    Notes: Abstract Growth impairment and growth hormone (GH) deficiency have been reported in children treated for acute lymphoblastic leukaemia (ALL). We have studied growth and GH secretion in a group of 50 patients, affected by ALL, during a 2- to 5-year period after diagnosis, and in 12 “long-term-survivors”. We observed a significant decrease in growth velocity during the 1st year (in particular during the first 6 months) of therapy and a catch-up growth after the end of therapy. “Longterm survivors” did not exhibit a significant reduction of height standard deviation score (SDS), as compared to height SDS at diagnosis. None of the patients showed GH deficiency. Our data indicate that chemotherapy significantly affects growth of patients treated for ALL, whereas radiotherapy-at the doses used in this study-does not induce GH deficiency, at least not within 9 years after diagnosis.

    Notes: Summary The study describes the advantages of using the positioning of the prosthesis in the pre-peritoneal space by a posterior approach in the repair of inguinal hernia. The research was carried out on 388 patients (372 (96%) men and 16 (4%) women), who had undergone hernioplasties with the insertion of a polypropylene prosthesis by a preperitoneal approach, between January 1994 and December 1996. 216 of these patients (56%) had an indirect inguinal hernia, 94 (24%) a direct inguinal hernia and 78 (20%) a double inguinal hernia (direct and indirect). In 21 cases (6%) it was a recurrent hernia. The following clinical parameters were considered: pain, sensitivity of the inguinal region, testicular pathology, wound pathology and recurrence. 384 patients out of 388 (99%) were discharged at latest one day after the operation. On average the patients went back to work 6 days after the operation. All the patients were checked at least three times out of four, with follow-ups at 30 days, 6 months, 1 year and 2 years after the operation. In 13 patients (1.8%) there was a minor complication, in 3 patients (0.8%) we found an early recurrence due to technical error. We consider that positioning the prosthesis in the preperitoneal space using this minimally invasive procedure can be carried out easily and with very good clinical results in terms of typical early and/or late complications, which range from 2% to 20% by the anterior approach.

    Notes: : Recent studies suggest that the pineal hormone melatonin may reduce chemotherapy-induced immune and bone marrow damage. In addition, melatonin may exert potential oncostatic effects either by stimulating host anticancer immune defenses or by inhibiting tumor growth factor production. On this basis, we have performed a randomized study of chemotherapy alone vs. chemotherapy plus melatonin in advanced non-small cell lung cancer patients (NSCLC) with poor clinical status. The study included 70 consecutive advanced NSCLC patients who were randomized to receive chemotherapy alone with cisplatin (20 mg/m2/day i.v. for 3 days) and etoposide (100 mg/m2/day i.v. for 3 days) or chemotherapy plus melatonin (20 mg/day orally in the evening). Cycles were repeated at 21-day intervals. Clinical response and toxicity were evaluated according to World Health Organization criteria. A complete response (CR) was achieved in 1/34 patients concomitantly treated with melatonin and in none of the patients receiving chemotherapy alone. Partial response (PR) occurred in 10/34 and in 6/36 patients treated with or without melatonin, respectively. Thus, the tumor response rate was higher in patients receiving melatonin (11/34 vs. 6/35), without, however, statistically significant differences. The percent of 1-year survival was significantly higher in patients treated with melatonin plus chemotherapy than in those who received chemotherapy alone (15/34 vs. 7/36, P 〈 0.05). Finally, chemotherapy was well tolerated in patients receiving melatonin, and in particular the frequency of myelosuppression, neuropathy, and cachexia was significantly lower in the melatonin group. This study shows that the concomitant administration of melatonin may improve the efficacy of chemotherapy, mainly in terms of survival time, and reduce chemotherapeutic toxicity in advanced NSCLC, at least in patients in poor clinical condition.

    Notes: The microbiological quality of water, sediments and seafood products of three Sicilian marine fish farms (Castellammare Gulf, Capo d'Orlando and Porto Palo) was investigated in order to draw a complete picture of the health conditions from the start (environment) to the end (seafood product) of the productive cycle. Before the beginning of fish farming, low concentrations of faecal contamination indicators (faecal coliforms, Escherichia coli, enterococci) were found in the water of each examined area. Due to the enhanced organic load released from cages, the set-up of fish farming significantly altered the distribution of faecal indicators and sometimes that of halophilic vibrios in the pelagic compartment. Significant differences in the density of heterotrophic bacteria were sometimes recorded at the sediment level. Despite this increase in microbial abundance, the microbiological conditions remained acceptable for the productive process. Pathogens (Salmonella spp., Vibrio parahaemolyticus, Listeria monocytogenes, Staphylococcus aureus) were mostly absent in seafood products. The study underlines that the achievement of good quality levels in aquaculture strongly depends on the conformity of the rearing environment to qualitative microbiological standards, highlighting the importance of sanitary controls along the different steps of the productive cycle.

    Notes: Abstract The incidence of type I (insulin-dependent) diabetes was determined in the district of Catania (eastern Sicily) in children under 15 years of age over a 3-year period (1 January 1989 to 31 December 1991). Two independent sources of information were used. The primary source was contact with all medical services in the province, and the secondary source was the personal identification cards issued to all diabetic patients by the National Health System necessary for obtaining free medical care. The information obtained was 99.6% complete. The overall incidence was 10.2/105 per year. This study provides the first standardized data on the incidence of type I diabetes in Sicily and is consistent with the possibility of regional deviations from the proposed north to south gradient.

    Notes: Abstract Recent studies indicate an important role of endogenous oxidative stress in the onset and/or in the progression of mitochondrial encephalomyopathies. In particular, the increased production of radical species caused by altered mitochondrial functionality could affect both mitochondrial and nuclear DNA integrity. We performed the micronucleus assay coupled with fluorescence in situ hybridization (FISH) to detect chromosome damage in peripheral blood lymphocytes in a group of patients affected by different forms of mitochondrial encephalomyopathies. Moreover the comet assay has been carried out to detect primary and oxidative damage in the nuclear DNA. Our results show a siginificant presence of both DNA and chromosome damage in patients compared to a matched group of controls. A reduction in DNA alterations is also observed in patients after treatment with coenzyme Q10.

    Notes: Abstract An impairment of mitochondrial function may contribute to the pathophysiology of myotonic dystrophy (MyD). Coenzyme Q10 (CoQ10) deficiency has been previously observed, even if in an restricted sample of patients. The aim of this investigation was to obtain more information about coenzyme Q10 and its relationships to the aerobic metabolism in a group of MyD patients. Serum CoQ10 appeared significantly reduced with respect to normal controls: 0.93±0.22 vs. 1.58±0.28 μg/ml (p〈0.05). Moreover, the results demonstrated an inverse tendency between CoQ10 levels and the CTG expansion degree. Basal blood lactate levels were significantly higher than controls (p〈0.05). A borderline inverse correlation between CoQ10 and lactate, corresponding to lactate threshold, was found. These data suggest a possible role of CoQ10 in the pathogenesis of MyD, which may be mediated by mechanisms of cellular damage common to the oxidative pathway. Therapeutic strategies may be devised by virtue of this rationale.

    Notes: Abstract Human mitochondrial transcription factor A (h-mtTFA or Tfam), a 25-kDa protein encoded by a nuclear gene located on chromosome 10q21, is involved in the control of replication and transcription of mtDNA. To better understand the complex relationship between h-mtTFA and mitochondrial function, we assessed by western blot the levels of h-mtTFA in skeletal muscle from 7 patients affected by mitochondrial diseases (MD) caused by different mtDNA mutations. We related these results to exercise lactate production as well as to the pathologic features of the underlying myopathy. h-mtTFFA levels were significantly inversely related to blood lactate and the percent of RRF, borderline to cox negative fibers. Our results indicate that h-mtTFA may be involved in the cascade of events which determine functional impairment of MD.
