    Notes: Abstract In end-stage heart failure due to idiopathic dilated cardiomyopathy β1-adrenoceptors are downregulated and G1α-proteins are upregulated. The aim of the present study was to investigate the influence of the angiotensin-converting enzyme inhibitor captopril on β-adrenoceptor density and Giα-proteins in sequential endomyocardial biopsies. Nineteen patients with mild to moderate congestive heart failure due to idiopathic dilated cardiomyopathy (NYHA Class II–III) were studied before and after 8–11 weeks of therapy. Patients were randomised into a captopril and a control group; 9 patients received captopril 12.5–50 mg per day, (divided in 2–3 doses) p.o. in addition to “conventional” therapy with digoxin and diuretics, and 10 controls received “conventional” therapy only. Echocardiography, spiroergometry, right heart catheterisation and endomyocardial biopsies were performed before (baseline) and after treatment. Compared to baseline, captopril increased total β-adrenoceptor density by selectively increasing β1-adrenoceptors (31.6 vs 41.2 fmol·mg−1; p〈0.05) but had no significant effect on Giα-proteins. The results indicate that treatment with angiotensin-converting enzyme inhibitors partly restores myocardial β1-adrenoceptor density, and this action effect may contribute to the clinical improvement of patients with idiopathic dilated cardiomyopathy treated in this way.

    Description / Table of Contents: Abstract. The application of ultrasound has become a standard imaging technique in clinical routine. During the last years enormous technical advances could be achieved concerning the ultrasound scanners as well as the software for computer-aided analysis of image data. In this paper we show some results of the research project “computer and sensor supported surgery” (SFB 414) as examples for the progress of clinical use of ultrasound in different medical fields. One example shows three-dimensional echography (diagnosis of bone tumors and their affect on the surrounding tissue). Two examples belong to the field of echocardiography (assessment of heart valve anuli and diagnosis of mitral regurgitation by volumetric measurements and visualization). The echocardiographic applications mainly differ by the kind of ultrasound data. The assessment of atrioventricular anuli is based on morphological data obtained from backscatter data whereas the measurement of flow volumes is based on Doppler data.
    Notes: Zusammenfassung. Die Verwendung der Ultraschalltechnik als bildgebendes Verfahren gehört in den unterschiedlichsten Bereichen der Medizin zur klinischen Routine. In den letzten Jahren sind sowohl bei der Weiterentwicklung der Echoscanner selbst als auch auf der Softwareseite im Bereich der computer-gestützten Auswertung von Ultraschalldaten enorme Fortschritte erzielt worden. Die Ergebnisse der Tätigkeiten im Sonderforschungsbereich 414 “Rechner- und sensorgestützte Chirurgie” sind bedeutsame Beispiele der Fortschritte in der klinischen Anwendung in unterschiedlichen medizinischen Fachrichtungen. Wir zeigen ein Beispiel aus der 3D-Echographie (Diagnose von Knochentumoren und ihren Auswirkungen auf das umgebende Weichgewebe) und zwei Beispiele aus der Echokardiographie (Vermessung von Herzklappenringen und Diagnose von Herzklappeninsuffizienzen durch Volumetrie und Visualisierung). Die echokardiographischen Anwendungen unterscheiden sich dabei hinsichtlich der verwendeten Ultraschallinformation. Während für die Vermessung von Herzklappenringen die morphologische Information aus den Backscatterdaten gewonnen wird, wird bei der Bestimmung von Flußvolumen auf die Dopplerdaten zurückgegriffen.

    Description / Table of Contents: Zusammenfassung Wir berichten über eine 56jährige Patientin mit einer großen intrazerebralen Blutung links parietal. Die Patientin ist soporös; die Schutzreflexe sind abgeschwächt. Beim Versuch, eine Magensonde zu legen, kommt es zur Fehlplazierung in die Lunge mit Pneumothorax rechts, der problemlos mit einer Pleuradrainage behandelt werden kann. Klinische Kriterien sind zur Beurteilung der korrekten Lage einer Magensonde nicht ausreichend. Eine radiologische Lagekontrolle ist vor allem bei bewußtseinsgetrübten und unkooperativen Patienten nach jeder Anlage einer Magensonde erforderlich.
    Notes: Summary We report a 56 year old woman with a large, left-sided intracerebral hemorrhage. The patient is soporose; the gag and caugh reflexes are almost absent. After insertion of a fine-bore silicone nasogastric feeding tube, a right-sided pneumothorax is detected due to misplacement of the tube into the bronchial system. Over the next few days, the pneumothorax can be successfully treated with a pleural drainage without further complications. We conclude that the clinical criteria for the proper position of gastric tubes are unreliable. Therefore, a routine chest X-ray is mandatory after placement of each nasogastric tube, in particular, if the patient is uncooperative or unconscious.

    Description / Table of Contents: Summary Epidermoid cysts are rare lesions in the head and neck. Most often the are located in the submental region, although they sometimes can impose certain diagnostic problems. We report the case of a 19-year-old girl who presented with a fluctuant, painless, midline swelling of the floor of her mouth that had persisted for three months. Ultrasonographic examination showed a 5×5×4 cm cystic tumor with the unusual aspect of multiple smaller spherical formations. The differential diagnosis of the lesion included dermoid cysts, epidermoid cysts, ranula and parasitic lesions. Computed tomography and magnetic resonance tomography were not diagnostic. The patient underwent excision of the lesion, during which the cyst was completely removed. Histopathological diagnosis was consistent with an epidermoid cyst. The striking sonographic aspect of multiple spherical formations in the lesion was caused by multiple spherical keratin formations.
    Notes: Zusammenfassung Epidermoidzysten sind im HNO-Bereich relativ seltene und meist in der submentalen bzw. submandibulären Region lokalisierte Tumoren, die mitunter jedoch diagnostische Probleme aufwerfen können. Wir berichten über den Fall einer 19jährigen Patientin mit seit drei Monaten bestehender submentaler Schwellung. Bei der sonographischen Untersuchung zeigte sich das typische Bild einer ca. 5 Zentimeter großen Zyste. Auffällig waren multiple echoreiche kreisrunde Formationen, die anfangs zur Verdachtsdiagnose einer parasitären Erkrankung führten. Auch die CT- und MR-Diagnostik zeigten Veränderungen, die dem Ultraschallbefund entsprachen. Eine weitere differentialdiagnostische Eingrenzung war präoperativ nicht möglich. Die diagnostische Exstirpation und histologische Aufarbeitung der Raumforderung ergab die Diagnose einer Epidermoidzyste, wobei die in der Bildgebung auffälligen Formationen durch kugelige Anhäufungen von Keratinlamellen hervorgerufen wurden.
