    Notes: The Mueller matrix approach is used to analyze the problems associated with the calibration of a modern CD spectropolarimeter constructed on the basis of a modulation technique in polarization spectroscopy. As a result, the performance of the CD spectropolarimeter is found to be determined by the characteristics of the photomultiplier as a partial polarizer, the residual static strain birefringence of the modulator, and the first harmonic response of a lock-in amplifier. Furthermore, the CD signal is greatly affected by the circular birefringence of a sample in addition to the above three factors. Thus, the necessity of checking these factors before the calibration is carried out using a standard sample is pointed out.

    Notes: We report a case of primary malignant melanoma of the male urethra in a patient whose penile shaft was successfully preserved, but who proceeded to acute renal failure (ARF) after interferon (IFN)-β adjuvant immunotherapy. Primary malignant melanoma of the male urethra is rare and usually shows highly malignant potential. Therefore, urologists must often perform phallectomy, which impacts on the patient both sexually and mentally. A 64-year-old man presented at Saiseikai Sendai Hospital with asymptomatic gross hematuria and was diagnosed as distal urethral tumor. We predicted the highly malignant potential of this tumor from the urethroscopic finding and from urinary cytological examination. We did not select trans-urethral resection (TUR), but selected partial urethrectomy.This patient proceeded to ARF 1 month after natural IFN-β treatment as an adjuvant immunotherapy. As IFN-β rarely induces the delayed renal failure, urologists should be aware of renal dysfunction after IFN-β therapy for the treatment of malignant melanoma.

    Prognostic value of tumor-associated macrophage count in human bladder cancer (2000)

    Notes: Background : We determined the tumor-associated macrophage (TAM) count to investigate its importance in predicting clinical outcome or prognosis in patients with bladder cancer. Methods : The TAM count and microvessel count (MVC) were determined immunohistochemically in 63 patients with bladder cancer, including 40 superficial bladder cancers and 23 invasive bladder cancers. To examine the relationship between TAM count and clinical outcome or prognosis in bladder cancer, cystectomy rates, distant metastasis rates, vascular invasion rates and 5 year survival rates were compared between patients with low (〈 67) and high (≥ 67) TAM counts. Results : The TAM count in invasive bladder cancers (154.22 ± 11.98) was significantly higher than in superficial bladder cancers (49.05 ± 7.76; P 〈 0.0001). The MVC in invasive bladder cancers (71.55 ± 10.44) was also significantly higher than in superficial bladder cancers (47.02 ± 5.57; P 〈 0.05). There was a positive correlation between TAM count and MVC (r = 0.30; P = 0.02). Immunohistochemical staining using CD68/horseradish peroxidase monoclonal antibody showed more infiltrating cells in invasive than superficial bladder cancers. Patients with a high TAM count (≥ 67) showed significantly higher rates of cystectomy, distant metastasis and vascular invasion than those with a lower TAM count (〈 67). The 5 year survival rate estimated using the Kaplan– Meier method was significantly lower in patients with a high TAM count than in those with a low TAM count (P 〈 0.0001). Conclusions : Our results suggest that determination of TAM count in bladder cancer tissues is of value to predict the clinical outcome or prognosis and to select appropriate treatment strategies in patients with bladder cancer.

    Notes: Abstract A case is reported of a 30-year-old woman with a long fibroepithelial polyp in the middle ureter treated with the Ho-YAG laser endoscopically. She presented with an intermittent macroscopic hematuria and lower abdominal pain lasting for 1 year. The filling defect on urography occupying one-third of the ureter was migratory depending on the patient position. Transurethral flexible ureterorenoscopy showed a large pedunculated tumor with a small base at the middle ureter. About 1 month after the endoscopic irradiation of the Ho-YAG laser to the base of tumor, the tumor was spontaneously discharged and pathologic examination revealed it to be a fibroepithelial polyp without malignant component. Postoperatively, the patient remained asymptomatic and follow-up excretory urographs showed no abnormal findings.

    Notes: Background : The purpose of the present paper was to investigage the clinical significance of thymidylate synthase (TS) expression in bladder cancer and its association with proliferation markers, such as p53, Ki-67, and proliferating cell nuclear antigen (PCNA).Methods : Thymidylate synthase gene expression in 54 patients with bladder cancer was measured by the reverse transcription polymerase chain reaction (RT-PCR) method using glyceraldehyde-3-phosphate dehydrogenase (GAPDH) as an internal standard. The TS expression was also examined immunohistochemically. Immunohistochemical studies of p53, Ki-67, and PCNA expression were carried out to examine the correlation between TS expression and the expression of proliferation markers in the tumors. Prognostic and clinical outcome factors such as vascular invasion and distant metastasis were also examined along with TS expression.Results : Twenty-four patients with invasive bladder cancer had TS levels of 5.07 ± 0.77 (mean ± SE), while 30 patients with superficial bladder cancer had TS levels of 2.28 ± 0.38. There was a significant difference in TS levels between invasive and superficial bladder cancer (P = 0.001). There was a positive correlation between TS expression and each proliferation marker of p53 (r = 0.686, P 〈 0.01), Ki-67 (r = 0.715, P 〈 0.0001) or PCNA expressions (r = 0.670, P 〈 0.0001) in these patients. Patients with high TS levels (TS ≥ 2.63, the median value) had significantly higher rates of vascular invasion and distant metastasis. Kaplan–Meier analysis demonstrated that patients with high TS levels (TS ≥ 2.63) had unfavorable prognosis compared to patients with low TS levels (TS 〈 2.63; P = 0.034). Furthermore, patients with high TS staining had a significantly poorer prognosis than those with low staining (P = 0.012).Conclusion : Determination of level of TS expression may help in the selection of an appropriate treatment for bladder cancer because TS expression influences the biological characteristic of bladder tumor.

    Notes: Intravesical thermometers are occasionally encountered in urologic practice. In the present paper, we describe the removal of a thermometer from the bladder of a woman who presented with irritable bladder symptoms. The thermometer was removed intact transurethrally using a rigid nephroscope and forceps, even though both ends of the thermometer were embedded in the bladder wall. Our retrieval technique may be of general use in such cases, and it should be attempted before resorting to open surgery.

    Notes: Background : Prediction of the extent of calcium supplement will facilitate safe and efficient management of hypocalcemia in the early postoperative stage of total parathyroidectomy with autotransplantation (PTXa) in patients with renal osteodystrophy. Methods : The correlation between the extent of calcium deficiency, estimated by the amount of calcium supplement over 48 h after PTXa and using various parameters such as carboxy terminal parathyroid hormone (c-PTH), intact PTH (i-PTH), alkaline phosphatase (ALP), serum calcium, serum phosphorus, duration of hemodialysis, total weight of resected parathyroid glands and degree of subperiosteal resorption of the middle phalanx was examined in 49 patients who underwent PTX with subcutaneous autotransplantation. Bone mineral density (BMD) was also determined before, 3 months and 1 year after PTXa with dual energy X-ray absorptiometry (DEXA) in 13 patients. Results : There was a positive correlation between pre-operative i-PTH level (r = 0.56, P 〈 0.0005) or ALP level (r = 0.50, P 〈 0.0005) and the amount of calcium supplement over 48 h after PTXa in these patients. Furthermore, the degree of subperiosteal resorption, determined by Jensen’s classification, was significantly correlated with the amount of calcium supplement after PTX (P 〈 0.05). Bone mineral density 3 months after (P 〈 0.0005) and 1 year after PTXa (P 〈 0.001) significantly increased compared with BMD before PTXa in all patients examined. Conclusion : These findings suggest that the pre-operative determination of i-PTH, ALP levels and degree of subperiosteal resorption allow the management of hypocalcemia safely and efficiently in renal osteodystrophy patients after PTXa.

    Notes: A 49-year-old woman had been on hemodialysis for 18 years. She presented with left back pain and macrohematuria. Radiologic studies demonstrated a left renal tumor with acquired cystic disease of the kidney. Her serum erythropoietin (EPO) level was 78.4 U/L despite no history of EPO supplementation. Left radical nephrectomy was performed. Pathologic examination revealed EPO-producing renal cell carcinoma. After surgery, the patient's serum EPO level decreased markedly to 15.1 U/L. The measurement of serum EPO levels may be useful for detecting and monitoring a recurrence of renal cell carcinoma with acquired cystic disease of the kidney in patients on long-term hemodialysis.
