    Notes: Conclusion L'échec du traitement médical d'un syndrome douloureux évoquant un syndrome du tunnel radial justifie l'exploration chirurgicale du tunnel radial à la recherche de structures de compression avec la BPNR, en gardant à l'esprit tous les sites possibles de conflit. L'abord entre les mm. court et long extenseur radiaux du carpe permet alors une approche directe et rapide aux deux structures les plus fréquemment en cause : l'arcade du CERC et l'arcade supérieure du chef superficiel du m. supinateur. Cet abord est aussi approprié pour l'exploration de l'ensemble du tunnel radial en restant à distance des multiples branches nerveuses collatérales pour les mm. brachioradial et LERC. La difficulté réside alors dans le repérage de la branche collatérale unique pour le CERC, particulièrement lorsqu'elle naît de la BPNR à proximité de la zone explorée.

    Notes: Abstract The anatomy of the radial tunnel was studied on twenty-five elbow preparations. We noted five different elements that could affect the deep branch of the radial nerve in the radial tunnel and cause an entrapment syndrome: a capsule-tendon-aponeurotic complex on the anterior aspect of both the humeroradial joint and the radial head, the vascular arcade formed by the radial recurrent a. and its branches, the arcade formed by the medial edge of extensor carpi radialis brevis muscle, and the superior and inferior arcades of the superficial layer of the supinator muscle. These results are compared with recent anatomical and clinical data, and the relevance of a surgical approach between the extensor carpi radialis brevis and longus muscles is discussed.

    Description / Table of Contents: Résumé Une étude anatomique du tunnel radial a été réalisée sur vingt-cinq préparations de coude. Nous avons observé cinq structures pouvant entrer en conflit avec la branche profonde du nerf radial dans le tunnel radial et être responsable d'un syndrome canalaire : le complexe capsulo-tendino-aponévrotique situé à la face antérieure de l'articulation huméro-radiale et de la tête radiale, l'arcade vasculaire de l'artère récurrente radiale et ses branches, l'arcade du bord médial du muscle court extenseur radial du carpe, les arcades supérieure et inférieure du chef superficiel du muscle supinateur. Nous discutons nos résultats avec les données récentes des études anatomiques et cliniques, ainsi que l'intérêt de la voie d'abord chirurgicale entre les muscles court et long extenseurs radiaux du carpe.

    Description / Table of Contents: Résumé Les variations d'origine de l'artère ophtalmique sont rares et bien expliquées par l'étude de l'anatomie comparée et de l'embryologie humaine. Nous rapportons le cas exceptionnel d'une variation d'origine de l'artère ophtalmique à partir de l'artère méningée moyenne associée à une occlusion de l'artère centrale de la rétine. Les variations embryologiques pouvant donner naissance à cette origine inhabituelle sont discutées. Bien que cette association puisse être totalement fortuite, nous pensons qu'une variation d'origine de l'artère ophtalmique peut constituer un facteur favorisant pour une occlusion de l'artère centrale de la rétine.

    Description / Table of Contents: Résumé Dans cette étude, les auteurs rapportent les résultats d'une série de 50 dissections anatomiques de l'a. choroïdienne antérieure chez l'Homme. Cinquante hémisphères cérébraux préparés selon les techniques classiques de conservation et d'injection vasculaire ont ainsi été étudiés. Nous avons observé une origine ectopique dans 4 % des cas. Lors de son trajet intra-cisternal, l'a. choroïdienne antérieure réalise avec la face ventrale du tractus optique une "pince vasculo-nerveuse" autour de la veine basale. Bien que la plupart des branches collatérales à destinée intra-parenchymateuse naissent du segment cisternal de l'a. choroïdienne antérieure, nous avons observé des branches collatérales intra-plexuelles se dirigeant vers le tractus optique, le corps géniculé latéral et le thalamus. Les anastomoses avec le système vertébro-basilaire sont constantes et se font le plus souvent avec les aa. choroïdienne postéro-latérale et cérébrale postérieure. Nous discutons l'intérêt d'une parfaite connaissance anatomique de l'a. choroïdienne antérieure et de son environnement pour l'appréhension chirurgicale des anévrysmes artériels et malformations artério-veineuses de la région.
    Notes: Summary In this study, the authors present the results of 50 dissections of the anterior choroidal a. in man. Fifty cerebral hemispheres were prepared with the classic techniques of preservation and vascular injection. An ectopic origin was observed in 4% of cases. The intracisternal segment of the anterior choroidal a. forms a neurovascular bundle with the optic tract and basal v. Most of its intraparenchymatous branches arise from the cisternal segment, while branches supplying the optic tract, lateral geniculate body and thalamus arise from the intraplexual segment. Constant anastomoses exist with the vertebrobasilar system, specially the postero-lateral choroidal and posterior cerebral aa. We discuss the importance of an adequate knowledge of the anatomy of the anterior choroidal a. and its relations in the surgical approach to arterial aneurysms and arteriovenous malformations of the region.

    Notes: Abstract Intramedullary abscesses of the spinal cord are uncommon. Most of them occur in association with heart, pulmonary or urogenital infections. We report two cases of intramedullary spinal cord abscesses secondary to congenital dermal sinus. Only 14 cases of such an association have previously been reported. In our cases, dermal sinus was associated with an epidermoid tumour. The clinical presentation, pathogenesis, magnetic resonance imaging findings, surgical management and outcome are discussed.
