Summary Blood volume, total red cell mass and life span have been determined in 36 patients with biopsie proved myelo- or osteomyelosclerosis. Although 30 of the 36 patients showed a decreased peripheral PCV, in only 5 of these subjects was the total red cell mass significantly reduced. This seems partly to be due to an increased blood volume, which shows some relation to the degree of splenic enlargement. At any level of PCV, patients with moderate, marked or massive splenomegaly had a red cell mass greater by 13, 56 or 105 percent, respectively, than a control group of 125 subjets without a major degree of splenic enlargement. This increase was higher than expected by the blood volume changes and may be due to the stagnation of a large part of the red cell mass in splenomegaly. Further, the red cell life span was moderately reduced in 25 of 34 patients causing an accelerated turnover of the comperatively increased red cell mass. In three patients the effect of splenectomy on blood volume, red cell mass and life span have been estimated.

Zusammenfassung Bei 36 Patienten mit bioptisch gesicherter Myelo- und Osteomyelosklerose wurden Blutvolumen, Gesamtmenge zirkulierender Erythrocyten und Erythrocytenlebensdauer mittels51Cr markierten Erythrocyten bestimmt. Obwohl 30 der 36 Patienten einen erniedrigten peripheren Hämatokrit zeigten, war die gesamte Erythrocytenmenge nur in einem Siebentel der Patienten signifikant vermindert. Als Ursache dafür konnte erstens eine Verdünnungsanämie durch ein erhöhtes Blutvolumen nachgewiesen werden, das zur Größe der Milz in Beziehung stand. Zweitens fand sich eine relative Vermehrung der gesamten Erythrocytenmenge, die beim Vergleich mit einer Kontrollgruppe von 125 Personen ohne Milztumor über einen weiten Hämatokritbereich deutlich war und über einen reinen Verdünnungseffekt hinausging. Die Patienten mit hochgradiger Splenomegalie hatten zur Erreichung eines bestimmten Hämatokrites eine etwa doppelt so große Erythrocytenmenge notwendig als die Vergleichsgruppe ohne Milztumor. Die Verkürzung der mittleren Erythrocytenlebensdauer auf durchschnittlich die Hälfte führte darüber hinaus zu einem beschleunigten Umsatz dieser erhöhten Erythrocytenmenge. Bei drei Patienten wurde der Einfluß der Splenektomie auf Erythrocytenmenge, Blutvolumen und Erythrocytenlebensdauer festgestellt.
