    Notes: Fifteen cases of desmoplastic melanocytic naevi are described in 12 female and three male patients, ranging in age from 8 to 83 years. The commonest site for the lesions was the upper limb. The lesions were characterized by a dense sclerotic or desmoplastic appearance of the dermal collagen surrounding a population of predominantly epithelioid naevus cells, some of which had a bizarre but non-malignant cytology. The cause of the stromal change is not established. We suggest that these lesions constitute a discrete entity which should be described in standard textbooks of dermatopathology.

    Notes: One hundred and twelve melanocytic naevi measuring less than 5 cm in largest diameter were excised from 85 patients referred because of minor concern about possible growth or malignant potential of these naevi. Sections from all 112 naevi were examined histologically, without access to the clinical history, and the following features noted: type of melanocytic proliferation in the basal layer area, contiguity of epidermal and dermal components, total width and depth of naevus, involvement of subcutaneous fat and of skin appendages, the presence or absence of an ‘Indian file’ type of naevus cell infiltration, cellular atypia and evidence of maturation of these cells. On the basis of these features, naevi were assigned to either a ‘congenital type’ or ‘acquired type’ classification, and their distribution in these two classifications then compared with the clinical history. It was found that in 107 of 112 lesions correct assignation had been made on histological grounds alone, 43 as congenital and 64 as acquired. A histological pattern not previously reported to be associated with congenital naevi was seen in 17 (34%) of the congenital lesions. This study suggests that it is possible to differentiate the majority of small congenital naevi from acquired naevi on histopathological grounds alone. This observation could be of considerable value in the examination of excised primary malignant melanomas for evidence of a pre-existing naevus of either type.

    Notes: Adhesion molecules are considered to have an important role in inflammatory reactions. We investigated the kinetics of ICAM-1 expression on keratinocytes and correlated this with the numbers of lymphocytes expressing LFA-1 in the dermis and epidermis of evolving allergic contact dermatitis reactions. In nickel-sensitive individuals, after application of a nickel patch, increased expression of ICAM-1 on keratinocytes was observed as early as 3 h and reached a maximum at 48 h. The number of lymphocytes expressing LFA-1 in the dermis and epidermis was greatest at 48 h. The LFA cells were observed to be in close proximity to keratinocytes expressing ICAM-1, thus supporting the hypothesis that T-lymphocytes attach to keratinocytes via LFA-1/ICAM-1 molecules.

    Notes: Counts of melanocytic naevi of 2 mm or greater diameter on all body sites were conducted in 197 Caucasian adults in Scotland. Mean naevus numbers were greater in young than in older adults, and in females than in males. Naevus counts on the upper limbs, lower limbs, and trunk were strongly and significantly correlated with each other and with whole body counts. Thus, where whole body counts are impractical, counting naevi at any of these individual sites should provide a satisfactory method for studying naevus distribution in large populations.The rapid rise in the incidence and mortality of malignant melanoma over recent years has led to increased interest in the epidemiology of malignant melanoma and especially its relationship to melanocytic naevi. Several recent case-control studies have shown that naevus counts are a strong risk factor for melanoma,1–4 but relatively little is known about the distribution of and the risk factors for naevi. There have been few studies of whole body naevus counts in populations; such counts are time consuming and can be difficult to conduct in non-hospitalized patients. In the present study we have examined the relationship between naevus numbers at different sites, and also between site-specific naevus counts and whole-body counts, in a Caucasian population in Scotland.

    Notes: Total body mole counts have been performed on 432 normal healthy Caucasian subjects aged 4 days to 96 years (204 males and 228 females). The mean total body mole count in the first decade of life is three for females and two for males, rising rapidly in the second decade to a mean of 23 for females and 18 for males. In the third decade numbers are highest, with a mean of 33 for females and 22 for males. Thereafter, numbers of moles slowly drop until in the eighth decade they have fallen to levels similar to those seen in pre-pubertal children. There is a significant association between the presence of naevi in early childhood (prior to 5 years of age) and the development in later life of large numbers of moles. In women there is no significant association between total mole counts and either parity or use of the oral contraceptive.

    Notes: Background  Prepubertal malignant melanoma is rare, pathological criteria are difficult and follow-up data on patients are lacking in the literature.Objectives  To review prepubertal cases of melanoma diagnosed in the West of Scotland 1979–2002.Methods  Twenty cases were identified in whom melanoma was diagnosed before the age of 15. Pathological review was possible for 13 of 20 cases, and current follow-up information is available for all 20. Three pathologists not responsible for the original diagnosis reviewed the slides independently, in every case without knowledge of the outcome.Results  Of the 13 cases reviewed, there was concordance of diagnosis between the three pathologists in 12 cases. Eight of the 13 cases reviewed were considered to be unusual naevi rather than melanoma. One child has died of melanoma and all three pathologists agreed with the original pathological diagnosis. One patient has experienced nodal metastases but is alive and disease-free 12 years later. The remaining 18 cases have had no recurrence since primary surgery 2–21 years ago.Conclusions  There may be a tendency to overdiagnose prepubertal melanoma. Good communication between clinician and pathologist and the use of an expert pathology panel is recommended before making the diagnosis.

    Notes: Background There is a current need for a reliable prognostic marker for melanoma patients, particularly those with stage 2 and stage 3 disease, so that adjuvant therapies can be directed appropriately. Objectives To establish whether or not the use of tyrosinase-specific or melanA/MART-1-specific reverse transcriptase-coupled–polymerase chain reaction (RT–PCR) of peripheral blood cells detects preclinical disease progression in patients with malignant melanoma. Methods Two hundred and ninety-nine patients with melanoma in clinical stages 1–4 were observed in this study. Samples were obtained sequentially from 153 of these patients at 4-week intervals over a period of up to 2 years and correlated with clinical evidence of disease activity. Tyrosinase and melanA/MART-1 amplicons were analysed by agarose gel electrophoresis and Southern blot hybridization subsequent to a single round of amplification. Results We demonstrated a statistically significant increase in tyrosinase RT–PCR positivity with advancing stage of melanoma progression. The percentage tyrosinase positivity in 910 samples tested was: stage 1, 135 samples, 34% positive; stage 2, 196 samples, 51% positive; stage 3, 423 samples, 50% positive; and stage 4, 156 samples, 65% positive. The positivity rate for individual patients tested sequentially was higher if only one positive test was required to label a patient positive, at 42%, 65%, 82% and 81% for patients in stages 1–4, respectively. However, we did not find a clear pattern of conversion from negativity to positivity in patients who progressed during the study from stage 2 to stage 3 or stage 3 to stage 4, and found no clear evidence of increased positivity rates in the 6-week period following melanoma-related surgery in patients with stage 3 and 4 disease. The positivity rate for melanA/MART-1 was lower for both patients and samples, and no melanA/MART-1-positive sample was negative for tyrosinase. Conclusions We conclude that the presence of circulating tyrosinase-positive cells as detected by this method appears to be a discontinuous rather than a continuous phenomenon, even in patients with stage 4 disease. For this reason the assay cannot be recommended as a method of sequentially monitoring individual patients in a clinical setting.

    Notes: Summary The incidence of basal cell carcinoma and squamous cell carcinoma (non-melanoma skin cancer) is increasing in the U.K., and the importance of this has been recognized in the ‘Health of th Nation’ target of halting the annual increase in the incidence of skin cancer by the year 2005. An accurate assessment of incidence is necessary both in meeting this target and in planning skin cancer services. We have examined the ways in which basal cell carcinoma and squamous cell carcinoma are diagnosed and treated in Greater Glasgow and have determined how many of these tumours are recorded by the West of Scotland Cancer Registry. Our results show that there is under-registration of both basal cell carcinoma and squamous cell carcinoma. Overall, 39 of 127 basal cell carcinomas (31%; 95% confidence interval [CI] 23–39%) and 11 of 25 squamous cell carcinomas (44%; CI 26–63%) were not registered by the cancer registry. We also showed that dermatologists rarely treat clinically suspicious tumours without obtaining pathological proof of the diagnosis. Accurate data collection by selected representative cancer registries is suggested as a possible solution.
