endoscopio. El FSMG apareció severamente alterado en los pacientes con AAA y moderadamente alterado en los casos de ASO. El recuento de plaquetas y los niveles de fibrinógeno aparecieron significativamente deprimidos en los 38 casos de AAA, en comparación con los controles; además, los niveles del producto de degeneración del fibrinógeno (PDF) fueron superiores a 20 μg en 36.8% de los pacientes con AAA. Con base en estos hallazgos, los pacientes con FSMG reducido fueron tratados postoperatoriamente con un bloqueador H-2, y los pacientes con CC fueron tratados con heparina. Luego de tales formas de tratamiento, sólo uno de 38 de los pacientes con AAA desarrolló sangrado GI alto y éste pudo ser manejado en forma conservadora, ello posterior a 1988, en tanto que años anteriores 17 de 112 pacientes desarrollaron sangrado GI postoperatorio y 3 requirieron cirugía de urgencia.
    Notes: Abstract We have previously reported that the decrease in gastric mucosal blood flow (GMBF) and consumption coagulopathy (CC) mainly contribute to the development of postoperative bleeding in the upper gastrointestinal (GI) tract in patients with abdominal aortic aneurysm (AAA). In the present study, we investigated whether our treatment for patients with low GMBF or CC effectively prevented postoperative gastric bleeding induced by gastric ulcer or acute gastric mucosal lesion. Preoperative endoscopic examinations in 66 patients with AAA and 60 patients with arteriosclerosis obliterance (ASO) revealed that 50.0% of AAA and 16.6% of ASO patients had accompanying gastric ulcer or multiple erosions. In 38 AAA and 36 ASO patients, GMBF was measured using a laser Doppler flowmeter through an endoscope. The GMBF was severely impaired in AAA patients and moderately impaired in ASO patients. The platelet counts and fibrinogen levels were significantly decreased in these 38 AAA patients compared with controls. Furthermore, fibrinogen degeneration product (FDP) levels in 36.8% of AAA patients were more than 20 μg/dl. Based on these findings, patients with low GMBF were treated postoperatively with H2-blocker, and patients with CC were treated preoperatively with heparin. After these treatments, only 1 of 38 AAA patients developed postoperative upper GI tract bleeding and could be treated conservatively after 1988, whereas in prior years post-operative upper GI tract bleeding developed in 7 of 112 patients, three of whom required emergency surgery.

    Notes: Abstract Cystic disease of the popliteal artery is a rare disorder in which most cases involve the formation of an adventitial cyst that disturbs the popliteal artery blood flow. We present herein the case of a patient presenting with popliteal artery occlusion due to compression by a cyst which formed at the media of the popliteal artery. The onset occurred during a baseball game in which he played catcher. Preoperative magnetic resonance imaging demonstrated a communication of the cyst with the adjacent knee joint. This unusual case could provide important clues to help identify the pathogenesis of this disease.
