    Notes: Summary Diabetic patients exhibit a higher incidence of post-surgical sepsis, as well as a higher rate of mortality from sepsis, than their non-diabetic counterparts. This may be a result of cardiovascular deterioration associated with diabetes mellitus. This study was designed to characterize the cardiovascular sequelae associated with endotoxin shock in a canine model of diabetes. Diabetes was induced with alloxan (50 mg/kg) and streptozotocin (30 mg/kg) in dogs weighing 19–25 kg. Thirty days later, anaesthetized dogs were instrumented to obtain blood pressures, blood samples, left ventricular chamber diameter, circumflex arterial blood flow, and aortic blood flow. Metabolic parameters were calculated according to the Fick principle, and myocardial inotropic state assessed with the end-systolic pressure-diameter relationship. After stable baseline measurements, Escherichia coli endotoxin (1 mg/kg) was infused over 1 h, and measurements were obtained every 30 min. After endotoxin administration diabetic dogs became more hypotensive than the non-diabetic dogs. Cardiac performance parameters were also depressed to a greater degree. These changes could be attributed to depressions in vascular resistance and myocardial inotropic state in diabetic dogs. Cardiac dysfunction occurred in association with a relative decrease in the supply to demand ratio for oxygen in the diabetic dogs, suggesting functional ischaemia. Data indicating a decrease in pre-load and vascular resistance in the diabetic group suggest a greater degree of vascular collapse, vascular pooling, or extravasation of fluid than occurred in the non-diabetic group. These data support the hypothesis that the cardiovascular system of diabetic subjects cannot tolerate a septic insult as well as their non-diabetic counterparts.

    Notes: Aims : To standardize the pathological analysis of total mesorectal excision specimens of rectal cancer following neoadjuvant chemoradiotherapy for locally advanced disease (T3/T4), including tumour regression.Methods and results : Standardized dissection and reporting was used for 60 patients who underwent total mesorectal excision following long-course chemoradiotherapy. Tumour regression was scored by two pathologists (K.S., D.G.) using both an established 5-point tumour regression grade (TRG), and a novel 3-point grade. Both scores were evaluated for interobserver variability. A complete or near-complete pathological response (3-point TRG 1) was found in 10 patients (17%). Using the 5-point TRG, there was good agreement between both pathologists (κ = 0.64). Using the 3-point grade, agreement was excellent (κ = 0.84). No disease recurrence has been reported in patients with a complete, or near complete pathological response (3-point TRG 1), after a mean follow-up of 22 months.Conclusion : Tumour regression grade is a useful method of scoring tumour response to chemoradiotherapy in rectal cancer. TRG 1 and 2 can be regarded as a complete pathological response (ypT0). A modified 3-point grade has the advantage of better reproducibility, with similar prognostic significance.

    Notes: Zusammenfassung In früheren Arbeiten wurde der Beweis erbracht, daß zwischen einfachen Diffusionsvorgängen und den Sekretionserscheinungen der Niere in bezug auf die Konzentrations- und die Zeitverhältnisse sowie auf die Größe der Konstanten, die bei verschiedenen Substanzen ermittelt wurden, ein enger Parallelismus besteht. Nun können Diffusionsvorgänge als physikalische Annäherung an einen Gleichgewichtszustand aufgefaßt werden, was die Umkehrbarkeit des Vorgangs bei Änderung des Gleichgewichts in sich schließt. Von diesem Gesichtspunkt aus ist in der vorliegenden Arbeit geprüft worden, ob auch in der tätigen Niere eine derartige Umkehr beobachtet werden kann, und in der Tat ließ sich dabei der Beweis für diese Erscheinung führen. Es ist damit ein weiterer Beleg dafür erbracht, daß die physikalische Diffusion bei der Bildung eines konzentrierten Harnes nicht nur ein wesentliches, sondern geradezu das führende Moment darstellt.

    Notes: Summary Thermostable, extracellular α-amylase and α-glucosidase were produced byLipomyces starkeyi CBS 1809 in a medium containing maize starch and soya bean meal. Contrary to published findings which suggested a single cell-bound amylolytic system for another strain ofL. starkeyi, this study revealed the presence of two enzymes — an α-amylase and an α-glucosidase inL. starkeyi CBS 1809. The enzymes were separated by solvent and salt precipitation and ion-exchange chromatography on DEAE-Biogel-A. The α-amylase and α-glucosidase had pH optima at 4.0 and 4.5 and temperature optima at 70°C and 60°C, respectively. While the low pH optima are not unique the enzymes are very distinctive in yeasts in having very high temperature optima. The α-glucosidase had highest activities on maltose and isomaltose (100) with relative rates of activity on maltotriose, isomaltotriose and p-nitrophenyl-α-d-glucoside of 59, 48 and 22, respectively. It was inactive towards sucrose. Both the α-amylase and α-glucosidase ofL. starkeyi were located extracellularly and had molecular weights of 76,000 and 35,000, respectively.

    Notes: Summary Bacillus firmus ATCC 14575 produces both an α-amylase and α-glucosidase. Production of α-glucosidase was influenced greatly by carbon source, e.g. maltose induced seven times the activity obtained with starch. The enzyme had optima for activity at pH 8.0 and 40°C, was highly specific for maltose and to a lesser extent p-nitrophenyl-α-D-glucoside and maltotriose and was inactive towards a range of other substrates including isomaltose and sucrose.
