    Notes: Summary This study compares the results of a new computer assisted evaluation programm of sonographic images of the prostate with the histological examination of the operative specimen. This computer program is based on the fact that the human eye is not capable of distinguishing picture distribution points which differ in higher than second order statistics. The sensitivity of the method was 89%, the specificity 86% and the diagnostic accuracy 88%. The results are discussed by comparing them to results of transrectal sonography and digital palpation as reported in the literature.

    Notes: Summary The response of LH and FSH to synthetic gonadotropin releasing factor (GRF) was investigated in 19 patients with malignant germ cell cancers of the testicle prior to radical orchiectomy. The study showed: 1. Patients with circulating beta-HCG presented with increased plasma levels of oestradiol. Base line FSH and response to GRF were significantly decreased. 2. In patients without detectable beta-HCG plasma concentrations of oestradiol and testosterone were within the normal ranges as compared to healthy age matched controls. Base line levels of FSH and LH were increased and an exaggerated response to GRF was observed. From the results of this study it can be concluded that hypergonadotropic dysfunction of pituitary-gonadal axis exists in patients with testicular cancer of germ cell origin. Beta-HCG production by tumour tissue results in hyperoestrogenism and interferes with the pituitary-gonadal axis in terms of inhibition of pituitary gonadotropin release.

    Notes: Abstract Casodex is an orally active non-steroidal antiandrogen that is highly selective for androgen receptors in animals and man. It is indicated for the non-surgical treatment of advanced prostate cancer in man. The present open controlled study in 13 Casodex-treated and 21 orchidectomy-alone (control) patients addressed the hypothesis that chronic administration of antiandrogens will result in Leydig cell hyperplasia as a result of feedback inhibition of the pituitary resulting in increased luteinising hormone (LH) stimulation of Leydig cells. Although Casodex has been shown to produce a moderate rise in circulating plasma testosterone concentration on chronic treatment in prostate cancer patients, a controlled histopathological and morphometric assessment of the testis following orchidectomy in relapsed Casodex patients showed no effect on Leydig cell populations compared with an orchidectomy alone (control) group. No evidence for induction of Leydig cell hypertrophy or hyperplasia as a result of chronic oral administration of 50 mg Casodex daily was obtained in this study.

    Notes: Summary The effects of long term GnRH treatment with the biodegradable depot formulation of ICI 118.630 on hormone levels and spermatogenesis were investigated in 18 males with advanced prostate cancer. Plasma levels of FSH, LH, testosterone, DHEA-S and SHBG were monitored at regular intervals. The drug suppressed FSH, LH and testosterone significantly and did not affect DHEA-S and SHBG plasma levels. Tissue specimens for histologic assessment and quantitative analysis of germinal cell types were obtained at secondary orchidectomy in 16 patients immediately following GnRH analogue treatment. Germinal cell maturation was arrested at the spermatogonial stage. In two patients discontinuing treatment histologic assessment of secundary orchidectomy specimens 9 and 10 months after the last GnRH analogue depot injection resulted in germinal cell maturation to late spermatides in part of the tubule cross sections. These results indicate that long term administration of the GnRH analoge fails to produce complete testicular sclerosis and spermatogenic arrest might be reversible.

    Notes: Summary Plasma prolactin was measured in 10 patients with prostatic cancer during treatment with cyproterone acetate (300 mg/week i.m.) Prolactin was assayed during a six month period at weekly intervals during the first 4 weeks and then at monthly intervals. Orchiectomy was not carried out. After 6 months prolactin levels were elevated compared with pre-treatment levels. It is concluded from this study that cyproterone acetate interferes with prolactin secretion by the pituitary gland.

    Notes: Abstract Stimulation response of the pituitary to LHRH and testicular biopsy were investigated in 17 infertile men in comparison to 10 healthy controls. A significant elevation of basal FSH and LHRH-stimulated FSH serum levels was observed in Sertoli cell only syndrome and spermatogenetic arrest, while LH and testosterone remained normal. The LHRH test seems to be of value for differentiation of pretesticular, testicular and posttesticular infertility.

    Description / Table of Contents: Zusammenfassung In der Lamina propria menschlicher Hodenkanälchen wurden spezialisierte Fibroblasten beobachtet. Die Zellen sind durch Bündel parallel geordneter Plasmafilamente gekennzeichnet, deren Durchmesser rund 80 Å beträgt. Die Filamentbündel verlaufen parallel zur Oberfläche der lamellär ausgebreiteten Zellen und inserieren in elektronendichtem, granulärem Material, das der Innenseite der Zellmembran anliegt. Es wird angenommen, daß dieses Fibrillensystem kontraktil ist; die dichtere oder lockerere Vernetzung der Filamente innerhalb der Bündel würde dem kontrahierten oder erschlafften Zustand der Zellen entsprechen. Die beschriebenen spezialisierten Fibroblasten sollten der Gruppe der Myofibroblasten zugeordnet werden (Gabbiani, Ryan und Majno, 1971).
    Notes: Summary A special type of fibroblast is observed in the connective tissue of seminiferous tubules in human testes. These cells are characterized by a fibrillar system consisting of parallel arranged cytoplasmic filaments. The filaments have a mean diameter of 80 Å. Bundles of filaments run parallel to the surface of the flattened cells. The filaments insert in a dense, granular material which is connected with the cell membrane. This fibrillar system is thought to be contractile; the dense or more loose texture of the filaments within the bundles may correspond to the contracted or relaxed state of the cell. The specialized fibroblasts described are supposed to belong to the group of myofibroblasts (Gabbiani, Ryan and Majno, 1971).

    Notes: Abstract Enzyme-histochemical investigations performed on testicular biopsies of 20 patients with various testicular disorders gave the following results: 1. Severe hypospermatogenesis and spermatogenetic arrest were accompanied 2. In the cases of germinal aplasia no alkaline phosphatase activity was present in the Sertoli cells. 3. The other enzymes investigated were not noticeably changed in comparison with the control group. 4. The number of Leydig cells seemed to be increased in some cases and they appeared as clusters between the atrophic tubules. There was no reduction of enzyme activity within these cells. Possible pathogenetic mechanisms are discussed.

    Notes: Abstract The effect of a polyene macrolid compound Nystafungin on serum luteinizing hormone, plasma cholesterol and the spermiogram of 14 men with prostate adenoma was studied. Contrary to animal experiments no alternations of prostatic excretion and sperm count was noted. Plasma cholesterol and LH remained normal. Interference of polyene macrolid in man with hormone status does not seem to exist.
