    Notes: Zusammenfassung Die Carbinolbasen des Anilinblaus, Baumwollblaus und Brillantreinblaus erwiesen sich als ausgezeichnete neue nekrotrope Verbindungen, sie vermochten aber das Carcinomgewebe ebenso wie die Carbinolbasen des Fuchsin S, Lichtgrüns und Wasserblaus nicht zu beeinflussen. Ebenso zeigten Brillantdianilblau und Brillantsäuregrün keine Elektroaffinität zum wuchernden Carcinomgewebe der Mäuse. Eine Anhäufung der Carbinole kommt nur dort zustande, wo die Geschwulst teilweise oder ganz nekrotisiert ist.

    Notes: Abstract Multiple seromuscular biopsies at three levels (narrow segment, transitional zone, and dilated segment) were taken and investigated intraoperatively to determine the extent of aganglionosis. Using the whole-mount preparation technique, circular muscle fibers were separated from the specimens. After a short prefixation, the muscle fibers were stained by the NADPH-diaphorase technique and were examined within 20–25 min. A fine and dense neuronal meshwork was observed between circular muscle fibers in the normal and ganglionic part of the bowel. In contrast, there was a complete lack of NADPH-diaphorase-positive fibers in the circular muscle of aganglionic colon. In the transitional zone, NADPH-diaphorase-positive fibers were markedly reduced compared to the ganglionic region. The density of these fibers increased and attained normal levels in the proximal bowel above the transition zone. These results suggest that whole-mount NADPH-diaphorase histochemistry is a three-dimensional technique suitable for the intraoperative evaluation of extend of aganglionosis. The technique is sufficiently rapid to be used in conjunction with routine frozen sections to assist in the diagnosis and in selecting the optimal level of resection at the time of pull-through operation.

    Notes: Abstract  The innervation of the human bowel wall and its structural and functional changes in Hirschsprung's disease (HD) are well-recognised. The luminal surface of the bowel acts as a multifunctional barrier, and modifications in its physiochemical properties can result in serious complications such as enterocolitis (EC). The whole-mount preparation (WMP) technique produces a three-dimensional (3D) picture to better demonstrate the neuronal networks and the relationship of branching and interconnecting nerve fibres to each other. The aim of this study was to investigate the innervation of the mucosal layer in normal and HD bowel using a WMP immunohistochemistry technique in order to better understand the pathophysiology of HD. Full-thickness bowel specimens were collected from 9 HD patients at pull-through operation. Normal control small- and large-bowel specimens were collected from 10 patients at the time of bladder augmentation. Suction rectal biopsies from 8 patients with chronic constipation and 2 patients with HD were also included in this study. A WMP of the mucosal layer was made and stained with various neuronal markers (S100, PGP 9.5, and LlCAM) using fluorescein immunohistochemistry. PGP 9.5, S100, and LlCAM immunofluorescence staining of the normal mucosa demonstrated a characteristic 3D meshlike neuronal network of uniform thickness surrounding the crypts. In the aganglionic bowel S100, PGP 9.5, and LlCAM-positive meshlike networks were replaced by thick nerve trunks in the muscosa without any interconnecting network. The present study demonstrates for the first time the 3D morphology of mucosal innervation in normal and aganglionic bowel. The WMP technique clearly demonstrated that the mucosal innervation in HD is morphologically abnormal, and this may adversely influence secretory and absorptive functions of the bowel. WMPs using suction rectal biopsy specimens may be a useful additional technique to diagnose HD.

    Notes: Abstract Objective: What is the frequency of occupational asbestos exposure among patients suffering from malignant respiratory tumours and how many of these tumours are associated with asbestos in Hungary? Methods: An internationally established questionnaire with 29 questions, covering the most characteristic activities of asbestos exposure at the workplace was completed for 300 patients with respiratory malignancies, i.e. 297 patients with lung cancer and three with mesothelioma of the pleura. From the questionnaire, the smoking habits were estimated and cumulative asbestos exposure was assessed in fibre-years. Additionally, lung X-rays were classified and the national data on the incidence of malignant pleura mesothelioma were analysed. Results: A cumulative asbestos exposure of 25 fibre-years or more was detected in 11 patients with lung cancer (4%) and in each of the three patients with pleural mesothelioma (100%). In a further 72 patients (24%), cumulative occupational asbestos exposure was assessed as below 25 fibre-years (between 0.01 and 23.9 fibre-years). In this group, car and truck mechanics, and installation and construction workers using asbestos-cement were registered. Among patients with an asbestos exposure of 25 fibre-years or more, six asbestos-cement production workers were observed, among them the three mesothelioma cases. A weak but significant association between positive X-ray findings and exposure estimates could be demonstrated. Additionally, results of the lung tissue fibre counts by scanning transmission electron microscopy were available for 25 of the lung cancer patients. A good correlation was observed between the asbestos fibre counts and the assessment of cumulative asbestos exposure. In Hungary, 84 cases of pleural mesothelioma were registered in 1997 and 73 in 1998. These numbers correspond to an annual incidence of about one new case per 100,000 inhabitants older than 15 years. Conclusions: The annual incidence of lung cancer in Hungary is about 6,000. Since in our series of lung cancer patients about 4% were observed, which could be accepted as representing occupational disease because of a cumulative exposure to 25 fibre-years or more, the annual asbestos related lung tumour incidences may be estimated to be approximately 150 or more. The proportion of nearly two estimated cases of lung cancer per case of pleural mesothelioma corresponds to international experience. Up to now, lung cancer cases only exceptionally have been registered as occupational diseases, i.e. they were seriously under-diagnosed in Hungary. For improving this situation, diagnostic assistance by a self-interview with a questionnaire covering the working history for all newly diagnosed lung cancer patients would be helpful.

    Description / Table of Contents: Zusammenfassung Wachen Kaninchen wurde in die beiden lateralen Ohrvenen Natriumnitroprussid (SNP) und Hydroxocobalamin (HC) 4 h im Molverhältnis 1 ∶ 4, 1 ∶ 5 bzw. 1 ∶ 8 gleichzeitig infundiert. Kontrolltiere erhielten nur HC. Natriumthiosulfat (ST) wurde mit SNP im molaren Verhältnis 1 ∶ 4, 1 ∶ 5 bzw. 1 ∶ 10 infundiert. Im Blut aus einer Ohrarterie wurden pH, PCO 2 und der Basenexcess (BE) bestimmt und Leber, Herz, Niere 48 h nach Infusionsende histologisch untersucht. Die Beobachtungszeit betrug 48 h. Bei der niedrigsten Dosis von HC (1 ∶ 4) verursachte eine 1,25%ige SNP-Infusion eine schwere metabolische Acidose: Drei von zehn Tieren verstarben während der Infusion, sechs weitere innerhalb von 24 h. Das 1 ∶ 5-Verhältnis verursachte eine Reduktion der Acidose, aber drei von sieben Tieren starben innerhalb von 24 h. Die höchste Dosis (1 ∶ 8) verhinderte das Entstehen der Acidose, aber immer noch verstarben drei von acht Tieren. Unabhängig davon, ob HC allein oder mit SNP infundiert wurde, verursachte es in jeder Dosierung histologische Veränderungen in Leber, Myocard und Niere. Im Gegensatz hierzu bot ST im Mol Verhältnis 1 ∶ 5 einen kompletten Antidotschutz; eine Acidose trat nicht auf, alle Tiere überlebten. Nach Infusion aller drei Dosen waren wohl histologische Veränderungen in der Leber, nicht aber im Myocard oder der Niere vorhanden. ST allein verursachte keine histologischen Organveränderungen. Daher ist ST als Antidot geeigneter als HC und diesem zur Vermeidung von Intoxikationen mit SNP vorzuziehen.
    Notes: Abstract Sodium nitroprusside (SNP) and hydroxocobalamin (HC) — in molar ratios of 1 ∶ 4, 1 ∶ 5, and 1 ∶ 8, respectively — were infused simultaneously during 4 h into two veins of seperate ears of conscious rabbits. Controls received HC only. Sodium thiosulphate (ST) was infused with SNP at molar ratios of 1 ∶ 4, 1 ∶ 5, and 1 ∶ 10. The observation period was 48 h. With the lowest dose of HC (1 ∶ 4), SNP produced a severe metabolic acidosis; three of ten animals died during the infusion, an additional six within 24 h. When the 1 ∶ 5 ratio was administered, the acidosis was less marked, but still three of seven animals succumbed within 24 h. The highest dose (1 ∶ 8) prevented acidosis, however three of eight animals died. All doses of HC caused histological changes in the liver, the myocardium, and the kidney, independently if given alone or with SNP. In contrast to this, ST had a complete antidote effect, if administered in a 1 ∶ 5 ratio; no acidosis was demonstrable and death did not occur. In neither dosage ST could prevent histological changes in the liver, but the kidney and the heart were not affected. In contrast to HC ST alone did not cause histological alterations. Consequently, ST is the preferable antidote and is superior to HC for preventing or treating intoxications with SNP.

    Notes: Zusammenfassung Die Behandlung der krebskranken Tiere mit einem aktiven Leberpräparat (Perhepar Richter) sowie die Phosphorvergiftung der Leber vermochten die biologischen Eigenschaften der Tumoren nicht zu beeinflussen, und sie ließen somit keine direkten Beziehungen zwischen Krebs und Leber erkennen. Bezüglich ihres Stoffwechsels waren jedoch Beziehungen zwischen Krebs und Leber des kranken Tieres festzustellen, welche auf humoralem Wege zustande kommen. Die Gewebsatmung der Leber war bei den carcinomkranken Tieren herabgesetzt, unter Erhöhung der aeroben und anaeroben Glykolyse. Die Größe der Atmungsstörung war niedriger als bei der Cumarinvergiftung der Leber.
