    Notes: Summary A major disadvantage in the intracavernous application of a vasoactive drug is the need for the patient to prepare a syringe shortly before the injection. To improve drug administration, a pen for intracavernous self-injection has been developed. The Andropen consists of the pen mechanism and a disposable cartridge containing a combination of 15 mg papaverine hydrochloride and 0.5 mg phentolamine mesylate per milliliter. The injection volume is predetermined and controlled by a key available only to the physician to avoid involuntary overdosing.

    Notes: Summary Sixty unselected patients from two centers (40+20) with complaints indicating detrusor overactivity or hypersensitivity entered a prospective double-blind cross-over trial to test the efficacy of drug treatment. Two active drugs and a placebo were taken at random during consecutive 3-week periods. The active drugs used were emepronium bromide, which was given to all patients, and flavoxate hydrochloride (given to 30) and oxybutinin chloride (given to the other 30 patients). Anamnestic and cystometric data were recorded after each treatment period and compared with the values on admission. Effect was scored by patient and physician separately. Serious side effects were noted during the administration of emepronium bromide, while flavoxate hydrochloride had even fewer and milder side effects than placebo. Although no objective significant changes could be demonstrated, the total scoring results of oxybutinin chloride were better than that of the three other treatments. The other two active drugs scored no better than placebo.

    Notes: Summary In 26 patients with erectile impotence and 8 healthy volunteers the value of continuous monitoring of penile tumescence and rigidity by means of the RigiScan was tested. Based on history and routine screening tests the patients were divided into 3 groups of preliminary diagnosis: psychogenic (5 patients), organic (10 patients), mixed (11 patients). Real-time measurements of tumescence and rigidity were performed during direct visual stimulation and after intracorporeal injection of 80 mg papaverine hydrochloride according to a specific protocol. A total of 21 patients were monitored during sleep studies for 2 or 3 consecutive nights. The rigidity figures of the volunteers correlated well with their subjective interpretation of the erection. Evaluation of the recordings of the impotent patients enabled reclassification of the mixed group into 3 patients with mainly organic and 8 patients with mainly psychogenic impotence. RigiScan recording of penile tumescence and rigidity appears to be of great value in the diagnosis of impotent patients. Real-time monitoring during direct visual stimulation and after papaverine injection can generally replace nocturnal measurements. If the patient shows a positive response on visual sexual stimulation (VSS) alone, then his impotence is of psychogenic origin. Failing rigidity during VSS after injection indicates vascular impotence. In patients who show negative results during VSS alone, and positive response after injection or during subsequent VSS, nocturnal monitoring will differentiate between psychogenic and neurogenic impotence.

    Notes: Summary The present discussion on the natural history, growth rate, pathophysiology, morbidity, and possible treatment of benign prostatic hyperplasia (BPH) reinforces the need to objectify mechanical BPH-induced obstruction with pressure-flow study. Different methods for the quantification of BPH-induced obstruction exist and the grade of simplification depends on their working mechanism. To review the limitations of the current concepts, 118 BPH patients were selected for computerized pressure-flow data analysis according to quadratic passive urethral resistance relation (PURR) function. The CHESS classification differs from all other concepts due to its two-dimensional character and was developed as a result of the following way of thinking: (1) the whole individual pressure-flow plot is the most accurate basis for further analysis; (2) PURR as the true low-pressure flank of the individual plot is the most favorable definition for mechanical obstruction; (3) PURR quantification requires at least two parameters, footpoint and curvature/slope, with regard to the documented missing significant correlation of both; and (4) a two-parameter-based classification is two-dimensional (CHESS classification).

    Notes: Summary Patients in whom an abnormal rectal examination leads to the suspicion of prostatic carcinoma underwent both fine-needle aspiration biopsy for cytological evaluation and core biopsy for histological evaluation. The data from 826 paired observations, collected over a 7-year period are compared. The material available was inadequate for cytology in 94 cases (11.4%) and for histology in 18 cases (2.2%). Cytology results were classified as positive (Papanicolaou 4–5) or negative (1–3) for carcinoma, and histology results as indicative of malignancy or no proven malignancy. Initially the cytology results correlated with the histology results in 84.4%, with a sensitivity of 89.7% and a specificity of 81.5%. After revision of all discrepant slides these figures increased to 88.6%, 92.8% and 86.4%, respectively. Clinical or histological follow up of patients with discrepant results was conclusive in 36 patients, giving evidence of malignancy in 24 patients and of benign pathology in 12. In this “discrepancy” group, comparison of both cytological and histological results with these clinical data gave a sensitivity for cytology of 63% as against 37% for histology, and a specificity for cytology of 17%, as against 83% for histology. It is concluded that fine-needle aspiration biopsy cytology is a reliable, simple and sensitive technique that can be used to confirm a suspicion of prostatic carcinoma.

    Notes: Summary An on-line personal computer (PC)-based expert system was constructed for pressure-flow study of the lower urinary tract. An “empty” expert-system “shell” and supplementary programs were used for this system; the latter served to compensate the defects of the “shell” and are referred to as data programs in this report. The data programs were written in the C programming language and were used for on-line data sampling, data entry using a photoscanner, calculation of parameters, saving and loading of data, graphic display, and printing. The “urodynamic” pressure-flow expert system necessitates on-line data transfer, since it requires a large volume of data. However, off-line data recorded on paper was also made available for ready input by a photoscanner. Reasonable diagnostic accuracy has been achieved by our new expert system for pressure-flow studies. It should be useful in the clinical setting as well as for teaching urological students.

    Notes: Abstract A muscle model based on the sliding filament concept is put forward. The model is simplified in such a way that it may easily be simulated on a computer (analog or digital). The model shows quite a few muscle properties rather well e.g. Hill's relation, quick-stretch and quick-release series elasticity and some dynamic transfer properties. It is found that, using an antagonistic muscle model pair and an appropriate load, realistic arm movements can be simulated.

    Notes: Summary During the years 1981–1985, 251 female patients with stress incontinence consulted the department. The average age was 47.3 years. Surgical treatment consisting of cystourethropexy ad modem Marshall-Marchetti-Krantz, Stamey-Pereyra, or by fascial sling and/or periurethral injection with Teflon was performed in 185 patients, with a total of 215 procedures. In a retrospective study, factors which could possibly predict the results of the procedures were investigated. Obesity, parity, bladder capacity, detrusor compliance, maximum voiding pressure, and urethral instability showed no statistically significant relation to the success rate of the various procedures. Previous pelvic surgery or detrusor instability impaired the results in this group. The overall success rates of the 215 procedures were: Marshall-Marchetti-Krantz 76% (54 procedures), Stamey-Pereyra 65% (49), fascial sling 40% (35) and Teflon injection 43% (77). Restricting the results to those procedures that were primary in patients with genuine stress incontinence did not change the success rate for Marshall-Marchetti-Krantz: 74% (38), but showed increased success rates for the other procedures: Stamey-Pereyra 88% (34), fascial sling 64% (22) and Teflon injection 54% (52).
