    Notes: Summary The present animal experimental study showed that intraoperative hepatic ultrasonography using an echo contrast medium can visualize small hepatomas (with a diameter of between 3 and 15 mm) induced in the rat liver, although they were not recognizable with plain ultrasonography. A homogeneous increase in the echogenicity of the liver tissue was achieved by using an echo contrast medium (Echovist) based on galactose microparticles. Selfmade bubble preparations such as those used in echocardiography were far less effective. When the dosage was optimal (0.01−0.003 ml/g liver weight with concentrations of 200 and 300 mg/ml Echovist), homogeneous contrast enhancement of the liver was achieved for at least 10 min after a single bolus injection via all routes of contrast adminstration (hepatic artery, portal vein, bile duct). As a result, hepatomas appeared as hypodense formations (portal vein and bile duct) or as hyperdense zones (hepatic artery).

    Notes: Summary In many clinics, intraoperative sonography is used as a routine method in general surgery as well as in neurosurgery. The availability of sterilizable applicators in diverse shapes and sizes enables the optimal approach to the site to be examined. In processes involving the liver, the pancreas, the kidneys, or the brain, ultrasonic examination, together with an exact topographic localization, proved to be helpful in determining the dimensions of an altered structure. In patients with colorectal carcinoma, the discovery of metastases that had not been previously detected by percutaneous sonography proves the necessity of regular intraoperative ultrasonic examination to augment staging accuracy in tumor surgery.

    Notes: Summary In 66 patients with renal transplants 246 sonographic examinations were performed. The patients were divided into two groups based on their immunosuppressive protocol. Group I was treated with Cyclosporin A (CsA) and group II with azathioprin. A compensatory hypertrophy with a volume increase of 20% could be seen in nearly all grafts. During acute tubular necrosis only minimal sonographic changes could be found. In each group 16 patients developed an acute rejection episode. Sonographic signs of acute rejection were: (1) a hypoechoic enlargement of the renal pyramides; (2) an increase in cortical echogenicity; (3) an increase in graft volume greater than compensatory hypertrophy; (4) an indistinct parenchyma-pelvic border; (5) dilation of the pelvis with a parenchyma-pelvic index 〉2.3:1 (in group II cases). Chronic rejection is characterized by graft shrinkage. No specific signs were evident. The increasing use of CsA diminishes the value of sonography in follow-up of acute rejection after kidney transplantation. Nevertheless, it is of great value for follow-up examination concerning other complications.

    Description / Table of Contents: Zusammenfassung Mit der Sonographie bei 517 notfallmäßig eingewiesenen Patienten ließ sich eine Vielzahl von Diagnosen präzisieren. Bei Erkrankungen an Gallenwegen lieferte die Sonographie 95% richtige Diagnosen. Bei der Pankreatitis war die Treffsicherheit erst im Stadium II ausreichend hoch. Beim Verdacht auf eine akute Appendicitis wurde eine Vielzahl anderer Erkrankungen abgegrenzt. Beim Polytrauma ließen sich intraabdominelle Verletzungen schnell erfassen. In der Hand eines erfahrenenChirurgen wird der Wert der Sonographie beim chirurgischen Notfall durch die rasche gezielte und für die weitere chirurgische Therapie relevante Befunderhebung erhöht.
    Notes: Summary Ultrasonography performed on 517 patients presenting as surgical emergencies permitted many specific diagnoses. In 95 % of patients with bile duct pathology, diagnosis was correct. In patients with pancreatitis, ultrasonography was sufficiently reliable only in stage II and beyond. When acute appendicitis was suspected, ultrasonography was of great assistance in differential diagnosis. In polytraumatized patients intra-abdominal injuries could be evaluated swiftly. The rapid determination of surgically relevant findings makes ultrasonography a valuable tool for the experienced surgeon during an emergency.
