    Description / Table of Contents: Résumé Une nouvelle méthode de dérivation du flux sanguin hépatique a été mise au point pour étudier l'influence des hormones pancréatiques et du flux sanguin hépatique sur la fonction de régénération du foie. Cette nouvelle technique n'emploie pas de greffe veineuse. La veine splénique est disséquée et anastomosée au segment proximal de la veine porte qui se distribue au foie droit alors que la veine mésentérique est disséquée et anastomosée à la branche gauche de la veine porte au niveau du hile du foie, laquelle se distribue au foie gauche. Des chiens de race “Mongrel” pesant environ 10 kg ont été employés. Le foie droit recevait le sang gastro-spléno-pancréatique alors que le foie gauche recevait le sang intestinal. Après résection de 40% du parenchyme hépatique à droite et de 10% du parenchyme hépatique à gauche, le volume du flux sanguin fut presque équivalent des 2 côtés. Dans ces conditions le poids du parenchyme du foie droit augmenta de façon significative par rapport à celui du foie gauche, alors que le taux d'extraction du vert-indocyanine au niveau desu eux lobes fut très voisin. A l'examen histologique le foie droit montra des signes typiques de régénération mais le foie gauche ne présenta pas de signe d'atrophie. Bien que les hormones pancréatiques aient été amenées au foie gauche par la voie de l'artère hépatique elles n'ont pas été dégradées. Les hormones pancréatiques se présentent comme un facteur susceptible de modifier l'importance de la régénération du parenchyme hépatique. L'étude quantitative et qualitative du foie régénérée est importante.
    Notes: Abstract A new hepatic blood flow diversion method was undertaken to investigate further the influence of pancreatic hormones and hepatic blood flow on the functional regeneration of the liver. The new technique does not use venograft. The splenic vein was dissected and anastomosed to the proximal portal vein, which supplied the right lobes, and the mesenteric vein was dissected and anatomosed to the left portal vein at the hilus of the liver, which supplied the left lobes. Mongrel dogs weighing approximately 10 kg were used. The right lobe received pancreatico-gastrosplenic blood. The left lobe received intestinal blood. After hepatectomy, which removed 10% from the right side and 40% from the left side, blood flow volume to the right and left lobes was almost equal. The right lobes increased in weight significantly more than the left lobes. The extraction rates of indocyanine green of the right and left lobes were not significantly different. The right lobe revealed typical regeneration histologically, but the left lobe showed no signs of atrophy. Although pancreatic hormones were sufficiently supplied to the left lobe through the hepatic artery, they were not degraded. Pancreatic hormones could be a modifying factor affecting the quantity of hepatic regeneration. The study of both the quantity and the functional quality of the regenerated liver is important.

    Notes: Abstract To determine whether the morphometric indices of hepatocellular carcinoma (HCC) correlated with the prognoses, the microscopic morphometric values for 84 HCC cases treated by hepatic resection were studied using an image analyzer in relation.to the survival rate and the gross classification. The mean survival time (MST) was 58 months in cases with a nucleocytoplasmic area ratio (N/C) of less than 0.28; this was significantly longer than the 38-month MST in cases with an N/C of more than 0.28 (P〈0.05). In stage III disease, the MST for cases with an N/C of less than 0.28 was 63 months, which was significantly longer than the MST of 13 months for cases with an N/C of more than 0.28. After relatively noncurative hepatic resection, the MST for cases with an N/C of less than 0.28 was 49 months, and this was significantly longer than the MST of 8 months for cases with an N/C of more than 0.28. The MST was 71 months for cases with a coefficient of variance of the nuclear form factor (NCV) of less than 5.5%, which was significantly longer than the MST of 33 months for cases with an NCV of more than 5.5% (P〈0.05). In stage III disease, the MST was 69 months for cases with an NCV of less than 5.5%, and this was significantly longer than the MST of 29 months for cases with an NVC of more than 5.5% (P〈0.05). In cases with an N/C of less than 0.28, 18% had vascular invasion and 38% had intrahepatic metastases, whereas in those with an N/C of more than 0.28, 62% had vascular invasion and 67% had intrahepatic metastases (P〈0.01,P〈0.05). Based on the results of these morphometric studies on HCC cases treated by hepatic resection, N/C and NCV may be useful as prognostic factors.

    Notes: Abstract Endoscopic assistance during terminal esophago-proximal gastrectomy (TEPG), was performed in 86 patients with portal hypertension to confirm the complete disappearance of esophageal varices and eradicate any remnants. The utility of endoscope assisted surgery (EAS), and the mechanisms of recurrent varices were thereby analyzed. Some postoperative recurrence is unavoidable after direct attack surgery as portal hypertension is often still present. Nevertheless, satisfactory disappearance was maintained in 56 cases (65 per cent) from 2 months to 9 years of follow up. Recurrent varices were observed in the other 30 cases, the causes presumably being: atrophy of the liver in 15 cases, thrombus formation in the portal vein in 10 cases, and various other causes in 5 cases; indicating increased resistance on the portal blood flow as a uniform mechanism. The episodes of rebleeding due to recurrent varices or staple ulceration in 8 cases were successfully treated by endoscopic injection sclerotherapy and medication. Actual proof of the disappearance of varices through EAS and an analysis of the mechanisms of recurrence are essential for improving the surgical techniques and treatment in cases of recurrent esophageal varices.

    Notes: Abstract In a 42 year old female, a fistula developed between the splenic vein and the pancreatic duct through the cavity of a pseudocyst in the tail of the pancreas and resulted in an extrahepatic portal hypertension. The fistula was visualized by endoscopic retrograde cholangiopancreatography and percutaneous transhepatic portography, then was successfully resected by surgery. The possible etiology of extended obstruction of both splenic and portal veins in chronic pancreatitis with pseudocyst was discussed.

    Notes: Abstract NADPH-protochlorophyllide oxidoreductase (POR) catalyzes the photoreduction of protochlorophyllide a in the chlorophyll biosynthetic pathway. Recently, two POR genes, the expressions of which were differently regulated by light, were isolated in barley and Arabidopsis, and it is suggested that in angiosperms, the chlorophyll synthesis is controlled by the differential action of the two distinct POR isozymes. Meanwhile, we have isolated POR cDNA from cucumber, and reported that its expression was enhanced by illumination. In this study, we analyzed the gene expression and organization of cucumber POR in more detail. Northern blot analysis with 5′- or 3′-noncoding gene-specific and full length probes and RNase protection assay revealed that cucumber POR mRNA is transcribed from a single gene. Run-on assay revealed that the transcript level of isolated nuclei from illuminated cotyledons was 6-fold that of dark control, indicating that the light-enhanced expression of cucumber POR is caused by transcriptional activation. Using degenerated primers and the genomic DNA of cucumber as template, PCR showed the presence of only one type of fragment encoding cucumber POR. The PCR product was hybridized with the genomic Southern blot from cucumber and only one band was detected under low stringency conditions. From these results, we conclude that there is a single POR gene in cucumber and PORs are organized by different gene families among higher plants, and in cucumber the single POR may play the roles of two POR genes, which are present in other higher plants such as Arabidopsis and barley.

    Notes: Oxidation of a polymer (1) having β-alkylmercaptoenoate moieties in the main chain by m-chloroperbenzoic acid (MCPBA) is described. Oxidation of 1 proceeded selectively to give polymers having vinyl sulfoxide or vinyl sulfone moieties in the main chain by using an appropriate amount of MCPBA. In the case of 1.1 equiv of MCPBA (relative to the sulfur atom), a polymer (4) having vinyl sulfoxide moieties selectively in the main chain was obtained in quantitative yield without side reactions.     Notes: Tri-n-butylphosphine-catalyzed polyadditions of activated internal diynes (bifunctional β-substituted propiolate, 1B and 1C) with diols are described. Although a terminal bispropiolate (1A) could not produce soluble polymers, with secondary diols, the polyaddition of 1B or 1C with primary as well as secondary diols gave corresponding polymers (3, only composed of E isomeric units) in high yield. The rate of the present polyaddition was estimated by a model reaction of benzyl alcohol with methyl 2-heptynoate (4), from which the introduction of alkyl groups at the β-position of propiolate moieties was found to decrease the rate of the reaction by 80 times. Furthermore, the rate-determining step on this polymerization system was speculated to be a protonation step of zwitterionic intermediates with protons from diols. © 1996 John Wiley & Sons, Inc.
