Zusammenfassung 1. Es wird über alle Fälle mit Subarachnoidalblutung berichtet, die in die Neurochirurgische Klinik der Universität Mailand zwischen dem 1. Januar 1964 und dem 30. Juni 1966 eingewiesen wurden. Die Langzeitergebnisse werden dargestellt. (Aus dem Bericht sind lediglich die Fälle mit Subarachnoidalblutung ausgeschlossen worden, bei denen eine arteriovenöse Gefäßmißbildung als Ursache nachgewiesen werden konnte.) 2. Bei Fällen mit sackförmigem arteriellem Aneurysma wurde niemals eine konservative Behandlung bevorzugt, wenn der Zustand des Patienten eine Operation erlaubte. Die Gesamtzahl der Fälle war 192. Bei 119 (62%) konnte angiographisch ein intrakranielles sackförmiges Aneurysma als Blutungsquelle nachgewiesen werden. Bei 64 Patienten wurde das Aneurysma direkt durch intrakraniellen operativen Eingriff angegangen. Die Langzeitergebnisse der operierten bzw. nicht operierten Fälle waren folgende: Gut oder ausgezeichnet: 75% bzw. 18%; Mäßig: 6,3% bzw. 22%; Todesfälle: 18,8% bzw. 60%. 3. Um diese Zahlen besser auswerten zu können, wurden die Patienten in zwei Gruppen (günstige und ungünstige Risiken) entsprechend ihrem klinischen Zustand zur Zeit der Einweisung unterteilt. Die Ergebnisse dieses Vergleiches zwischen operierten und nicht operierten Fällen in bezug auf ihren Zustand sind in Tab. 3 wiedergegeben. 4. Bei 73 Patienten (38%) blieb die Ursache der Subarachnoidalblutung unbekannt. Die Langzeitergebnisse dieser Fälle sind ebenso tabellarisch zusammengestellt.

Résumé 1. Tous les cas d'hémorragie sous-arachnoïdienne admis à la clinique neurochirurgicale de l'Université de Milan entre le 1er janvier 1964 et le 10 juin 1966 font l'objet de cette étude. Les patients porteurs de malformations artérioveineuses en sont exclus. 2. Si l'état du malade permettait le chirurgie d'un anévrysme sacculaire le traitement conservateur n'était jamais appliqué. Chez 119 (62%) des 192 cas, l'angiographie a démontré la présence d'un anévrysme sacculaire comme source de l'hémorragie. L'anévrysme fut attaqué directement chez 64 patients. Les résultats obtenus chez les patients opérés et ceux qui furent traités médicalement sont les suivants : bon ou excellent=75%, vs=18%; médiocre=6,3%, vs=22%; déces 18,8%, vs=60%. 3. Les patients furent divisés en deux catégories selon leur état au moment de l'admission:bon risque ou mauvais risque. Le tableau n° 3 établit la comparaison des résultats obtenus chez les opérés et ceux qui furent traités médicalement, compte tenu de l'état du malade à l'admission. 4. La cause de l'hémorragie sous-arachnoïdienne n'a pu être démontrée chez 73 patients (38%) et l'évolution de ces cas est indiquée dans un tableau.

Summary 1) All oases of subarachnoid haemorrhage admitted to the Neurosurgical Clinic of the University of Milan, between January 1st, 1964 and June 30th, 1966, are reported and the long-term results are presented. (Only those cases of subarachnoid haemorrhage in which an arteriovenous malformation was demonstrated were excluded from this study.) 2) In cases of saccular aneurysm, conservative treatment was never preferred if the condition of the patient allowed operation. The total number of cases was 192. In 119 (62%), angiography showed an intracranial saccular aneurysm to be the source of the bleeding. 64 patients underwent intracranial operation for direct attack to the aneurysm. The long-term results of the operated vs. non-operated cases were as follows: good or excellent: 75% vs. 18%; poor: 6.3% vs. 22%; deaths: 18.8% vs. 60%. 3) In order better to evaluate these figures, the patients were divided into two categories (favourable and unfavourable risk) according to their clinical state at the time of admission. The results of the comparison between operated and non-operated cases in relation to this state are shown in a Table (Table 3). 4) In 73 patients (38%) the cause of the subarachnoid bleeding remained unknown. The long-term results of these cases are also tabulated.
