    Notes: Summary Serum levels of immunosuppressive acidic protein (IAP) were determined in 48 healthy controls (age: $$\bar x = 37.8 \pm 13.8$$ years) and 72 pregnant women (age: $$\bar x = 27.05 \pm 5.5$$ ). The mean value of IAP in pregnant women (345.3±94.4 µg/ml) was significantly lower than that of the healthy control group (P〈0.0001; 452.0±95.0 µg/ml). Correlated to the clinical status were 207 IAP serum values of 63 patients with ovarian cancer (age: $$\bar x = 56.9 \pm 11.1$$ ). The mean value of patients with tumor progression ( $$\bar x = 799.3 \pm 292.2\,\mu g/ml$$ ) was significantly increased compared to those of patients in remission ( $$\bar x = 511.0 \pm 111.9\,\mu g/ml$$ ) and the control group ( $$\bar x = 463 \pm 146.8\,\mu g/ml$$ ;P〈0.0001). However, no significant difference of the IAP levels was found between the group in remission and the healthy controls. With a threshold value of 640 µg/ml we found right-negative values in 88.7% and right-positive values in 65.1% during the follow-up of patients with ovarian cancer.

    Notes: Zusammenfassung Angesichts der derzeit zur Verfügung stehenden operativen Behandlungsmöglichkeiten werden intraurethrale Injektionen mit zunehmender therapeutischer Effizienz und Sicherheit in der Therapie der weiblichen Streßharninkontinenz eingesetzt. Die Anforderungen, die an das Material gestellt werden, umfassen einfache Handhabung, hohe Gewebeverträglichkeit, fehlende Ablagerung in anderen Geweben und eine hohe Beständigkeit des Materials am Applikationsort. Die vorliegenden Kurzzeitbeobachtungen sind vielversprechend und könnten in weiterer Folge nicht zuletzt aufgrund der geringen Invasivität der intraurethralen Injektionen zu einer Änderung im Behandlungskonzept der weiblichen Streßharninkontinenz führen. Darüber hinaus eröffnet sich ein Weg in der Durchführung dieser Methoden auf ambulanter Basis mit geringen Kosten und einer, wenn überhaupt, dann kurzen Hospitalisierung und Rehabilitation. Freilich fehlen noch Langzeitergebnisse, um eine endgültige Aussage über die Effizienz der intraurethralen Injektionen in der Therapie der weiblichen Streßinkontinenz machen zu können. Die besten Ergebnisse der intraurethralen Injektionen sind bei normaler Detrusorfunktion und ausreichender Blasenkapazität ohne anatomischen Defekt zu erzielen. In der Therapie der Rezidivstreßharninkontinenz stellt die Anwendung von intraurethralen Injektionen aufgrund der hohen Akzeptanz bei den Betroffenen eine, wenn auch noch in Entwicklung befindliche Alternative zu den hier in Frage kommenden und wesentlich invasiveren Operationstechniken dar.

    Notes: Zusammenfassung Bei einem 10jährigen Knaben kommt es im Rahmen einer Subsepsis allergica Wissler nach vorübergehender Besserung schließlich in der 10. Klinikaufenthaltswoche zur Blutungsneigung. Im Serum des Patienten konnten zu dieser Zeit vorübergehend unspezifische Antikörper (pos. Coombstest) und Hemmkörper nachgewiesen werden, die scheinbar zu der Blutungsbereitschaft Anlaß gaben. Gleichzeitig kam es im peripheren Blutbild zu einer vorübergehenden Reticulocytopenie, Leukopenie und im Knochenmark zum Auftreten von Riesenproerythroblasten mit deutlicher Verminderung der Erythroblasten. Diese erythroblastische Krise, die erstmalig von Gasser bei allergischen Erkrankungen beschrieben wurde, wird auf eine Reifungssperre, die scheinbar meist nur vorübergehend durch auftretende Autoimmunkörper ausgelöst wird und mit Änderung der Reaktionslage des Organismus wieder schwindet, zurückgeführt. Der klinische Verlauf und die erhobenen serologischen Befunde stützen diese Annahme und unterstreichen die allergische Genese der Subsepsis allergica Wissler. Der gleichzeitige Nachweis von Antikörpern, Reticulocytopenie und das Auftreten von Riesenproerythroblasten im Knochenmark läßt uns annehmen, daß diese sog. Reifungssperre durch eine vorübergehende Immunoreaktion-Antigen-Antikörperreaktion bedingt und erklärt werden könnte. Herrn Doz. Dr. Deutsch von der I.     Notes: Summary Five patients with carcinoma of the cervix and one with carcinoma of the vagina were examined by both cystoscopy and endosonography. Both techniques revealed malignant infiltration of the bladder in 4 patients. In the other 2 cystocopy showed an intact bladder mucosa despite cystosonographic evidence of invasion of the bladder wall by tumor.

    Notes: Summary In order to assess the incidence and type of urologic complications associated with Wertheim's radical hysterectomy we studied all 154 patients undergoing radical hysterectomy at our department between 1980 and 1986. More than 2/3 of patients had stage I carcinoma of the cervix, the mean age was 46.9 years (range 27–73 years). Various intraoperative reconstructive procedures were required in 5%. A comparison with the experience of Wertheim (before 1919) and that of a subsequent time period (1952–1967) showed that while the incidence of intraoperative injury to the lower urinary tract remained stable, the incidence of postoperative fistula formation decreased significantly (from 6.4% to 0.6%), possibly due to changes in the operative technique. We conclude that Wertheim's radical hysterectomy now involves only a very low risk of injury to the bladder or to the ureter.

    Notes: Abstract. Using introital sonography as an alternative to lateral chain urethrocystography during urodynamic studies, a prospective investigation was carried out in 47 patients with lower urinary tract complaints, to determine the correlation of sonographic assessment with and without bladder catheterisation. The apparent location of the bladder neck in relation to the pubic symphysis and measurements of the posterior urethrovesical angle at rest and during straining during cystometry with and without intravesical urodynamic pressure transducers were determined. In 22 (46.8%) patients the bladder neck at rest was below the lower edge of the symphysis. This finding was not influenced by the presence of a catheter. Correlation coefficients (r) for posterior urethrovesical angles at rest and during straining measured with and without catheters were 0.95 (95% confidence limits 0.92–0.98; P〈0.01) and 0.98 (95% confidence limits 0.96–0.99; P〈0.01), respectively. The coefficients of determination (r 2) at rest and during straining were 0.92 and 0.95, respectively. Sonography with and without cystometry equipment in place gave similar results.
