    Notes: Summary Hepatocellular carcinoma is a major malignant disease in parts of Africa and Asia, including Korea. Surgical resection, which represents the best hope for cure, is limited by the extent of the disease and the high incidence of concurrent liver cirrhosis in Korea. We designed a phase H trial of combined external radiotherapy and hyperthermia for hepatocellular carcinoma that was unresectable due to either locally advanced lesions or associated liver cirrhosis so as to evaluate the efficacy and the safety of this combination regimen. This trial was performed at Yonsei Cancer Center between April 1988 and July 1988. External radiotherapy was delivered to a total dose of 3060 cGy/3.5 weeks. Hyperthermia was applied twice a week for a total of six treatment sessions using an 8-MHz radiofrequency capacitive-type heating device, i. e., Thermotron RF-8 and Cancermia. In all cases, hyperthermia was carried out within 30 min of the radiotherapy for a period of 30–60 min. The temperature in the tumor was measured by inserting a thermocouple into the tumor mass under ultrasonographic guidance in patients who did not have a bleeding tendency. The tumor response was assessed by CT scan after completion of the designed treatment. No complete response was obtained. However, a symptomatic improvement in abdominal pain was observed in 78.6% of cases and a partial response was achieved in 40% of the patients. The most important factor affecting the tumor response was the type of tumor (single massive, 71.4%; diffuse infiltrative, 20%; multinodular, 0;P〈0.005). The 1-year survival values determined for all patients and for the partial responders were 34% and 50%, respectively. The overall median duration of survival was 6.5 months. The median duration of survival for the partial responders was longer than that for the nonresponders (11 vs 5 months;P〈0.05). A mild degree of heat sensation, fever, first-degree burns of the skin, and nausea were observed as treatment-related adverse reactions. In conclusion, although this study is being continued, the results obtained thus for indicate that combined radiotherapy and hyperthermia seem to be effective in providing local tumor control and pain palliation in unresectable hepatocellular carcinoma while producing an acceptable level of toxicity.

    Notes: Summary Radioiodinated fatty acid esters, such as lipiodol or ethiodol, are localized in the hypervascular hepatocellular carcinoma (HCC) for a long time following intra-arterial hepatic injection, enabling delivery of high internal radiation to the tumor. The desired radiation can easily be delivered to small HCC, less than 5 cm in diameter, in single or multiple procedures with an 8-week interval. For larger tumors, [131I]lipiodol or [131I]ethiodol in conjunction with chemotherapy emulsion, Ivalon embolization or all three combinations should be considered for maximal clinical results. A strong beta emitter with shorter physical half-life, i.e.90Y will be more effective in the management of HCC if one can label lipiodol with90Y.

    Notes: 16 different lanthanum-aluminosilicate glasses were prepared to evaluate the effect ofalkalis (Na2O, K2O) and alkaline earths (MgO, CaO) on thermal expansion coefficients (CTEs) of theglasses. Analysis was performed with an aid of the Taguchi method and orthogonal arrays to elucidatefour factor interactions between 4 two-level factors using two level L16(215) orthogonal arrays. Theobserved CTEs were in the range of 6.29×10-6 C-1 to 8.22×10-6 C-1. The addition of alkalis was moreinfluential to CTE of the glass than that of alkaline earth, however, co-addition of the mixed alkaliswas detrimental to CTEs of the glasses due to the crystallization caused by the mixed alkali effect.However, the highest strength of the glass infiltrated alumina composite was observed at a CTE of6.4×10-6 C-1 due to the compressive stress caused by the thermal mismatch, indicating that CTEdifference between the glass and the alumina should be above 1.0×10-6 C-1

    Notes: Abstract Biomaterials play a critical role in the engineering of new functional genitourinary tissues for the replacement of lost or malfunctioning tissues. They provide a temporary scaffolding to guide new tissue growth and organization and may provide bioactive signals (e.g., cell-adhesion peptides and growth factors) required for the retention of tissue-specific gene expression. A variety of biomaterials, which can be classified into three types – naturally derived materials (e.g., collagen and alginate), acellular tissue matrices (e.g., bladder submucosa and small-intestinal submucosa), and synthetic polymers [e.g., polyglycolic acid, polylactic acid, and poly(lactic-co-glycolic acid)] – have proved to be useful in the reconstruction of a number of genitourinary tissues in animal models. Some of these materials are currently being used clinically for genitourinary applications. Ultimately, the development or selection of appropriate biomaterials may allow the engineering of multiple types of functional genitourinary tissues.

    Notes: This paper presents the feasibility of a sensor system consisting of piezoelectric oscillatorsensors to detect local damages and ultrasonic sensors to detect global damages in a structure. Theoscillator sensor is composed of a feedback oscillator circuit and a piezoceramic lateral mode vibratorto be attached to a structure. Damage to the structure causes a change in the resonant frequency of thevibrator, which is detected by the oscillator circuit. However, the response of the oscillator sensor islimited to the area around the sensor, thus local measurement. The ultrasonic sensor generates Lambwaves and the waves traveled over a long distance are received by another piezoceramic patch on thestructure. The received wave form reflects all the defects encountered during the propagation, thusglobal measurement is possible. The two sensor types are combined as a sensor network, and itsoperation system is developed as a portable unit for practical applicability

    Notes: Summary Among 80 patients with advanced hepatocellular carcinoma, treated by systemic chemotherapy, 19 patients (24%) achieved partial response or stable disease. The median survival time of responders was 20 weeks. Radioiodinated fatty acid esters ([131I]lipiodol) are localized in hepatoma tissue for a considerable time. Taking advantage this phenomenon, there is a potential therapeutic effect of [131I]lipiodol, since the radioactive oil can deliver a therapeutic dose of internal radiation. The response rate of patients who had a small hepatoma (less than 5 cm in diameter) was 80%. Among 25 responders, 12 cases are alive after more than 1 year. A pilot study of hyperthermia, using a heating device capable of delivering radiofrequencies, combined with radiotherapy or chemotherapy, demonstrated that this modality is safe and effective in the treatment of advanced hepatocellular carcinoma. Of the 19 patients treated, 47% showed partial remission or stable disease. The median survival time of 9 responders was 9 months. Among the 9 responders 3 patients survived for over 1 year.

    Notes: Abstract The aim of the present study was to deliver a high internal radiation dose to small hepatocellular carcinoma (HCC) lesions in an attempt to treat this disease. A total of 18 patients with HCC lesions measuring less than 4.5 cm in diameter (25 lesions) were treated with superselective intra-arterial injection of I-131-labeled Lipiodol (370–1,100 MBq in 3–5 ml) using a 5-F or coaxial catheter. All the lesions were nodular, multinodular, or hypervascular on pretreatment angiography. In all, 15 lesions that received over 180 Gy of cumulative radiation decreased in size in proportion to the Lipiodol retention on CT, and no pericapsular recurrence was found on angiography after 14–54 months of follow-up. In five patients who subsequently underwent surgery, 65% to 100% tumor necrosis was detected. No abnormal change in liver function tests or untoward clinical symptom of the lung, thyroid, or bone marrow was detected in patients who survived for more than 3 years after the treatment. Superselective high-dose internal radiation therapy of small HCC offers hope of treatment and long-term local control without complications.
