Summary In the experimental cortisol pneumonia of the young rat the swelling of the alveolar cells is related to a considerable increase of the histochemically detectable activity of non-specific esterase. In addition, the pulmonary alveoli become infected with pneumocystis carinii which likewise show an intense activity of non-specific esterase. If the animals are treated briefly (after the 45th or 46th experimental day) with single or repeated inhalations of nebulized esterase inhibitor (Mintacol=E 600=diethyl-p-nitrophenyl phosphate), the cortisol pneumonia is effected in the following way: 1. The focal swelling of the alveolar cells is prevented; the destruction of the free alveolar macrophages is enhanced. The obvious tendency towards a normalization of the increased esterase activity, which occurs after repeated inhalations, is related apparently to a general inhibitory effect of E 600 on the alveolar epithelium; this effect at present cannot be better defined. 2. Intra-alveolar groups of pneumocystis show advanced or complete loss of histochemical activity of esterase and evidences of degeneration. The possibility of treating human pneumocystosis with an aerosol of E 600 is discussed.

Zusammenfassung Die Alveolarzellschwellung bei der experimentellen Cortisol-Pneumonie der Jungratte ist mit einer betrÄchtlichen Zunahme der histochemisch nachweisbaren AktivitÄt an unspezifischer Esterase verbunden. Die im weiteren Verlaufe die Lungenalveolen besiedelnde Pneumocystis carinii ergibt ebenfalls starke AktivitÄt an unspezifischer Esterase. Wenn man die Tiere nach dem 45. oder 46. Versuchstage einer ein- oder mehrmaligen Inhalationsbehandlung mit einem vernebelten Esterasehemmer aussetzt (Mintacol=E 600=DiÄthyl-p-Nitrophenylphosphat), wird die Cortisol-Pneumonie in folgender Weise beeinflu\t: 1. Das Allgemeinbefinden der Tiere bessert sich auffallend. 2. Die herdförmige Alveolarzellschwellung wird unterbunden, der Zerfall der freien Alveolarmakrophagen beschleunigt. Die nach mehrmaliger Inhalation deutliche Tendenz zur Normalisierung der gesteigerten Esterase-AktivitÄt steht offenbar im Zusammenhang mit einer allgemeinen, noch nicht nÄher definierbaren Hemmwirkung des E 600 auf das Alveolarepithel. 3. IntraalveolÄre PneumocystisverbÄnde zeigen weitgehenden oder vollstÄndigen histochemischen AktivitÄtsverlust der Esterase und Zerfallserscheinungen. Die Möglichkeit einer Spray-Behandlung der menschlichen Pneumocystis-Pneumonie mit Fermenthemmern wird diskutiert.
