    Notes: Macroscopic features of temporomandibular joints (TMJs) were studied in young adults who comprise the largest portion of individuals seeking TMJ treatment. Deviation in form (DIF), arthrosis, size, shape and disc displacement were evaluated on ninety-five autopsied TMJs. Few TMJs (13%, 12/95) showed no intracapsular changes. Thirty-nine per cent (37/95) of the TMJs displayed mild-to-marked DIF in all three TMJ components. Smaller changes were more prevalent and tended to appear in the younger TMJs. Condylar changes were more exuberant and extensive compared to the other components. Minor arthrotic lesions were visible in 3% (3/95), and all displayed DIF. Disc displacement was found in 12% (11/95) and was more common in women (P 〉 0.05). Folding and deformation of the articular disc was associated with disc displacement (P 〈 0.01), the direction of which was mostly anteromedial. Most of the unchanged condyles' components had curved, slightly rounded, convex, and elliptical shapes when viewed from different planes (P 〈 0.01). Applied in diagnosis, the presence of DIF can be inferred from features which deviate from the above shapes. The concept that the above macroscopic changes might be a precursor to TMJ arthropathy in susceptible individuals is compatible with the results of this study, but the most apt characterization is that TMJ changes in this age group are adaptive phenomena occurring in order to cope with the details of articular fit and function.

    Description / Table of Contents: Zusammenfassung 84 Patienten mit tiefen Atemwegsinfektionen wurden in einer randomisierten, doppelblind parallel geführten multizentrischen Studie mit Cefaclor oder Amoxicillin behandelt. Dabei wurde der Einfluß der Antibiotikatherapie auf die Kolonisationsresistenz untersucht. 40 Patienten erhielten Cefaclor, 44 Amoxicillin; die Dosierung betrug jeweils 250 mg dreimal täglich per os. Die Behandlung wurde sieben Tage lang durchgeführt. Vor, während und nach Antibiotikatherapie wurden Proben von Sputum, Abstriche aus dem Oropharynx und Stuhl entnommen. Die mikrobiologischen Untersuchungen dienten einerseits der Erregeridentifikation, andererseits der Verlaufskontrolle der Veränderungen der Mikroflora. Unter Cefaclor wurden 92,5%, unter Amoxicillin 88,4% der Patienten geheilt oder gebessert. Dieser Unterschied war nicht statistisch signifikant.Haemophilus influenzae, Branhamella catarrhalis undStreptococcus pneumoniae waren die häufigsten Erreger der Atemwegsinfektionen. In der Erregereradikationsrate unterschieden sich die beiden Therapien nicht. Unter Cefaclor traten keine wesentlichen Veränderungen der oropharyngealen Flora auf. Amoxicillin führte zu einer signifikanten Reduktion der Keimzahlen vonStreptococcus salivarius undVeillonella, die Enterobakterien nahmen in der oropharyngealen Mikroflora zu. Cefaclor führte zu einer signifikanten Verminderung der Keimzahlen von Streptokokken, Staphylokokken und anaeroben Kokken im Stuhl während Enterokokken, EnterobakterienBacteroides undCanadida albicans signifikant zunahmen. Auch Amoxicillin hatte zu Veränderungen der intestinalen Mikroflora mit Zunahme der Enterobakterien, anaerober grampositiver Stäbchen undBacteroides geführt. Stärkere Störungen der intestinalen Kolonisationsresistenz traten mit keiner der beiden Therapien auf.
    Notes: Summary Eighty-four patients with lower respiratory tract infections participated in a randomised double-blind parallel multicenter trial in order to compare the efficacy of cefaclor and amoxycillin as treatment for lower respiratory tract infections and their ability to influence colonization resistance. Cefaclor was given to 40 patients and amoxycillin to 44 patients perorally in doses of 250 mg t.i.d. for seven days in a double-blind fashion. Sputum, oropharyngeal and intestinal specimens were taken for microbial analysis to isolate the causative pathogen of the lower respiratory tract infection and to follow the microflora changes before, during and after antibiotic treatment. The clinical outcome showed 92.5% cured or improved patients on cefaclor versus 88.4% on amoxycillin. The difference in clinical outcome between the two treatment groups was not statistically significant. Among the pathogenic bacteria isolated,Haemophilus influenzae, Branhamella catarrhalis andStreptococcus pneumoniae dominated. There was no difference between the two treatments with regard to microbiological efficacy. Treatment with cefaclor did not cause any significant impact on the oropharyngeal microflora. Administration of amoxycillin caused a significant reduction of the number ofStreptococcus salivarius andVeillonella cocci, while an increase in number of enterobacteria was seen in the oropharyngeal microflora. In the intestinal flora, cefaclor significantly reduced the number of streptococci, staphylococci and anaerobic cocci, while the number of enterococci, enterobacteria, bacteroides andCandida albicans significantly increased. The intestinal microflora was partly influenced by amoxycillin treatment. Thus there was a significant increase in the number of enterobacteria, anaerobic gram-positive rods and bacteroides. In conclusion, none of these agents caused any major disturbances in the colonization resistance in patients with lower respiratory tract infections.

Das Durchschnittsalter war 73 Jahre, Frühsterblichkeit kam nicht vor, zwei Hüften luxierten. Nach zwei Jahren lebten 38 Patienten noch and wurden wieder untersucht.     Notes: Summary Forty patients were treated by means of total hip arthroplasty after nailing of femoral neck fractures had failed. Their average age was 73 years. Thirty-eight patients were still alive after 2 years and were reviewed. Two hips dislocated, but there was no early mortality. The results regarding pain, walking capacity, and function were good, and total hip replacement is considered to be the salvage operation of choice.

    Notes: Abstract We describe the overall properties of a new catalogue of metallicities, ages, and galactic orbits for a large, complete sample of F and G dwarfs in the solar neighbourhood. Based on a magnitude-limited sample of ∼ 14000 stars, it is volume-complete to ∼ 40 pc. Together with the astrophysical parameters of direct relevance to models of the evolution of the disk, it will contain the basic photometric, astrometric, and radial velocity data from which they are derived. Information on duplicity is also included. The full exploitation of the data will require a lengthy analysis, in particular to assess the degree of completeness of subsamples of stars of different population types. An early result on the effects of diffusion of galactic orbits in the disk – essential for understanding the scatter in the age-metallicity diagram and estimating the birth radius of stars – is briefly illustrated.

    Notes: Abstract Application of digital cross-correlation spectroscopy to the spectra of the W Serpentis binaries SX Cas and RX Cas has allowed an accurate determination of the orbits and rotations of the (mass-losing) K-subgiant secondary components. The distortion of the primary radial-velocity curves due to the influence of the prominent accretion disks in these systems has been modelled to first order. This enables us to estimate k 1, and thereby the mass ratio q ≈ 0.30, to within ∼ ± 20%. The absolute radii of the secondaries are derived independently from the observed rotations and periods, assuming synchronous rotation. They show that the stars fill their Roche lobes, or at least very nearly so. Rough fits to the available photometry shows both primaries to be unevolved mid-B stars; that in RX Cas appears completely obscured by the disk. Preliminary spectroscopic data for W Ser and W Cru show some promise for similar analyses of these systems.
