    Notes: Abstract Changes in the operations and structure of local mental health centers are occurring in response to policy and funding shifts at the state and federal levels. Clinical and administrative staffing changes reflect both cutbacks in funding for traditional outpatient and inpatient services, as well as increases in partial care, community, residential and case management services. Centers are diversifying their funding sources, especially through increased revenues from Medicare-Medicaid sources. Governing boards remain active in center policy making, with few changes in their composition. Current adaptation strategies focus mainly on enhancing efficiency, reviewing service costs, expanding services to more viable markets, and improving business practices. These actions and strategies are compared to findings from previous studies, and their implications are discussed.

    Notes: Abstract Major policy and funding shifts over the last several years have stimulated a series of changes in local mental health agencies. Services offered by CMHCs became less comprehensive and more oriented toward chronic and severely ill patients. Decreases occurred in other services, primarily indirect services. Changes in services and in clients served caused reconfiguration in staffing patterns, and also affected the types of activities some staff perform. The implications of these service and staffing changes for local mental health agencies are discussed.

    Notes: Abstract State mental health authorities are currently the primary locus of service policy and funding decisions. In assuming increased authority and responsibility for service delivery, many states are emphasizing program development and funding for the severely acutely and chronically mentally ill while deemphasizing consultation/education, prevention, and evaluation activities. Funding mechanisms are moving towards greater accountability and funding levels are relatively stable. State authorities are exercising more control through regulations, but state-center relations are viewed as generally supportive and positive. There were few other consistent trends regarding changes in state/local relations, funding or regulatory patterns, or policy shifts except that almost all of the states interviewed reported instituting increased accountability mechanisms.

    Notes: Abstract The shift in policy and funding authority from the federal to the state level of government has caused local mental health centers to reconsider some of their operations. Actions aimed at altering center relations with the external environment are not undergoing radical changes, and executive directors are not significantly increasing their contacts with state authorities, community groups, or local service agencies. Relations with state mental health authorities were described as positive or improving, as was the influence of statewide associations of center directors. The implications of this lack of increased attention to external relations for long-term center viability are discussed.
