    Description / Table of Contents: Zusammenfassung Nach einer Literatur-und pathomorphologischen Übersicht wird an Hand von 23 Fällen einer spinalen Malformation unterschiedlicher Genese das klinische Bild der cranialen Migrationshemmung herausgearbeitet. Für die Diagnose ist neben Hautveränderungen über der dorsalen Mittellinie, wie sie für den dysrhaphischen Formenkreis typisch sind, die neurologische Symptomatik der unteren Extremitäten richtungweisen. Der Beweis einer Aszensionshemmung kann nur durch die Myelographie erbracht werden. Die Bedeutung frühzeitiger operativer Maßnahmen zur Verhinderung progredienter Ausfälle wird besonders herausgestellt.
    Notes: Abstract After a review of the literature on, and the pathology of spinal malformations the clinical features of a restricted ascent of the spinal cord are described with reference to 23 cases of spinal dysrhaphism of varying genesis. The suppression of cranial migration is caused either by a diastematomyelia, a lipoma, or a dermoid sinus with direct connection to the spinal cord, or else by myelomeningoceles and cicatrices after arachnitis. The symptoms occur particularly during the early years of life and in the age group of between 20 and 30. The manifestations of the disease are changes over the dorsal midline which are typical for spinal dysraphism, neurological defects of the lower limbs, and abnormalities in the function of bladder and rectum. Deformations of the feet and trophic disturbances have been encountered. Important for the diagnosis is a radiographic investigation of the spine since a restriction of the cranial migration can only be proved by myelography. Operation with view of a release of the spinal cord is the method of choice. The importance of early operative measures for the prevention of progredient deficiencies is particularly stressed.

    Description / Table of Contents: Zusammenfassung An Hand der klinischen Analyse von 100 Fällen mit cystischen Dysrhaphien wird die Häufigkeit des begleitenden Hydrocephalus aufgezeigt. Es bestehen statistisch signifikante Beziehungen zwischen Lokalisation und Art der Dysrhaphie und des Kopfumfangs bei Geburt einerseits und der Entwicklung des Hydrocephalus andererseits. Die kombinierte operative Therapie der Dysrhaphie und des Hydrocephalus wird befürwortet. Unter besonderen Voraussetzungen ist die operative Hydrocephalusbehandlung als Erstmaßnahme vor dem spinalen Eingriff angezeigt. Auf die große diagnostische Bedeutung der Echoencephalographie wird verwiesen und die Möglichkeit, eine Liquorpassagestörung durch die Liquorszintigraphie nachzuweisen, aufgezeigt.
    Notes: Summary A study was conducted to evaluate the clinical data of one hundred cases of cystic dysraphia in order to elucidate the frequency of hydrocephalus as a concomitant symptom. It was found that there is a statistically significant correlation between type and localisation of the dysraphia and the cirumference of the head at birth, and the development of a hydrocephalus. A combined operative therapy of dysraphia and hydrocephalus is advocated. In special circumstances an operative treatment of the hydrocephalus should even precede the spinal operation. The diagnostic importance of echo-EG for the diagnosis is particularly stressed, and the possibility to diagnose an obstruction of the CSF passage by scintigraphy is pointed out.

    Description / Table of Contents: Zusammenfassung An Hand des eigenen Krankengutes wird, unterstützt durch Literaturangaben, ein Überblick über die Problematik der Medulloblastome des Kleinhirns gegeben. Es erfolgt unter Zugrundelegung des neurologischen und psychischen Befundes eine Stadieneinteilung, die für die Operations-prognose von Bedeutung ist. Die möglichst frühzeitige Erkennung der Erkrankung für die Verbesserung der Frühprognose wird besonders herausgestellt. Die schlechte Spätprognose konnte bisher trotz kombinierter operativ-radiologischer Therapie nicht wesentlich beeinflußt werden. Dauerheilungen gibt es praktisch nicht. Die Methode der Wahl ist nach ausgiebiger operativer Entlastung, falls notwendig unter Drainage des Hydrocephalus, eine Herdbestrahlung mit 3000–6000 R, unterstützt durch eine spinale Liquorraumbestrahlung von 2000–3000 R und evtl. die cerebrale Liquorraumbestrahlung.
    Notes: Summary The problems of cerebellar medulloblastomas are demonstrated again on the survey of 53 cases of the own collective and by literature. On the base of neurological and psychic findings it is made a classification of tumors with reference to the results of operation. Early recognition of the tumor is very important to the improvement of primary prognosis. Hitherto the bad prognosis in late sequella could not absolutely influenced by combined therapy. Cure of tumor seems to be impossible. Today's therapy is operation as radical as possible if necessary drainage of hydrocephalus by Pudenz — Heyer methode and a roentgen radiation focal with 3000–6000 r, the spinal fluid spaces with 2000–3000 r and eventually the cerebral ventricles.

    Description / Table of Contents: Summary After setting out technical and ballistic details of small arms and after a clinical introduction, a series of 20 personal cases with gun-shot wounds of the head are described. Sixteen patients had direct involvement of the brain, of whom 12 died. The importance of a knowledge of the weapon used and of the course of the missile as seen in the radiographs is referred to. It is demonstrated that in these patients the level of consciousness, the neurological symptoms and the degree of shock are of especial importance in assessing prognosis.
    Abstract: Résumé Après un bref rappel des détails techniques et ballistiques des armes légères, et après une introduction clinique, une série de 20 cas personnels de blessures cranielles par coup de fusil est rapportée. Sur 16 malades qui ont eu des lésions cérébrales: 12 sont décédés. L'auteur insiste sur l'importance de la connaissance de l'arme utilisée et du trajet de la balle vu sur les clichés. Chez ces malades, le niveau de conscience, les signes neurologiques et le degré du choc ont une grande importance dans le pronostic.
    Notes: Zusammenfassung Nach Darlegung technischer und ballistischer Einzelheiten über Handfeuerwaffen und eine klinische Einleitung wird über 20 eigene Fälle mit Schädelschußverletzungen berichtet. 16 Verletzungen gingen mit direkter Gehirnbeteiligung einher, 12 von denen verstarben. Es wird auf die Wichtigkeit der Kenntnis der Tatwaffen und des Schußkanalverlaufs im Röntgenbild verwiesen, und die besondere Bedeutung der Bewußtseinslage, der neurologischen Symptomatik und der Schocksituation zur prognostischen Bewertung dieser Verletzungen herausgestellt.

    Notes: Scarring impedes axon regrowth across the lesion site and is one major extrinsic constraint to effective regeneration in the adult mammalian central nervous system. In the present study we determined whether specific biochemical or immunochemical modulation of one major component of the scar, the basal membrane (BM), would provide a means to stimulate axon regeneration in the mechanically transected postcommissural fornix of the adult rat. Basal membrane developed within the first 2 weeks after transection in spatiotemporal coincidence with the abrupt growth arrest of spontaneously regrowing axons. Local injection of anticollagen IV antibodies or α, α′-dipyridyl, an inhibitor of collagen triple helix formation and synthesis, significantly reduced lesion-induced BM deposition. This treatment allowed massive axon elongation across the lesion site. Anterograde tracing provided unequivocal evidence that regenerating axons follow their original pathway, reinnervate the appropriate target, the mammillary body, and become remyelinated with compact myelin. Presynaptic electrophysiological recordings of regenerated fibre tracts showed recovery to nearly normal conduction properties. Our results indicate that lesion-induced BM is an impediment for successful axonal regeneration and its reduction is a prerequisite and sufficient condition for regrowing axons to cross the lesion site.
