Summary Regarding operative indication as well as operative technique in posttraumatic epilepsy one has to distinguish between the connective-tissue-scar and the more glious scar and the scar after closed injury. With the connective-tissue-scar operative indication should be guided not by epileptic symptoms only. Apart from the success of drug treatment operation should be undertaken as soon as possible. In contrast to this operation in cases with the glious scar and with closed brain injuries is only the last attempt of a cure, which is only permitted after failure of conservative drug treatment consequently carried through. Operation in cases with connective-tissue-scars should be limited to excision of the brain tissue, that is macroscopically alterated, electrical foci without macroscopical alterations should not be included. With glious scars or after closed injury one must be guided by the EEG and the ECG. No final principles can be given. Results of surgical treatment of posttraumatic epilepsies are without any doubt encouraging. But unfortunately it is impossible to give a sure or even a sufficiently probable prognosis for the individual case before operation.

Résumé Dans l'indication aussi bien que dans l'intervention, l'épilepsie posttraumatique exige une distinction très nette entre la cicatrice fibreuse, la cicatrice glieuse et celle après traumatisme fermé. En cas de cicatrice fibreuse, l'indication pour l'intervention n'est pas seulement conditionnée par les crises épileptiques. Elle doit être faite le plus tôt possible, indépendant du succès du traitement aux médicaments. L'intervention en cas de cicatrice glieuse et de traumatisme cérébral fermé n'est toutefois que le dernier secours qui peut uniquement être utilisé si le traitement médical a écouché. L'intervention en cas de cicatrice fibreuse devrait se limiter à l'extirpation du tissu cérébral atteint d'une altération macroscopique. L'extirpation de foyers électriques sans altération macroscopique n'est pas indiquée. La cicatrice glieuse et la lésion fermée sont à juger d'après l'E. E. G. et l'E. C. G. Les auteurs ne peuvent toutefois pas donner des règles définitives. Certes, les succès du traitement opératoire de l'épilepsie posttraumatiques sont bien encourageants. Dans le cas particulier, il n'est toutefois pas possible de faire un pronostic avec assez de certitude avant l'opération.

Zusammenfassung In der Indikation wie in der operativen Ausführung ist bei der posttraumatischen Epilepsie streng zu scheiden die bindegewebige Narbe von der mehr gliösen und der Narbe nach gedeckter Verletzung. Bei der bindegewebigen Narbe wird die Indikation zur Operation nicht nur durch das Anfalleiden allein bedingt. Sie sollte so bald als möglich gestellt werden, unabhängig vom Erfolg der medikamentösen Behandlung. Die Indikation bei der gliösen Narbe und bei der gedeckten Hirnverletzung ist dagegen immer nur ein letzter Versuch, der nur dann gestattet ist, wenn eine konsequent durchgeführte konservative medikamentöse Behandlung versagt hat. Die Operation sollte sich bei der bindegewebigen Narbe auf die Entfernung des makroskopisch veränderten Hirngewebes beschränken. Die Entfernung makroskopisch nicht veränderter elektrischer Krampfherde erscheint nicht angezeigt. Bei der gliösen Narbe bzw. nach der gedeckten Verletzung muß man sich vom Elektroencephalogramm und vom Elektrokortikogramm leiten lassen. Endgültige Richtlinien vermögen wir nicht zu geben. Die Erfolge der operativen Behandlung der posttraumatischen Epilepsie sind zweifellos ermutigend. Eine sichere oder nur auch genügend wahrscheinliche Prognose kann aber leider vor der Operation im Einzelfalle nicht gestellt werden.
