Notes: summary  The shape of a patient's face is commonly used as a reference to select the shape of the maxillary central incisors in edentulous patients. The validity of this relationship has not been proved. The objective of this clinical study was to determine whether a relationship exists between maxillary central incisors and face shapes. Casts were made of the maxillas of 50 men and 50 women. A standardized digital photographic procedure was used to record frontal views of each subject's face and of the maxillary central incisors of the dental casts. The shapes of the maxillary central incisors were compared with the face forms. Shape matches were evaluated according to their Hausdorff distance (HDD). The function h(A,B) is called the directed HDD from shape A to shape B (this function is not a true distance). It reflects the distance of the point of shape A that is farthest from any point of shape B and vice versa. The similarity of both shapes is given as a non-negative number. The value 0·0 indicates that the figures are identical (after scaling and shifting). Higher values indicate that shapes differ more substantially. Significant differences on the 5% level were calculated using the non-parametric Mann–Whitney U and Kruskal–Wallis tests. The face shape from the chin margin to the eyebrow line (superior edges of the eyebrows) produced a better match than the one from the chin to the hairline (P 〈 0·0001). On average, the maxillary central incisors displayed a variability (0·084 ± 0·028) that was higher by a factor of 1·9 than the face shapes (chin margin to the eyebrow line, 0·045 ± 0·015). In the interindividual comparison, the shapes of the maxillary central incisors of women displayed a significantly smaller HDD than the ones of the men (P 〈 0·0001).

Notes: Abstract Extensive physiological studies of swallowing have been carried out in laboratory animals; however, similar studies in humans have been limited by available technology. In this study we describe the use of a solid-state circumferential sphincter transducer to define manometric characteristics of the human pharynx and upper esophageal sphincter (UES). Effects of pharmacologic agents and thermal stimulation are also described. We studied nine normal volunteers on three separate days. All studies were done in the upright position and consisted of a station pull-through of the UES and six wet swallows with the sphincter transducer in the most proximal segment of the UES and a posteriorly oriented single transducer 5 cm proximal in the pharynx. Baseline studies preceded all drug studies. Effects of bethanechol were studied on day 1, cold stimulation and benzonatate on day 2, edrophonium and atropine on day 3. The UES resting pressure showed large intrasubject day-to-day variations; however, mean values did not differ. There were no effects on UES relaxation or swallow coordination with any of the pharmacologic agents, although benzonatate produced multiple pharyngeal contractions.

Notes: Summary A patient with chronic idiopathic pseudo-obstruction is reported and the results of a double blind therapeutic trial of metoclopramide are described. Within the limits of this trial metoclopramide was ineffective by all clinical criteria.

Notes: Abstract Fifteen patients with amebic abscess of the liver were seen at two California county hospitals over a period of 3 1/2 years. Of the 15 patients, 9 diagnoses were proven and 6 were presumptive. Fourteen of the patients had made a recent trip or had ready access to Mexico. The clinical finding of particular note was the high incidence of right lower lung field abnormalities. Useful laboratory studies included the combination of an elevated BSP, alkaline phosphatase, and direct reacting bilirubin. The value of the liver scan is emphasized. The high incidence of secondary infection of the abscesses is stressed. The uncomplicated postoperative courses of those patients treated by surgical drainage and postoperative chemotherapy for amebiasis is of particular importance since this approach has been considered in the past to be most hazardous.

Notes: Summary We have reported a patient with methyldopa-induced hepatitis who was inadvertently challenged with the drug. The histopathology of the liver was indistinguishable from that of viral hepatitis. The experience with our patient and a review of the literature indicated rapid recovery in all cases when the drug was discontinued.

Notes: Summary The familial occurrence of achalasia has been previously recorded and a genetic origin for the disease has been postulated. We present the first case of achalasia in monozygotic twins and suggest that concordance for the disease is consistent with a genetic factor in the etiology of achalasia.
