    Notes: Abstract Epidemiological and genetic variables for oral clefts were analysed for the years 1981–1989 in a case-control study of congenital malformations in the Emilia Romagna, Veneto, and Friuli regions, and in the Trento and Bolzano hospitals. Birth prevalence for all cases of cleft lip with or without cleft palate (CL(P)) was 8.2 per 10,000 births, and that for cleft palate only (CP) was 6.1 per 10,000. Coexisting abnormalities were found in 23% of CL(P) cases and in 43% of CP. No clusters in time or space were detected. For isolated clefts, a predominance of males among CL(P) and of females among CP was found; epilepsy was the only maternal risk factor correlated with clefts, and an association between clefting and consanguinity was found. Empirical recurrence risks were calculated in both isolated CL(P) and CP.

    Notes: Summary In a study of 514 spontaneous abortions, 194 were found to have a chromosome anomaly. Of these, 4 (2.1%) were unbalanced translocations. Three of the translocations were Robertsonian (13q14q) and one was reciprocal. Each translocation was ascertained independently and each was associated with a balanced rearrangement in a carrier parent.

    Notes: Abstract Chronic renal failure represent one of the more common complications of diabetes. End-stage renal disease develops in about 50% of type 1 (insulin-dependent) diabetic patients between 10 and 30 years from the onset of the disease, and in a consistent number of type 2 (non-insulin-dependent) diabetic patients. In the past, diabetic patients were not included in dialysis programmes, but in the 1980s, in Western countries, a progressively increasing number of them were accepted for renal replacement therapy (RRT). According to the EDTA (European Dialysis and Transplantation Association) Registry, the prevalence of diabetic patients in the dialysis population in Europe varies consistently from country to country (6.8% in France, 9% in Southern Europe, 16% in Germany and 34% in Finland). In Italy, according to the ANED (National Association of Dialysed Patients) Registry, at the end of 1990 1929 of 27106 patients (7.11%) were classified as diabetic. A survey of the diabetic population on RRT was suggested by Catalano in 1988. Palatients were classified as having type 1 diabetes if they required insulin therapy at the start of diabetes or within 2 years of the onset of the disease. The data obtained showed that in Italy the ratio of type 1 to type 2 patients on RRT was 1:3. Another inmportant observation of this survey was that 79% of diabetic patients on RRT were affected by diabetic nephropathy and 21% by other nephropathies complicated by diabetes. About 50% of diabetic patients on RRT in Italy suffered from micro-and macroangiopathy before the start of replacement therapy. It is interesting that in the southern regions of the country the proportion of type 2 diabetic patients was greater than in the north, and that in Sardinia (probably as a result of genetic factors) 71% of diabetics on RRT had type 1 diabetes. In Italy the most common dialysis modality was haemodialysis (81%), while peritoneal diaysis accounted for 14% of patients; only 5% of diabetic patients received a functioning graft. Kidney transplantation should be considered the best form of RRT for diabetic patients, but very few transplants are performed in Italy, mainly as a result of organi procurement problems. More recent data indicate that the number of diabetic patients accepted for RRT is progressively increasing and that about 10 new diabetic patients pmp/year enter Italian RRT programmes. Diabetic patients on RRT have a shorter survival than the general dialysis population and suffer from many important complications. However, their life expectancy, which was very poor in the last decade, is now progressively improving.

    Notes: Abstract: Breast reduction or amputation in female-to-male surgery presents a specific surgical problem: obtaining a good breast shape of the masculine type. Over a 2-year period, 17 patients (12 female-to-male transsexuals and 5 extreme gynecomastias) were operated on using the circumareolar approach for subcutaneous mastectomy. The nipple-areola complex was left on a very wide deepithelialized dermal pedicle, and the final closure of the wound was performed using a round-block technique followed by numerous fine sutures to reduce wrinkling. This technique provides naturally flat masculine breasts, leaving sufficient dermal vascularization for the nipple-areola complex which is of the utmost importance. All the patients were very satisfied with the result because of the periareolar scar only. Two areolar necroses occurred due to perforation of the thin vascular dermal pedicle: one superficial which epithelialized spontaneously in a short period of time and one deeper which required skin grafting.

    Notes: Abstract The discussion of the problems of observation of tenuous intergalactic and/or halo clouds in recombination emission is presented. We elaborate the possibility that clouds are illuminated only by metagalactic ultraviolet radiation flux, and assume some plausible values for the parameters determining the physical conditions in such objects. A simple, semiqualitative approach may give specific observational predictions for the two major signposts of fluorescent emission: Lyα and Hα lines. We conclude that Hα detection of Lyman-limit systems at low redshift is within present observational capabilities. Open questions for further research are put in a convenient framework.

    Notes: Abstract The problem of influence of the ambiental intergalactic pressure on extended gas associated with normal field galaxies is briefly discussed. An empirically established characteristic radius of absorption can not be explained in the context of simple two-phase halo model as a consequence of pressure equilibrium of the hot halo with the smooth intergalactic medium. This conclusion is based upon the stringent constraints on the temperature and density of the intergalactic plasma obtained through the CMBR measurements. On the other hand, ram-pressure stripping caused by peculiar motions of galaxies does present a viable alternative for the hot halo truncation. It is shown that for a particular set of chosen parameters, a simple model is capable of producing the absolute upper limit on the extent of gas associated with the galaxy. The values obtained are compatible with the results of the recent QSO absorption-line studies, and are significantly higher than the radius at which the cooling timescale of the halo gas equates the dynamical timescale.

    Notes: Abstract The influence of recent detections of a finite vacuum energy(`cosmological constant') on our formulation of anthropic conjectures,particularly the so-called Final Anthropic Principle is investigated. Itis shown that non-zero vacuum energy implies the onset of a quasi-exponentialexpansion of our causally connected domain (`the universe') at some point inthe future, a stage similar to the inflationary expansion at the verybeginning of time. The transition to this future inflationary phase ofcosmological expansion will preclude indefinite survival of intelligentspecies in our domain, because of the rapid shrinking of particle horizonsand subsequent depletion of energy necessary for information processes withinthe horizon of any observer. Therefore, to satisfy the Final AnthropicHypothesis (reformulated to apply to the entire ensemble of universes), itis necessary to show that (i) chaotic inflation of Linde (or some similarmodel) provides a satisfactory description of reality, (ii) migration betweencausally connected domains within the multiverse is physically permitted, and(iii) the time interval left to the onset of the future inflationary phaseis sufficient for development of the technology necessary for suchinter-domain travel. These stringent requirements diminish the probabilityof the Final Anthropic Hypothesis being true.

    Notes: Abstract The discussion of the relationship between cooling radius in two-phaseadiabatic halo models and observationally established maximal radiusof absorption in luminous galaxies at low redshift is presented. It isof paramount importance that any viable model of the halo is able toreproduce empirical value of the absorption cross-section of normalgalaxies and its scaling with luminosity. Several ways for bringingthe theoretical and observational values into agreement are outlined.It is of great methodological importance to fully assess the difficultiesany model is likely to encounter when testing predicted extent ofabsorption and column density distribution against recently obtainedlow-redshift Lyα absorption data.

    Notes: Abstract If a significant fraction of QSO absorption line systems arises in halos of normal galaxies, absorber statistical data will tell us a lot about the structure of the gaseous envelopes of galaxies. Comparison of a class of density profile models for the metal-absorbing halos of luminous galaxies with the recent Lyα empirical data is presented. Cosmological mass fraction contained in such gas is also briefly discussed. It is shown that important constraints on the model parameters can be inferred in this way.
