    Notes: Summary Insulin receptor substrate 2 (IRS-2) is a substrate of the insulin receptor and mediates the action of the insulin. Disruption of the IRS-2 gene in mice results in peripheral insulin resistance and relative insulin deficiency. It is therefore possible that defects in the IRS-2 gene contribute to Type II (non-insulin-dependent) diabetes mellitus. We have examined the gene for evidence of linkage to Type II diabetes in Ashkenazi Jewish families. Radiation hybrid panel mapping was used to refine the map position of the IRS-2 gene and, in the absence of polymorphic markers within the gene, to identify nearby markers. The IRS-2 gene was placed 23cR from the marker D13S1265 on chromosome 13q34. 200 affected sibpairs were genotyped for three markers across the region. Nonparametric linkage analysis (GENEHUNTER) used with this data found no evidence of excess allele sharing in the IRS-2 gene region. We therefore concluded that variation in the IRS-2 gene is unlikely to contribute to Type II diabetes in this discrete Caucasian population. [Diabetologia (1998) 41: 1389–1391]

    Notes: In the first part of this study, the microstructures and the mechanical properties of precious, semi-precious, and nonprecious dental casting alloys for the porcelainbaked- to-metal technique have been determined. The semi-precious alloys contained only 50 % gold, and palladium, silver, and some base metals. The nonprecious alloys were of the nickel-chromium type. Discs and miniaturized tensile bars have been cast and tested either in the as cast condition, or after a simulation of the various porcelain bakes. Proof stress, ultimate tensile strength, elongation, and plastic stiffness have been measured and results compared by use of analyses of variance. The microstructure examination shows that the simulation of the porcelain bakes improves the homogeneity of the precious and semi-precious alloys. Simultaneously, the mechanical properties of the same alloys are also improved. One semi-precious alloy, still under development at the time of these tests, has its mechanical characteristics markedly dovv'ngraded by the thermal treatments. The nickel-chromium alloys exhibit the best range of mechanical properties for the porcelain-baked-to-metal technique, when considering the three most relevant properties: proof stress, plastic stiffness, and modulus of elasticity.

    Notes: The hardening ability of a dental palladium–silver (Pd–Ag–Sn–In) bonding alloy has been investigated, and the distribution of the elements along the metal–ceramic interface have been studied by microprobe analysis. The alloy was found to be highly heterogeneous in its ‘as-cast’ condition, but homogenized alloy was obtained after heating the alloy at 1000°C for 2h. Hardening occurred after annealing at 650°C for 1 h. A longer thermal treatment caused the hardness to decrease (over-ageing). The change in hardness could be attributed to an internal oxidation phenomenon. The additional elements tin and indium segregated towards the ceramic–metal interface, with a diffusion of indium only into the porcelain. Diffusion of silver in the ceramics, supposed to be the cause of the ‘greening’ of dental porcelains baked on silver-rich alloys, has not been detected by the microprobe across the interface.

    Notes: A purified polyclonal antiserum directed against the isolated main 80kD IROMP (iron-regulated outer-membrane protein) from Pseudomonas aeruginosa PAO1 detected only the 80 kD polypeptide of outer-membrane proteins from PAO1 cells grown in iron deficiency in Western blots. It was also shown to inhibit the uptake of 59Fe pyoverdin by PAO1 cells as well as its binding to purified outer membranes. Immunofluorescence experiments with intact PAO1 cells confirmed that the receptor is present only at the surface of cells grown under conditions of iron deficiency. All these data allow us to conclude that the 80 kD main IROMP of P. aeruginosa is indeed the receptor for the siderophore ferripyoverdin.

    Notes: Abstract Forty-four dental casting alloys and pure metals have been evaluated for corrosion resistance in an artificial saliva. The electrochemical tests included measurement of the rest potential and tracing of polarization curves. Qualitative comparisons of the normalized polarization curves, and quantitative determination of corrosion resistance, and of corrosion and breakdown potentials, provided complementary means of evaluation. Alloys were assigned two families: alloys for crowns and bridges (C&B alloys), and alloys for ceramic veneering (PFM alloys). Among these two families, groups were based on the gold-content (high and low), and on the main non-gold component: silver for C&B alloys, and palladium for PFM alloys. Pure metals and some non-noble alloys have been included as references. From this study, it clearly appears that the PFM alloys, having a higher content of noble metals, are more corrosion resistant than the C&B alloys containing copper and silver. Some relationships between noble-metal content and electrochemical parameters such as rest potential and corrosion resistance, have been identified, but the strength of the correlations is somewhat lowered by the influence of additional parameters, such as the presence of non-noble components or the nature of the microstructure. The present investigation is part of a larger study including other electrochemical parameters as well as various biocompatiblity tests on the same set of 44 alloys.

    Description / Table of Contents: Summary In a regional quality assurance project for screening mammography (German Mammography Study), 27 335 women were screened in 40 participating, office-based mammography units from 1990–1992. Screening led to 317 biopsies with a positive predictive value of 0.33. All biopsy documentation available to the mammography physicians was analysed with a view towards biopsy interval and completeness of information fed back to the Mx physicians. Biopsy recommendations were acted upon in 29 % of cases within 2 weeks. With the exception of the dignity Mx physicians were incompletely informed about biopsy results. The surgical procedure was known in the doctors' offices in 62 % of the cases. Specimen radiographies were not done regularly. A pathology report was available overall in 42 of 106 malignant cases, respectively. With the exception of the histological diagnosis itself, no variable mentioned in the reports was documented completely. Only one third of the physicians received such reports routinely. Fail-safe information are requested by Mx physicians and can help them to better target biopsy recommendations. A (regional) quality assurance center should be made responsible to analyse the flow of information in mammography screening, to fill in gaps and to speed up professional cooperation.
    Notes: Zusammenfassung Im Rahmen eines Projekts zur flächendeckenden Erprobung von Qualitätssicherungsmaßnahmen für die Früherkennungsmammographie (Deutsche Mammographie-Studie) wurden 1990–1992 27 335 Screeningmammographien von 40 Praxen in 2 Regionen durchgeführt. Bei 317 Patientinnen wurden Biopsien durchgeführt (positiver Prädiktionswert: 33 %). Die gesamte Dokumentation, die den mammographierenden Ärzten vorlag, wurde mit der Frage analysiert, wie frühzeitig empfohlene Biopsien durchgeführt wurden und welche Informationen wie vollständig an die mammographierenden Ärzte zurückliefen. Eine mammographisch indizierte operative Intervention wurde in 29 % der Fälle innerhalb von 2 Wochen durchgeführt. Das Biopsieergebnis erreichte die veranlassenden Ärzte nur unvollständig. Lediglich die Dignität der Befunde ließ sich in allen Fällen feststellen. Das Operationsverfahren war in 62 % der Fälle in der mammographierenden Praxis bekannt. Präparatradiographien wurden nicht regelmäßig durchgeführt. Ein Pathologiebericht lag bei nur 42 der 106 malignen Fälle vor. Mit Ausnahme der histologischen Diagnose war keines der darin enthaltenen Merkmale durchgängig vollständig dokumentiert. Nur ein Drittel der mammographierenden Ärzte erhält routinemäßig Kopien der Pathologieberichte. Gesicherte Querinformation wird von den mammographierenden Ärzten gewünscht und kann ihnen helfen, Biopsieindikationen gezielter zu stellen. Die Verantwortung, den Informationsfluß im mammographischen Screening transparent zu machen, Lücken zu schließen und die Zusammenarbeit der Leistungserbringer zu beschleunigen, sollte einem (regionalen) Qualitätssicherungs-Zentrum übertragen werden.
