    Notes: Gleichzeitig auftretende Körperschäden, von denen mindestens zwei den klinischen Verlauf entscheidend beeinflussen, werden als Polytrauma definiert. Dabei wirken die einzelnen Solitärverletzungen hämodynamisch, metabolisch und immunologisch für den Organismus überbelastend und führen unter Umständen zum lebensbedrohlichen Multiorganversagen. Diverse Unfallstatistiken bezüglich des Schweregrads von Organverletzungen zeigen, daß Verletzungen, die das zentrale Nervensystem (ZNS) betreffen, allein oder in Kombination mit anderen Organverletzungen (z.B. im Bereich der Extremitäten oder des Abdomens) eine führende Rolle bei den Todesursachen einnehmen.

    Notes: Abstract Abdominal actinomycosis (AMC) is a rare infection in children. The appendix is the most common intra-abdominal organ involved. It presents as an undifferentiated mass, forming abscesses and fistulas in the right lower quadrant. The case of a 15-year-old girl with a AMC of the appendix detected by the pathologist after routine appendectomy is discussed. Long-term antibiotic treatment and follow-up by ultrasound and laboratory controls are necessary.

    Notes: Abstract Acquired tracheoesophageal fistula (aTEF) due to a decubital ulcer caused by an impacted foreign body is rare in the pediatric age group. The diagnosis of an aTEF is relatively easy by X-ray and endoscopy. Surgical intervention is difficult, and postoperative results are poor. We report an 11-year-old child with an aTEF caused by an ingested foreign body that was successfully treated without complication or recurrence.

    Notes: Abstract Since 1980, when we developed substitute for the internal anal sphicter, the smooth-muscle fold-over double-plasty (SMFD-plasty), we have operated on seven children using this method, usually in combination with the pull-through procedure, and on five children after one or more previous operations. Since 1983, 11 cases with previous operations have undergone the modified “door-wing” SMFD-plasty. The results are excellent if the operation is used in primary combination with the pull-through procedure or if the “door-wing” modification is combined with reconstruction of the pelvic floor. It is also necessary that all cases undergo a biofeedback conditioning program.
